Aims: This study was designed to evaluate the effect of gap width and graft placement on bone healing around implants placed into simulated extraction sockets in the mandibles of four beagle dogs. Materials and methods: Four Ti‐Unite® implants (13 mm × 3.3 mm) were placed on each side of the mandible. Three implants were surrounded by a 1.35 mm circumferential and a 5 mm deep gap around the coronal portion of the implants. A fourth implant was inserted conventionally into both sides of the mandibles as a positive control. The gaps were filled with either Bio‐Oss®, autogenous bone or with a blood clot alone. The study design was balanced for animal, side and modality. Ground sections were prepared from biopsies taken at 3 months, and computer‐aided histometric measurements of bone/implant contact and area of bone within threads were made for the coronal 5 mm. Data were analysed using analysis of variance. Results: The mean bone/implant contact was 9.8 mm for the control and ranged from 9.3 to 11.3 mm for the three test modalities. The corresponding values for area within threads were 1 mm2 and 1–1.2 mm2. Modality had a significant effect on both bone/implant contact (F=16.9; P<0.0001) and area within threads (F=16.7; P<0.0001). Conclusion: The results of this study suggest that both autogenous bone graft and Bio‐Oss® played an important role in the amount of hard tissue fill and osseointegration occurring within marginal bone defects around implants.  相似文献

The use of odds ratio (OR) in risk analysis is considered a useful means of cross-comparing risk factors by which a disease is influenced. This article outlines the interpretation of reported ORs with respect to periodontal disease, highlighting those factors that are most deserving of consideration. The claim that periodontal disease is implicated in some serious systemic diseases is investigated with respect to the evidence. Data retrieval was carried out, focusing primarily on ORs for factors shown to be positively correlated with the incidence of periodontal disease. Using the available data, a risk evaluation scoring system was proposed (the Cronin/Stassen BEDS CHASM scale). The results of the evaluation support the view that hygiene therapy, smoking cessation and control of the glycaemic state offer the largest and most easily achieved reduction in risk with respect to attachment loss. The literature on systemic diseases indicates that the mechanisms with which periodontitis may interact seem biologically plausible, compellingly so in the cases of coronary heart disease and diabetes mellitus. However, statistical evidence to confirm these suggested interactions is equivocal, with the data retrieved in some instances being less than decisive. Further investigation is recommended.  相似文献

PURPOSE: We investigate the patterns of failure in the treatment of glioblastoma(GBM) based on clinical target volume(CTV) margin size,dose delivered to the site of initial failure,and the use of temozolomide and intensity-modulated radiotherapy(IMRT).METHODS: Between August 2000 and May 2010,161 patients with GBM were treated with radiotherapy with or without concurrent temozolomide.Patients were treated with CTV expansions that ranged from 5 to 20 mm using a shrinking field technique.Patterns of failure and time to progression and overall survival were compared based on CTV margin,use of temozolomide,and use of IMRT.Kaplan Meier analysis was used to estimate survival times,and χ test was used for comparison of cohorts.RESULTS: For patients treated with 5-,10-,and 15-to 20-mm CTV,79%,77%,and 86% experienced failures in the 60 Gy volume,respectively.Forty-eight percent,55%,and 66% of patients with 5-,10-,and 15-to 20-mm CTV experienced failures in the 46 Gy volume,respectively.There was no statistical difference between patients treated with 5-,10-,15-to 20-mm margins with regard to 60 Gy failure(P=0.76),46 Gy failure(P=0.51),or marginal failure(P=0.73).Eighty percent of patients receiving temozolomide experienced failures in the 60 Gy volume.There was no increased likelihood of marginal failures in patients receiving IMRT(P =0.97).CONCLUSIONS: Modern treatment techniques including use of concurrent temozolmide,limited CTV margin size,and IMRT have not greatly changed the patterns of failure of GBM.  相似文献

This study evaluated healing, with emphasis on root resorption, following root surface treatment with 1% aqueous stannous fluoride (SnF2), saturated citric acid (CA), or saline control (C) in conjunction with periodontal flap surgery. Supraalveolar periodontal defects were surgically created and immediately treated in the mandibular premolars in 6 beagle dogs. The defect height approximated 5 to 6 mm from the reduced alveolar bone to the cemento-enamel junction. Root treatments were rotated between experimental teeth within jaw quadrants and duplicated in left and right quadrants in the dogs. Flaps were raised to cover most of the crowns of the teeth and sutured. The dogs were sacrificed 12 weeks after surgery and tissue blocks with teeth and adjacent structures were processed for histometric analysis. SnF2-treated teeth healed with significantly longer junctional epithelium, less connective tissue repair to the root surface, and less bone regeneration than CA and C-treated teeth. New cementum formation was limited in all treatment groups. Root resorption was observed in almost all teeth exhibiting connective tissue repair, however to a lesser amount and not as frequent in SnF2 treated teeth due to limited connective tissue repair. No differences were found in amount and frequency of root resorption in CA and C-treated teeth. An inhibitory effect on root resorption of SnF2 could not be disclosed in this experiment, however, it may be concluded that CA treatment of the root surface in conjunction with reconstructive periodontal flap surgery does not seem to enhance root resorption.  相似文献

