The case of a 9-year-old boy with hemangioendothelioma of the liver and spleen who presented with consumptive coagulopathy one month after sustaining a blunt trauma to his abdomen is reported. A contrast enhanced computed tomography scan of the abdomen showed a ruptured spleen with multiple lesions in the liver that were enhancing with contrast. On exploration, the child was found to have splenic rupture with multiple vascular lesions of the liver. A splenectomy with liver biopsy was done. The histopathologic examination found that both the liver and spleen had a similar tumor morphology characteristic of an epithelioid and spindle cell hemangioendothelioma. OBJECTIVES: We have previously demonstrated an importance of endothelin-1 in diabetic patients undergoing bypass surgery. Recent evidence suggests that cardiomyocytes might also produce endothelin-1, which might directly impair myocyte contractility by increasing intracellular calcium levels. Because hyperglycemia is a potent stimulus of endothelin-1 production, we hypothesized that increased production, action, or both of endothelin-1 might be a mediator of direct cardiomyocyte injury in diabetes. Therefore we studied the effects of endothelin receptor blockers (BQ-123 and bosentan) on hyperglycemia-induced endothelin-1 production and cellular injury after ischemia-reperfusion. METHODS: Using a human ventricular heart cell model of simulated ischemia-reperfusion, we studied the effects of normoglycemia (5 mmol/L, 48 hours) and hyperglycemia (25 mmol/L, 48 hours) on cellular injury and endothelin-1 production. Furthermore, the effects of selective endothelin-A and mixed endothelin-A/B receptor antagonism (with BQ-123 and bosentan, respectively) were evaluated. RESULTS: Cellular injury, as assessed by means of trypan blue uptake, was higher in human ventricular heart cells subjected to hyperglycemia and simulated ischemia-reperfusion injury (P =.01); this effect was prevented with both BQ-123 and bosentan (P =.01). In addition, heart cells from the hyperglycemic group elaborated more endothelin-1 after ischemia-reperfusion (P =.02). CONCLUSIONS: Endothelin-1 production and cellular injury were greater in human ventricular heart cells subjected to hyperglycemic conditions and simulated ischemia-reperfusion. These effects are mediated by endothelin-A receptors because both BQ-123 and bosentan exerted similar degrees of protection. Endothelin receptor blockade is a novel strategy to improve the resistance of the diabetic heart to cardioplegic arrest and reperfusion. 相似文献
BACKGROUND: Motor vehicle collisions that are caused by large animals affect motorists in most parts of the world and tend to be increasing in incidence. Although traffic accidents involving kangaroos are common in Australia, we are not aware of any series that reported human injuries caused by such accidents. We aimed to study the mechanism, type, and outcome of these injuries. METHODS: Forty-six patients (32 male and 14 female patients; median age, 31.5 years) who presented to Royal Perth Hospital and who had been involved in a motor vehicle collision that was related to kangaroos between July 1994 and June 2000 were studied. RESULTS: The patients had a median Injury Severity Score of 9 (range, 1-41); 67.4% were car collisions and 32.6% were motorbike collisions, 41.3% had direct collision with a kangaroo, 34.8% hit a secondary object, and 32.6% rolled over. Most of these injuries affected the head and face (54.3%), upper extremities (57.4%), and lower extremities (40.4%). Only four had intracranial injuries (8.7%). Ninety percent of these collisions occurred at night, 74% in the countryside and 85% on highways or streets. The incidence was reduced during winter. Only one patient in this series died (2.2%). CONCLUSION: Most kangaroo-related motor vehicle collisions occurred at night, in the countryside, and on highways when the driver tried to avoid a kangaroo. This study aimed to examine the attitudes of intensivists and haematologists to the use of blood and blood products using a scenario-based postal questionnaire. One hundred and sixty-two intensivists and 77 haematologists responded to the survey. In four scenarios, the baseline haemoglobin thresholds for red cell transfusion ranged from 6 to 12 g.dl(-1). There was significant variation between scenarios (p <0.005). Increasing age, high Acute Physiology and Chronic Health Status II score, surgery, acute respiratory distress syndrome, septic shock and lactic acidosis significantly (p <0.005) modified the transfusion threshold. There were greater variations in the baseline threshold for platelet transfusion. The majority of respondents (72.3%) selected a baseline haemoglobin threshold between 9 and 10 g.dl(-1). The thresholds for platelet transfusion were far less consistent. 相似文献
