Cranial computed tomography in the diagnosis of systemic lupus erythematosus.

The cranial computed tomograms of 29 patients with systemic lupus erythematosus (SLE) were reviewed. Twenty-two patients had a clinical course consistent with central nervous system involvement. Of these, 20 had abnormal CT studies during the course of their CNS symptoms. The most common finding was sulcal enlargement, either with or without ventricular enlargement, and it was prominent in patients with either psychosis or dementia. Infarcts and intracranial hemorrhages were seen as well. Seven CT studies were obtained in SLE patients without a clear diagnosis of CNS involvement. Only one of these was abnormal.     

Potential future application with therapeutic agents

Several new approaches to radiation therapy with radionuclides have been discussed. Iron 55 is selectively utilized in the red cell developmental cycle and in therapeutic doses, can lower marrow and circulating erythrocyte levels with much smaller degrees of effect on other cell lines. A serious complication, noted in animal studies, is the induction of neoplasma, especially osteosarcoma. Selective irradiation of the cell nucleus is possible with 125IUdR. This results in highly efficient cell killing due to the highly concentrated region of ionization. High concentrations of densely ionizing radiation in the malignant cell may also be accomplished with 211At. The use of labeled liposomes is an additional approach to the delivery of intracellular irradiation. None of these approaches is applicable for the practical treatment of human malignancy at the present time. The importance of these approaches is their value as models for future development of methods that can provide highly selective radiation to target sites.     

Carcinogenicity and teratogenicity vs. psychogenicity: psychological characteristics associated with self-reported Agent Orange exposure among Vietnam combat veterans who seek treatment for substance abuse

This study asked, "What are the psychological characteristics of Vietnam combat veterans who claim Agent Orange exposure when compared with combat-experienced cohorts who do not report such contamination?" The question was researched among 153 heroin addicts, polydrug abusers, and chronic alcoholics who were seeking treatment: 58 reported moderate to high defoliant exposure while in combat; 95 reported minimal to no exposure while in Vietnam. The null hypothesis was accepted for measures of childhood and present family social climate, premilitary backgrounds, reasons for seeking treatment, patterns and types of illicit drug and alcohol use, interpersonal problems, intellectual functioning, and short-term memory. The null hypothesis was rejected for personality differences, however, those who self-reported high Agent Orange exposure scored significantly higher on MMPI scales F, Hypochondriasis, Depression, Paranoia, Psychasthenia, Schizophrenia, Mania, and Social interoversion. The results suggest that clinicians carefully assess attributional processing of those who report traumatic experience.
