Pancoast's syndrome (superior pulmonary sulcus tumor): review of the literature

Pancoast's syndrome is produced by an apical lung tumor, with a local extensión to inferior brachial plexus, paravertebral sympathetic chain, vertebral bodies and first, second and third ribs. Its major cause is the non-small cell lung cancer, and this syndrome may produce shoulder pain and Horner's syndrome. The best diagnostic method is transthoracic needle aspiration, because of its peripheral location. Neoadjuvant chemoradiotherapy followed by complete surgical excision is the preferred approach to these tumors.     

Effect of Cryopreservation on Semen Sample

There are varied indications of preserving a semen sample for utilization at a later day. However, the freeze-thaw cycle is bound to impart deleterious effect on the fertilizing potential of the sample. 60 semen samples divided into three groups were preserved in this study. On subsequent thawing, after three months it was seen that the yield varied between 66% to 72%. The post thaw percent yield bears a linear relationship with the pre-freeze quality. 5 pregnancies were achieved in the study testifying the preservation of fertilizing ability of the thawed samples. Sub normal semen samples which might be otherwise acceptable in a therapeutic insemination, do not withstand freezing.Key Words: Cryopreservation, Semen     

Effect of short-term Psychiatric Intervention in Cancer Patients

A total of 50 patients undergoing cancer treatment at Malignant Disease Treatment Centre were included in the present study aimed at evaluating the psychological status of cancer patients. All patients filled a specially designed proforma and the following psychological questionnaires : General Health Questionnaire, Carroll Rating Scale for Depression, State-Trait Anxiety Inventory, PGI General Well-being Scale and Quality of Life Scale. Analysis of the results showed that 22 (44%) of the cancer patients had psychiatric disorders and this number had reduced to 12 (24%) after therapy. The difference was statistically significant. Psychiatric treatment also resulted in a statistically significant reduction in level of depression as measured by Carroll Rating Scale for depression. Short term psychiatric treatment was found to be very useful in treating psychiatric morbidity and depression in cancer patients.Key Words: Cancer, Depression, Psychiatric morbidity     

Laparoscopic Management of Adnexal Masses

Retrospective study of 121 cases of adnexal masses which were managed laparoscopically was carried out. The aim of study was to evaluate the safety and effectiveness of laparoscopic management of adnexal masses. In 120 cases, procedure was completed safely with minimum morbidity. In one case laparotomy had to be done to complete the procedure. In 76 cases cystectomy was done, 26 required salpingo-oophorectomy and 19 required only salpingectomy. Histologic evaluation revealed 30 functional cysts, 36 endometriotic cysts, 11 dermoids, 9 serous cystadenomas, 3 mucinous cystadenomas, 11 parovarian cysts, 19 cases of hydrosalpinx and 2 cases of tuberculosis.Key Words: Adnexal mass, Laparoscopy, Ovarian cyst     

ABC transporters and drug resistance in parasitic protozoa

Parasitic protozoa are responsible for a wide spectrum of diseases in humans and domestic animals. The main line of defence available against these organisms is chemotherapy. However, the application of chemotherapeutic drugs has resulted in the development of resistance mechanisms, which limit the number of antiprotozoal drugs that are effective in the treatment and control of parasitic diseases. Knowledge about the resistance mechanisms involved may allow the development of new drugs that minimise or circumvent drug resistance or may identify new targets for drug development. This review focuses on the role of protozoal ATP-binding cassette (ABC) transporters in drug resistance. These membrane proteins mediate the ATP-dependent transport of a wide variety of chemotherapeutic drugs away from their targets inside the parasites. The genome sequence of Plasmodium falciparum and Plasmodium yoelii has recently been completed, and the sequencing of other parasitic genomes are now underway. As a result, many new membrane transporters belonging to the ABC superfamily are being discovered. We review the ABC transporters in major parasitic protozoa, including Plasmodium, Leishmania, Trypanosoma and Entamoeba species. Transporters with an established role in drug resistance have been emphasised, but newly discovered transporters with a significant amino acid sequence identity to established ABC drug transporters have also been included.     

Use of pulse oximeter for determining distal perfusion in neonates &; infants with suspected vascular compromise

Conclusion The use of a pulse oximeter, which is readily available in the ICU, can be of immense benefit to determine the status of distal limb perfusion in neonates and children who are suspected of having a vascular compromise, and the pulsations cannot be appreciated due to the presence of limb edema. Limb warmth and capillary filling are the primary indicators of vascularity but may be misleading in some patients. The presence of poor quality tracings or absent tracings is a definite indicator of significant ischemia. The use of a pulse oximeter, for determination of oxygen saturation, was declared by Severinghaus and Astrup to be “arguably the most significant technologic advance ever made in monitoring the well being of patients during anaesthesia, recovery and critical care. Perhaps the use of the pulse oximeter for determination of distal perfusion can further augment the statement. However we urge them to continue their study and obtain meaningful data, with proper statistical analysis. For the time being these observations may be considered preliminary.

Dentigerous cyst - a cyst of dental origin, though common in lower jaw is relatively uncommon in upper jaw. Here a 14 years, F, presented with huge swelling of unilateral maxilla and clinicoradiologically diagnosed as dentigerous cyst is being reported along with brief review of literature.