BACKGROUND: Although stentless aortic bioprostheses are believed to offer improved outcomes, hemodynamic benefits remain unsubstantiated. METHODS: Fifty-three patients were randomized to receive the stented C-E pericardial valve (CE) and 46 patients the Toronto Stentless Porcine valve (SPV). Annuli were sized for the optimal insertion of both valve types, such that surgeons were required to commit to specific valve sizes before randomization. Echocardiographic measurements and functional status (Duke Activity Status Index) were assessed at 3 and 12 months postoperatively. RESULTS: Although cardiopulmonary bypass times (CE: 118.6+/-36.3 minutes; SPV: 148.5+/-30.9 minutes; p = 0.0001) and aortic cross-clamp times (CE: 95.4+/-28.6 minutes; SPV: 123.6+/-24.1 minutes; p = 0.0001) were significantly prolonged in the SPV group, perioperative morbidity and mortality was similar between groups. Neither valve offered a superior internal diameter for any given annular diameter (mean decrease in left ventricular outflow tract diameter after valvular implantation: SPV: 3.4+/-1.11 mm versus CE: 3.7+/-1.33 mm; p = 0.25). Although labeled mean valve size was significantly larger in the SPV group, the actual mean valve size based on internal valvular diameter was no different between groups (CE: 21.9+/-2.0 mm; SPV: 22.3+/-2.0 mm; p = 0.286). Although effective orifice areas increased, and mean and peak transvalvular gradients decreased in both groups over time, no differences were demonstrated between groups at 12 months. Similarly, although significant regression of left ventricular mass was accomplished in both groups over time, no differences were demonstrated between groups. Finally, Duke Activity Status Index scores of functional status improved in both groups over time; however, no differences were noted between groups at 12 months postoperatively. CONCLUSIONS: Although offering excellent outcomes, stentless valves did not demonstrate superior hemodynamic indices in comparison to stented valves up to 12 months after implantation. 相似文献
Background. The use of two internal mammary artery grafts in coronary artery bypass grafting has been associated with decreased risks of death, reoperation, and angioplasty. However, bilateral internal mammary artery takedown is associated with higher incidence of sternal wound infection, particularly in people with diabetes and in elderly and obese patients. This study was conducted to explore the feasibility of using right internal mammary artery (RIMA) and radial artery (RA) as a composite graft while preserving the distal two thirds of the RIMA to leave the sternal blood supply intact.
Methods. Eighteen patients underwent coronary artery bypass grafting using proximal RIMA and RA composite graft as one of the bypass conduits. The distal two thirds of the RIMA was left intact to preserve sternal blood supply. The graft-free flows of the RIMA and RA composite graft and of the left internal mammary artery graft and the length of the composite graft had been measured. The graft patency and the flow in the distal part of the unharvested RIMA was evaluated postoperatively 2 weeks after the procedure. In 6 of these patients the graft patency was evaluated by selective angiography.
Results. There was no hospital mortality or incidence of perioperative myocardial infarction. None of the patients needed intraaortic balloon pump support postoperatively. There was no sternal wound infection. The vessels grafted were distal right coronary artery (n = 7), posterior descending artery (n = 8), obtuse marginal branches (n = 3), and posterolateral ventricular branch (n = 1); 1 patient received the composite graft as a sequential graft to the posterior descending artery and posterolateral left ventricular branches. The mean graft-free flow of the RIMA and RA composite graft was 98.06 ± 16.93 mL/min compared to left internal mammary artery flows of 55.80 ± 8.99 mL/min. All 16 patients who had a good echo window showed patent grafts when evaluated by two-dimensional echocardiography and color Doppler echocardiography. All of the 6 patients in whom the angiogram was repeated postoperatively showed patent RIMA and RA grafts.
Conclusions. Myocardial revascularization using proximal RIMA and RA in situ pedicle graft was safe in patients with diabetes and in obese and chronic obstructive pulmonary disease patients. This graft was useful to revascularize posterior descending artery, posterolateral ventricular branches of right coronary artery, and obtuse marginal branches where a left internal mammary artery and RA composite graft cannot be used because of technical reasons.