OBJECTIVES: The effect of alteration of laser parameters on laser-dentine interaction, in particular the effect of pulse duration, has not been well documented. The aim of this in vitro study was to determine the effect on dentine crater depth of Nd:YAG laser pulse duration, and total delivered energy, dentine site and the presence or absence of dye. METHODS: Ninety-six sound third molars were extracted and sectioned transversely to provide 192 upper and lower cut surfaces. The upper surfaces were painted with a layer of dye (IR5) suitable for absorption at 1064 nm. The specimens were divided into 16 sub-groups and exposed to two Nd:YAG lasers; one of pulse duration 7 ms and the second of pulse duration 35 ps. Both lasers operated in a non-contact mode (spot diameter 165 microm) with repetition rates of 10.5 and 10 Hz, respectively. Four total energies (2.28, 2.64, 3.6, 4.2 J) were delivered to eight dyed and eight undyed sub-groups. Eight outer and five inner sites were irradiated on each specimen. Dentine crater depth was measured five times using a Reflex Microscope and a three-dimensional centre of gravity derived. An upper and lower specimen were taken from each sub-group and viewed under a SEM. RESULTS: ANOVA and multiple regression analysis were applied and the following factors were found to have a statistically significant effect on crater depth (p<0.0001): total delivered energy, pulse duration and inner/outer location. CONCLUSIONS: Increasing energy and pulse duration produced deeper craters. Similarly inner dentine sites produced deeper crater depths. Only craters produced at the ms pulse duration were carbonised. It would appear that laser-dentine interaction has a non-thermal component at picosecond pulse duration.  相似文献

Recent improvements in immunosuppressive therapy have improved the survival rate of post organ transplant patients. These developments have introduced a new group of medically compromised patients to the general dental practitioner. This paper reviews the medical and dental complications of organ transplant patients with special reference to heart transplant patients. A case report of the management of Cyclosporin A induced gingival hyperplasia in a post heart transplant patient is presented. Guidelines for dental management of heart transplant patients are suggested.  相似文献

Early stages of connective tissue reattachment to surgically denuded cementum and root dentin following citric acid application were studied in fenestration wounds. Block specimens were obtained after seven, 14, and 21 days. Continuity between newly deposited collagen fibrils in the granulation tissue and unmasked dental matrix collagen had been established within seven days by interdigitation of fibrils in an up to 0.5 micron-wide zone at the cementum or dentin surface. Splicing of collagen at the fibrillar level by direct attachment to the severed end of matrix fibrils or by juxtapositioning of new and old fibrils was seen in rare instances only. At 14 and 21 days, distinct bundles of collagen fibrils inserted deeply into the orifice of dentinal tubules. Arrested and reversed surface resorption was encountered at all time points. At these sites as well, new collagen fibrils interdigitated with denuded matrix collagen, while some resorbed surfaces were characterized by absence of collagen continuity. The results lend continued support to the concept of reattachment based on interdigitation of collagen fibrils. However, since attachment is established even in the absence of this feature, a significant role for specific attachment substances not shown can also be postulated.  相似文献

STATEMENT OF PROBLEM: There are insufficient data on the accuracy of resin indexing materials for the assembly of implant prostheses in vivo. MATERIAL AND METHODS: This in vitro study assessed the fit of an implant framework with the use of 2 common acrylic resins. PURPOSE: A standardized type IV gold alloy cast framework with 3 stainless steel implant replicas secured to 3 premachined gold cylinders placed 200 mm apart using a parallelometer was fabricated on a flat type IV stone model. Wax was placed around the center of each bar section, and a silicone matrix was fabricated to enable the same quantity of material to be used for each assembly. Each bar was sectioned at the center of the matrix, and indices (n=20) were fabricated from Duralay and GC Pattern acrylic resins. The materials were allowed to polymerize for 15 minutes before the assemblies were removed from the model. Frameworks were visually assessed for fit after an additional 15 minutes, after 2 hours, and after 24 hours, using the Sheffield 1-screw test. Observational outcomes are presented indicating visually perceived fit relative to the type of indexing resin. RESULTS: Both indexing materials were judged to be visually accurate at the 15-minute time interval only. CONCLUSION: Within the limitations of this in vitro study, both resin-indexed implant assemblies were judged visually accurate for fit 15 minutes after polymerization but not at subsequent test intervals.  相似文献
