Hepatic fibrogenesis is reduced in the absence of leptin. We hypothesized that leptin protects hepatic stellate cells (HSCs) from apoptosis and tested this in in vitro and in vivo systems. (i) Fas ligand (fas-L)-mediated apoptosis was induced in vitro in activated HSCs in the absence and presence of leptin. (ii) HSC apoptosis was also induced by UV irradiation in the absence and presence of leptin. (iii) Fas-L-mediated apoptosis was induced in vitro in HSCs from db/db mice in the absence and presence of leptin. (iv) Liver fibrosis was induced in wt and db/db mice. (v) Liver fibrosis was induced in wild-type mice with TAA, and mice received additional leptin or a control solution. HSC apoptosis was assessed by TUNEL staining. Western blot for α-SMA was used to determine differences in HSC activation. Results were as follows. (i) Fas-L induced significant apoptosis of HSC, and preincubation with leptin reduced this approximately threefold. (ii) Leptin provided no protection from UV-induced apoptosis. (iii) HSCs from db/db mice were not protected by leptin against fas-L-induced apoptosis. (iv) TAA-induced fibrosis was significantly less in db/db mice compared to wild type. (v) Wild-type mice receiving leptin had less apoptosis and more α-SMA than controls. We conclude that leptin protects HSC from in vitro and in vivo apoptosis. The antiapoptotic effect of leptin requires the long form of the leptin receptor and interacts with the apoptotic pathway proximal to mitochondrial activation.  相似文献

Aims

We compared predicted life‐time health‐care costs for current, never and ex‐smokers in Germany under the current set of tobacco control polices. We compared these economic consequences of the current situation with an alternative in which Germany were to implement more comprehensive tobacco control policies consistent with the World Health Organization (WHO) Framework Convention for Tobacco Control (FCTC) guidelines.

Design

German EstSmoke, an adapted version of the UK EstSmoke simulation model, applies the Markov modelling approach. Transition probabilities for (re‐)currence of smoking‐related diseases were calculated from large German disease‐specific registries and the German Health Update (GEDA 2010). Estimations of both health‐care costs and effect sizes of smoking cessation policies were taken from recent German studies and discounted at 3.5%/year.

Life‐time cost and outcomes in current, never and ex‐smokers.

Findings

If tobacco control policies are not strengthened, the German smoking population will incur €41.56 billion life‐time excess costs compared with never smokers. Implementing tobacco control policies consistent with WHO FCTC guidelines would reduce the difference of life‐time costs between current smokers and ex‐smokers by at least €1.7 billion.

Conclusions

Modelling suggests that the life‐time healthcare costs of people in Germany who smoke are substantially greater than those of people who have never smoked. However, more comprehensive tobacco control policies could reduce health‐care expenditures for current smokers by at least 4%.  相似文献

Corneal surface laser ablation procedures for the correction of refractive error have enjoyed a resurgence of interest, especially in patients with a possible increased risk of complications after lamellar surgery. Improvements in the understanding of corneal biomechanical changes, the modulation of wound healing, laser technology including ablation profiles and different methods for epithelial removal have widened the scope for surface ablation. Phytochemical investigation of Leonurus japonicus has led to the isolation of a labdane diterpene derivative, 15,16-epoxy-3α-hydroxylabda-8,13(16),14-trien-7-one (1), which was tested for its in vitro anti-inflammatory effects. The results demonstrated that 1 exhibits an inhibitory effect on LPS-stimulated RAW 264.7 macrophages. The anti-inflammatory action shown by 1 suppressed LPS-induced NF-κB activation, resulting in the down-regulation of iNOS and COX-2 protein expression, attributable to the inhibitory action of LPS-induced NO and PGE(2) production. Compound 1 inhibited LPS-induced phosphorylation and the degradation of inhibitory kappa B (IκBα) and decreased the nuclear translocation of p50 and p65. In addition, 1 exhibited an inhibitory effect on LPS-induced NF-κB-DNA and AP-1-DNA binding activity, using an electrophoretic mobility shift assay with NF-κB- and AP-1-specific (32)P-labeled probes. The LPS-induced mitogen-activated protein kinases (p-JNK, p-p38, and p-ERK) and p-Akt were inhibited after 30 and 10 min of LPS stimulation, respectively. The optimal dose of iron in ready-to-use therapeutic foods (RUTF) used to treat uncomplicated severe acute malnutrition (SAM) in community settings is not well established. The objective of this systematic review was to assess if an increased iron dose in RUTF, compared with the standard iron dose in the World Health Organization (WHO)-recommended peanut-based RUTF, improved outcomes in children aged six months or older. We searched multiple electronic databases and only included randomized controlled trials. We pooled the data in a meta-analysis to obtain relative risk (RR) and reported it with a 95% confidence interval (CI). Three studies, one each from Zambia, the Democratic Republic of Congo, and Malawi, were included. In all studies, the RUTF used in the intervention group was milk-free soya–maize–sorghum-based RUTF. The pooled results showed that, compared to the control group, a high iron content in RUTF may lead to increase in hemoglobin concentration (mean difference 0.33 g/dL, 95% CI: 0.02, 0.64, two studies, certainty of evidence: low) and a decrease in any anemia (RR 0.66, 95% CI: 0.48, 0.91, two studies, certainty of evidence: low), but also decrease recovery rates (RR 0.91, 95% CI: 0.84, 0.99, three studies, certainty of evidence: low) and increase mortality (RR 1.30, 95% CI: 0.87, 1.95, three studies, certainty of evidence: moderate). However, the CIs were imprecise for the latter outcome. Future studies with large sample sizes are needed to confirm the beneficial versus harmful effects of high iron content in RUTF in treating uncomplicated SAM in children aged 6-59 months in community settings.  相似文献

