It is uncertain whether patients with nonalcoholic fatty liver disease (NAFLD) and normal alanine aminotransferase (ALT) have a milder disease and should undergo liver biopsy. We reviewed the histological data of 458 Italian patients with NAFLD in whom liver biopsy was indicated by altered liver enzymes (395 cases, 86%), or persistently elevated ferritin or long-lasting severe steatosis (63 cases). Factors associated with nonalcoholic steatohepatitis (NASH) and fibrosis >/= 2 were identified by multivariate analysis. Patients with normal ALT were significantly older, had lower body mass index, fasting triglycerides, insulin resistance according to homeostasis model assessment (HOMA-IR), ALT, and gamma-glutamyltransferase, but a higher prevalence of hypertension. NASH was diagnosed in 59% and 74% of the patients with normal and increased ALT, respectively (P = 0.01). In the overall series of patients, NASH was independently predicted by ALT (odds ratio [OR], 1.11; 95% confidence interval [CI], 1.04-1.19 per 10-IU/mL increase) and diabetes (OR, 1.5; 95% CI, 1.1-2.0). The same variables were selected in patients with increased ALT, whereas in those with normal ALT, HOMA-IR and ALT were independent predictors. Severe fibrosis was independently predicted by serum ferritin (OR, 1.04; 95% CI, 1.001-1.08 per 50-ng/mL increase), ALT (OR, 1.07; 95% CI, 1.02-1.14), and diabetes (OR, 1.8; 95% CI, 1.4-2.3) in the overall series, serum ferritin and diabetes in those with increased ALT, and only HOMA-IR (OR, 1.97; 95% CI, 1.2-3.7) in patients with normal ALT. CONCLUSION: Normal ALT is not a valuable criterion to exclude patients from liver biopsy. Alterations in glucose metabolism and insulin resistance in subjects with normal ALT should also be considered in the selection of NAFLD cases for histological assessment of disease severity and progression. 相似文献

Fast axonal transport is characterized by the bidirectional, microtubule-based movement of membranous organelles. Cytoplasmic dynein is necessary but not sufficient for retrograde transport directed from the synapse to the cell body. Dynactin is a heteromultimeric protein complex, enriched in neurons, that binds to both microtubules and cytoplasmic dynein. To determine whether dynactin is required for retrograde axonal transport, we examined the effects of anti-dynactin antibodies on organelle transport in extruded axoplasm. Treatment of axoplasm with antibodies to the p150Glued subunit of dynactin resulted in a significant decrease in the velocity of microtubule-based organelle transport, with many organelles bound along microtubules. We examined the molecular mechanism of the observed inhibition of motility, and we demonstrated that antibodies to p150Glued disrupted the binding of cytoplasmic dynein to dynactin and also inhibited the association of cytoplasmic dynein with organelles. In contrast, the anti-p150Glued antibodies had no effect on the binding of dynactin to microtubules nor on cytoplasmic dynein-driven microtubule gliding. These results indicate that the interaction between cytoplasmic dynein and the dynactin complex is required for the axonal transport of membrane-bound vesicles and support the hypothesis that dynactin may function as a link between the organelle, the microtubule, and cytoplasmic dynein during vesicle transport. 相似文献

The interplay between a prosthetic and tissue represents an important factor for the fixation of orthopedic implants. Laser texturing tests and electropolishing were performed on two materials used in the fabrication of medical devices, i.e., CoCr and Ti6Al4V-ELI alloys. The material surface was textured with a diode-pumped solid state (DPSS) laser and its effect on the surface quality and material modification, under different combinations of laser power and marking speed, were investigated. Our results indicate that an increment of energy per unit length causes an incremental trend in surface roughness parameters. Additionally, phase transformation on the surface of both alloys was achieved. Chemical analysis by energy dispersive X-ray spectrometer (EDX) shows the formation of (Co(Cr,Mo)) phase and the M23C6 precipitate on the CoCr surface; while quantitative analysis of the X-ray diffractometer (XRD) results demonstrates the oxidation of the Ti alloy with the formation of Ti2O and Ti6O from the reduction of the α-Ti phase. The behaviors were both related with an increase of the energy per unit length. Control of the final surface roughness was achieved by an electropolishing post-treatment, minimizing the as-treated values. After polishing, a reduction of surface roughness parameters was obtained in a range between 3% and 44%, while no changes in chemical composition or present phases were observed. 相似文献

Cell replication integrates aberrations of cell cycle regulation and diverse upstream pathways which all can contribute to melanoma development and progression. In this study, cell cycle regulatory proteins were detected in situ in benign and malignant melanocytic tumors to allow correlation of major cell cycle fractions (G1, S-G2, and G2-M) with melanoma evolution. Dysplastic nevi expressed early cell cycle markers (cyclin D1 and cyclin-dependent kinase 2; Cdk2) significantly more (p?<?0.05) than common nevi. Post-G1 phase markers such as cyclin A, geminin, topoisomerase IIα (peaking at S-G2) and aurora kinase B (peaking at G2-M) were expressed in thin (≤1 mm) melanomas but not in dysplastic nevi, suggesting that dysplastic melanocytes engaged in the cell cycle do not complete replication and remain arrested in G1 phase. In malignant melanomas, the expression of general and post-G1 phase markers correlated well with each other implying negligible cell cycle arrest. Post-G1 phase markers and Ki67 but none of the early markers cyclin D1, Cdk2 or minichromosome maintenance protein 6 (Mcm6) were expressed significantly more often in thick (>1 mm) than in thin melanomas. Marker expression did not differ between metastatic melanomas and thick melanomas, with the exception of aurora kinase A of which the expression was higher in metastatic melanomas. Combined detection of cyclin A (post-G1 phase) with Mcm6 (replication licensing) and Ki67 correctly classified thin melanomas and dysplastic nevi in 95.9 % of the original samples and in 93.2 % of cross-validated grouped cases at 89.5 % sensitivity and 92.6 % specificity. Therefore, cell cycle phase marker detection can indicate malignancy in early melanocytic lesions and accelerated cell cycle progression during vertical melanoma growth. 相似文献

This study provides histochemical data of the hepatopancreatic cells of adult female pink-shrimp (Farfantepenaeus brasiliensis) at two different developmental stages (those with developed gonads and those with exhausted gonads). The F. brasiliensis females were collected in seawater off the Guarapari coast, Espirito Santo, Brazil. Five cell types were identified in this digestive gland: B (vesicular), E (embryonic), F (fibrillar), M (basal) and R (resorptive). The digestive gland was stained with the following techniques: PAS/Alcian blue (for polysaccharides), bromophenol blue (for protein), von Kossa (for bound calcium) and Baker (for lipids). Acid glycoconjugates were found inside vacuoles in the R cells, while neutral polysaccharides were present in the B cells and near to the microvilli. In females with exhausted gonads polysaccharides were also seen in the intertubular spaces and inside the lumina of the tubules. The F and M cells were the most marked by the presence of large amounts of proteins observed in R cells and also inside the vacuoles of B cells. The bound calcium was mainly found in the F and M cells. The F cells showed strong positive staining for lipid while the R cell only stained weakly. The E cells did not react to any of the applied staining techniques. The similarities in the histochemical composition of these hepatopancreatic cells in females with developed gonads, compared to exhausted ones, is justified by the fact that transfer of these elements to the oocytes occurs, in significant quantity, only during the initial stages of gonadal development in F. brasiliensis.
