Isolation of Legionella pneumophila (serogroup one) from shower-water in Ballarat

Five non-fatal cases of Legionnaires' disease occurred during 1979 and 1980 among patients living in Ballarat. The distribution of the dates of onset was sporadic, and no patient could be considered to have been at risk from faulty air-conditioning equipment; however, two of these patients, both female, had been resident, on separate occasions, in the reception ward of a long-stay institution during the incubation period of their illnesses. A fourfold rise in antibodies to the Philadelphia strain of Legionella pneumophila (serogroup 1) was demonstrated in all five of these patients, and subsequent examination of water in the shower block of the reception ward yielded a strain of the same serotype of L. pneumophila.     

A survey of pathological changes associated with long-term high tar tobacco smoke exposure in a murine model

Balb/c mice were exposed to the fresh smoke of a daily equivalent of thirty high tar filtered cigarettes for periods of up to 95 weeks. Detailed surveys of the gross and histopathological data are presented which indicates that there is the induction or production of significant numbers of malignant tumours of several types in the animals exposed to tobacco smoke. There are also significant histopathological changes which consist mainly of interstitial pneumonia and focal low grade emphysema. These features contribute significantly to the reduction in life span of the test animals in which the main causes of death are malignancies and inflammatory diseases of the lungs.     

Parathyroid adenomas evaluated by Tl-201/Tc-99m pertechnetate subtraction scintigraphy and high-resolution ultrasonography

Thallium-201/technetium-99m pertechnetate subtraction scintigraphy of the parathyroid glands was performed in a prospective study of 33 patients who had undergone bilateral neck exploration for elevated serum calcium and serum parathyroid hormone levels. In 31 cases, the Tl-201/Tc-99m subtraction technique yielded an overall sensitivity of 81%, specificity of 99%, and accuracy of 94% for identifying solitary parathyroid adenomas. Tl-201/Tc-99m subtraction scintigraphy correctly identified 73% of parathyroid adenomas weighing less than 499 mg, 79% of those weighing 500-1,499 mg, and 100% of adenomas weighing more than 1,500 mg. In a subgroup of 24 patients with solitary parathyroid adenomas who underwent both scintigraphy and high-resolution sonography, the sensitivity, specificity, and accuracy of both procedures were similar.     

Thoracic wall involvement by Hodgkin disease and non-Hodgkin lymphoma: CT evaluation

Thoracic computed tomographic (CT) scans of 250 patients with newly diagnosed or recurrent lymphoma revealed thoracic wall involvement in 24 patients (11 with Hodgkin disease, 13 with non-Hodgkin lymphoma). Thoracic wall involvement occurred without contiguous mediastinal or parenchymal involvement in 17 patients. Of these, 13 patients had masses beneath the pectoralis muscles or within the breast, and four had masses arising from the ribs. Five additional patients had mediastinal masses with thymic involvement and parasternal extension through the thoracic wall. Pulmonary parenchymal lymphoma with thoracic wall invasion was noted in the remaining two patients. In five of nine patients receiving radiation therapy, treatment plans were modified by CT demonstration of thoracic wall lymphoma.     

Non-invasive flow estimation in an implantable rotary blood pump: a study considering non-pulsatile and pulsatile flows

Non-invasive estimation of flow was investigated in an implantable rotary blood pump (iRBP) with a hydrodynamic bearing. The effects of non-pulsatile and pulsatile flows were studied using in vitro mock loops, and acute (N = 3) and chronic (N = 6) ovine experiments. Using the non-pulsatile and pulsatile mock loops an average flow estimation algorithm was derived from root mean square (RMS) pump impeller speed and RMS input power. These algorithms were programmed into the iRBP controller for subsequent validation in vivo. In the acute experiments, venous return and systemic vascular resistance were adjusted through pharmacological intervention and exsanguination to produce an average range of pump flows from 0.0 to 2.6 l min(-1). Over this range the RMS estimation error was 88 +/- 12 ml, with a linear correlation slope of 0.992 +/- 0.006 (R2 = 0.986 +/- 0.004). In the chronic experiments, animals were monitored daily for up to three months and an average range of flows from 2.8 to 4.8 l min(-1) recorded. A linear correlation between the estimated and measured pump flows yielded a slope of 1.005 +/- 0.006 (R2 = 0.966 +/- 0.004). The RMS estimation error was 120 +/- 11 ml. Using this algorithm it is possible to effectively estimate flow in a rotary blood pump without implanting additional invasive sensors.