Copolymerization of ethylene (E) with 5-vinyl-2-norbornene (VNB) catalyzed by ansa-metallocenes allows the precise control of essential polymeric properties such as comonomer incorporation, molecular weight (Mw), and polydispersity (Đ). Significant efforts have been devoted to synthesizing and developing novel catalysts, cocatalysts, and activators, although the fundamental elements of catalytic processes remain unclear. For example, it is questionable how polymeric catalysts are divided across dormant and active sites and how this distribution affects the order of monomers for the propagation rate, which widely vary in the literature. Furthermore, although the empirical correlation between the monomers and average Mw has been established in many systems, the fundamental processes of chain termination remain unknown. Furthermore, the involvement of ion-pairing in metallocene-catalyzed polymerization and the termination mechanisms are also contentious issues. In this study, we describe the use of a quenched-labeling technique based on acyl chloride to selectively quench the zirconium metal–polymeric bond, which can be used to study the kinetics, active site [Zr][C*] counting, copolymer microstructure, and molecular weight distribution (MWD) to determine the rate laws for chain initiation, chain propagation rate (Rp), propagation rate constant (kp) and chain termination. In addition, we also predict previously unknown chemical characteristics of E/bicyclic copolymerization processes, where either a cis-endocyclic double bond with steric properties or a vinyl exocyclic double bond affects the activity, i.e., [Zr]/[*C], (Rp) and (kp). All these properties require the implementation of a particular kinetic mechanism that assumes the low activity of the building copolymer chains incorporating a single ethylene/VNB unit, i.e., the Cp2Zr–C2H5 group, in the ethylene addition process in the Cp2Zr–C bond. Due to β-agostic stabilization, the Cp2Zr–C2H5 group exhibits a distinct feature. These effects were confirmed experimentally, such as the E/VNB co-polymer activity and VNB mol%, propagation rate decrease in the polymerization time (tp) of 120 s to 1800 s, crystalline properties, and significant increase in molecular weight. The active center [Zr]/[*C] fraction considerably increased in the initial (tp) 840 s, and subsequently tended to the steady stage of 33%, which is lower than previously reported E homo- and E/P copolymerization. The lower [C*]/[Zr] in both the early and stable stages, decrease in VNB mol%, and Rp with tp can be associated with the more significant fraction of Cp2Zr–CH2CH3-type dormant site by the β-agostic hydrogen interaction with the Cp2Zr metal. The tpversus RpE, RpVNB, kpE, kpVNB, and [Zr]/[C*] count could be fitted to a model that invokes deactivation of the growing polymer chains. In the case of the thermal behavior of the copolymers (melting temperature (Tm) and crystalline temperature (ΔHm)), Tm varied from 101 °C to 121 °C, while ΔHm varied from 9 to 16 (J g−1).

PURPOSE: To show that the limited quality of surfaces produced by one model of excimer laser systems can degrade visual performance with a polymethylmethacrylate (PMMA) model. METHODS: A range of lenses of different powers was ablated in PMMA sheets using five DOS-based Nidek EC-5000 laser systems (Nidek Technologies, Gamagori, Japan) from different clinics. Surface quality was objectively assessed using profilometry. Contrast sensitivity and visual acuity were measured through the lenses when their powers were neutralized with suitable spectacle trial lenses. RESULTS: Average surface roughness was found to increase with lens power, roughness values being higher for negative lenses than for positive lenses. Losses in visual contrast sensitivity and acuity measured in two subjects were found to follow a similar pattern. Findings are similar to those previously published with other excimer laser systems. CONCLUSIONS: Levels of surface roughness produced by some laser systems may be sufficient to degrade visual performance under some circumstances.  相似文献
