ABSTRACTObjectives: An Electroencephalogram (EEG) is the result of co-operative actions performed by brain cells. In other words, it can be defined as the time course of extracellular field potentials that are generated due to the synchronous action of cells. It is widely used for the analysis and diagnosis of several conditions. But this clinical data use to be multi-dimensional, context-dependent, complex, and it causes a concoction of various computing related research challenges. The objective of this study was to develop a computer-aided diagnosis system for epilepsy detection using EEG signals to ease the diagnosis process.Materials: In this study, EEG datasets for epilepsy disease detection were taken from a public domain (Bonn University, Germany). These EEG recordings contain 100 single-channel EEG signals with maximum duration of 23.6 seconds. This data set was recorded intra-cranially and extra-cranially with the help of a 128-channel amplifier system using a common reference point.Results: For a unique set of EEG signal features, the Optimized Artificial Neural Network model for classification and validation was developed with optimum neurons in the hidden layer. Results were tested on the basis of accuracy, sensitivity, precision, and specificity for all classes. The proposed Particle Swarm Optimized Artificial Neural Network provided 99.3% accuracy for EEG signal classification.Discussion: Our results indicate that artificial neural network has efficiency to provide higher accuracy for epilepsy detection if the statistical features are extracted carefully. Gliomas are recognized as a heterogeneous group of neoplasms differing in their location and morphological features. These differences, between and within varying grades of gliomas, have not been explained solely on the grounds of an oncogenic stimulus. Interactions with the tumor microenvironment as well as inherent characteristics of the cell of origin are likely a source of this heterogeneity. There is an ongoing debate over the cell of origin of gliomas, where some suggest a progenitor, while others argue for a stem cell origin. Thus, it is presumed that neurogenic regions of the brain such as the subventricular zone (SVZ) containing large numbers of neural stem and progenitor populations are more susceptible to transformation. Our studies demonstrate that K‐rasG12D cooperates with the loss of p53 to induce gliomas from both the SVZ and cortical region, suggesting that cells in the SVZ are not uniquely gliomagenic. Using combinations of doxycycline‐inducible K‐rasG12D and p53 loss, we show that tumors induced by the cooperative actions of these genes remain dependent on active K‐ras expression, as deinduction of K‐rasG12D leads to complete tumor regression despite absence of p53. These results suggest that the interplay between specific combinations of genetic alterations and susceptible cell types, rather than the site of origin, are important determinates of gliomagenesis. Additionally, this model supports the view that, although several genetic events may be necessary to confer traits associated with oncogenic transformation, inactivation of a single oncogenic partner can undermine tumor maintenance, leading to regression and disease remission. The traditional way to tackle Zenker’s diverticulum (ZD) has been flexible endoscopic septum division (FESD). Recently, the concept of per oral endoscopic myotomy has been found useful for managing diverticular diseases of the esophagus and has been termed DPOEM. We performed a systematic review and meta-analysis to evaluate the efficacy and safety of D-POEM in diverticular disease of the esophagus and to compare it with FESD. We systematically searched PubMed and Embase, for studies reporting clinical success, technical success and adverse events in D-POEM alone or D-POEM comparing with FESD. We computed pooled prevalence for D-POEM alone and risk ratio for D-POEM vs FESD using random effect method with inverse variance approach. Subgroup analysis for ZD, non-ZD and mixed diverticulum was conducted. Totally 19 studies including 341 patients were identified reporting on D-POEM. The pooled clinical, technical success and adverse event rates for D-POEM were 87.07%, 95.19% and 10.22%, respectively. The clinical success was significantly better than FESD while the technical success, adverse event rate, procedure time and length of hospital stay were comparable with FESD. The recurrence rate was negligible for D-POEM compared to FESD. On subgroup analysis by dividing into three groups of ZD, non-ZD and mixed, there was no difference between clinical, technical success and adverse event rate among the three groups. D-POEM is an effective and safe technique among both ZD and non-ZD patients and has better clinical success than FESD.
Cholangiograms of 15 patients with stenosis of distal portion of common bile duct due to chronic alcoholic pancreatitis were critically evaluated for stricture shape, length, and maximal diameter of the common bile duct. In these cholangiograms, different shapes of strictured segments were present, including the smooth tapering variety in eight, "funnel shape" in three, and "rat tail," "bent knee," "hourglass," and complete obstruction in one case each. The length of strictured segment ranged from 16 to 57 mm with a mean (+/- S.E.) of 40.7 +/- 4.1 mm. The mean (+/- S.E.) maximal diameter of the dilated portion of common bile duct was 18.9 +/- 1.3 mm with a range of 13-28 mm. There is a wide variety in the shape and length of the stenotic segments of the distal common bile duct in chronic alcoholic pancreatitis. Moreover, a particular radiologic configuration of common bile duct stricture is not helpful in predicting the nature of the underlying process in this group of patients. 相似文献
Japanese encephalitis is a serious public health problem with significant mortality in children and old people. It occurs throughout much of Asia (43 000 cases worldwide per year). In recent years it has caused many epidemics in different parts of the country. In view of the high mortality and severe sequelae which often leaves behind highly dependent and disabled survivors, the disease is assuming great importance.
