Ileocolic Intussusception Caused by a Giant Ulcerating Lipoma of Bauhin’s Valve: an Unusual Cause of Intestinal Obstruction in the Adult

We report a case of intestinal obstruction in a 73-year-old woman caused by ileocolic intussusception. The underlying cause was a giant submucosal ulcerating lipoma (6?×?3.3?×?3.8 cm) extending from the superior mucosal lip of Bauhin’s valve. Abdominal ultrasonography showed a pathognomonic target-like mass appearing as multiple concentric rings. CT scan confirmed the diagnosis. Because of apparent obstruction, a laparotomy with right hemicolectomy and ileocolic anastomosis was performed. Lipomas most frequently occur in the cecum and ascending colon where they represent the most common submucosal mesenchymal tumor. They typically occur in elderly women with an incidence that varies from 0.15 to 0.56 %. Pain, rectal bleeding, and obstruction are typical symptoms. When faced with an intussusception in children, reduction with air per rectum can be performed. In adults, however, malignant cell spreading and seeding is of big concern. Since approximately 20–50 % of all underlying causes are malignant, explorative surgery is favored in adults.     

促甲状腺素抑制治疗甲状腺癌患者的膳食调查分析

目的 对促甲状腺素(TSH) 抑制治疗期间的分化型甲状腺癌(DTC)患者进行膳食调查，为对患者进行个性化膳食指导及营养干预研究提供参考依据。方法 从湖南省肿瘤医院甲状腺内科的门诊患者中收集行TSH抑制治疗的DTC患者，根据抑制治疗副作用危险分层标准，收集低、中、高危患者各200例，共600例患者作为研究对象。对所有入选病例进行膳食调查:一年食物摄入频率和3天24小时回顾法膳食调查。结果 TSH抑制治疗的DTC患者整体人群膳食摄入不足且膳食结构不合理；整体人群畜肉类摄入过多，鱼类及水产品、蛋、奶、豆、蔬菜、水果则摄入过少；整体人群脂类、碳水化合物、维生素E摄入量基本达到要求；蛋白质、膳食纤维、钙、铁、锌、硒、维生素A、维生素B1、维生素B2、维生素C、烟酸摄入明显不足；碘摄入基本符合要求；低、中、高危组膳食摄入不足率分别是21.0%、8.0%、11.5%；中危组畜肉类摄入最多，碘摄入有0.5%的患者超标；高危组牛奶摄入最少。结论 TSH抑制治疗的DTC患者膳食结构不合理及多种营养素摄入不足，在临床工作中应加强对整体人群的营养教育及针对性的干预，以降低TSH抑制治疗对心血管系统及影响骨代谢的风险。     

A rare case of a periorbital respiratory (choristomatous) cyst

Respiratory cysts are benign lesions lined by normal respiratory epithelium. There are few reported cases localized to the orbit, while those of the eyelid are exceedingly rare. Respiratory cysts usually arise either from a non‐hereditary congenital malformation, where they are distinguished as choristomatous, or from trauma. Here, we report a case of a 53‐year‐old man who presented with a large right lower eyelid cyst that was histopathologically diagnosed as a respiratory cyst.
