学术背景:造血干细胞移植技术在临床上已得到广泛应用,移植后感染是关系到移植成败的重要因素。目的:探讨造血干细胞移植后各阶段的感染的特点、预防及治疗,以进一步减低造血干细胞移植后感染的发生率及死亡率。检索策略:由作者应用计算机检Medline 1994-01/2007-05关于造血干细胞移植及移植后感染的文章,检索词为"hematopoietic stem cell transplantation,infection",限定语言种类为"English";同时检索中国期刊全文数据库1994-01/2007-05相关文章,检索词"造血干细胞移植,感染,防治",限定语言种类为中文。纳入标准:随机对照研究;实验或临床研究包含平行对照组。排除标准:重复性研究文献评价:初检得到212篇文献,初审后选取与造血干细胞移植后感染有关的文章126篇,删除明显无关及相关性不强的文章,进一步查找全文,29个实验符合标准,予以纳入。29个研究包括324例患者和140个实验动物,分别阐述了造血干细胞移植后感染的原因、途径、特点、种类及各种感染的预防及治疗措施。资料综合:造血干细胞移植后感染发病隐匿,由于患者免疫力低下,感染不易控制,在不同阶段致病菌的种类有所不同,细菌感染普遍,在移植后各个时期均可出现,真菌感染和病毒感染致死性强,故预防和治疗感染至关重要,其治疗分为预防治疗、抢先治疗、经验性治疗和针对治疗。结论:造血干细胞移植后感染的治疗已成为影响移植疗效的一个重要原因,及早的诊断及正确的治疗将成为移植后感染治疗成功的关键。  相似文献

The ability of a number of prediction systems was examined todetermine how well they could predict Salmonella mutagenicity.Theprediction systems included two computer-based systems (CASE0and TOPKAT0), the measurement of a physiochemical parameter(ke) and the use of structural alerts by an expert chemist.The computer based systems operators and the chemist were suppliedwith the structures of 100 chemicals that had been tested formutagenicity in the Salmonella test; the actual chemicals wereneeded for the physiochemical measurement. None of the participantswas provided with the chemical names or Salmonella test resultsprior to submitting their predictions. BackgroundAccurate assessment of asthma control may help predict future asthma exacerbations.ObjectiveTo evaluate asthma guidelines impairment domain components as predictors of exacerbations in severe/difficult-to-treat asthma.MethodsChildren (aged 6–11 years; n = 289) and adolescents/adults (aged ≥12 years; n = 2,094) with complete baseline and 12-month data from The Epidemiology and Natural History of Asthma Outcomes and Treatment Regimens study were included. Asthma was categorized as very poorly controlled, not well-controlled, and well-controlled using impairment domain components. Effects of omitting each component on very poorly controlled and not well controlled groups were examined. Multivariable logistic regression determined the relationship of components in predicting asthma exacerbations.ResultsOmission of individual impairment domain components led to misclassification of asthma control in 11% to 39% of patients. A baseline exacerbation was the strongest independent predictor of exacerbation at month 12 in children (odds ratio = 2.94; P < .001) and adolescents/adults (odds ratio = 2.93; P < .001). In children, very poorly controlled asthma-based short-acting β2-agonist use was associated with a 2-fold higher exacerbation risk (odds ratio = 2.03; P = .011). In adolescents/adults, not well controlled or very poorly controlled asthma based on short-acting β2-agonist use (odds ratio = 1.49), lung function (odds ratio = 1.66), and the Asthma Therapy Assessment Questionnaire (odds ratio = 1.94) were also independent predictors of exacerbations (P < .001).ConclusionsAlthough the combined use of individual components of the impairment domain increases the sensitivity of identifying patients at high risk for future asthma exacerbations, specific components may be more important than others in severe/difficult-to-treat asthma. Prior exacerbations, short-acting β2-agonist use, lung function, and (in adolescents/adults) the Asthma Therapy Assessment Questionnaire were independent predictors of exacerbations.  相似文献

BACKGROUND: Air-pollution levels have been shown to be associated with increased morbidity of respiratory diseases. METHODS: Data for ambient air-pollutant levels, meteorologic factors, and hospitalization or emergency room (ER) visits for acute asthma in Singapore children over a 5-year period (1990-4) were obtained and analyzed for associations by time-series methods. RESULTS: Throughout this period, the annual mean and 24-h mean levels for sulfur dioxide (SO2), nitrogen dioxide (NO2), and total suspended particles (TSP) and maximum 1-h daily average for ozone were generally within the air-quality guidelines established by the World Health Organization (WHO). However, positive correlation between levels of each of these pollutants and daily ER visits for asthma was observed in children aged 3-12 years, but not among adolescents and young adults (13-21 years old). The association with SO2 and TSP persisted after standardization for meteorologic and temporal variables. An adjusted increase in 2.9 ER visits for every 20 microg/m3 increase in atmospheric SO2 levels, lagged by 1 day, was observed on days when levels were above 68 microg/m3. With TSP, an adjusted increase of 5.80 ER visits for every 20 microg/m3 increase in its daily atmospheric levels, lagged by 1 day, was observed on days with levels above 73 microg/m3. Similar results were also obtained after controlling for autocorrelation by time-series analysis. CONCLUSIONS: These associations were observed even though the overall levels of all pollutants were generally within the air-quality guidelines established by the WHO. These findings suggest that asthmatic children are susceptible to increased levels of air pollutants, particularly SO2 and TSP, although the ambient levels are generally within "acceptable" ranges.  相似文献

Nanocomposites of cross-linked methacrylate polymers with silver nanoparticles have been synthesized by coupling photoinitiated free radical polymerization of dimethacrylates with in situ silver ion reduction. A polymerizable methacrylate bearing a secondary amino functional group was used to increase the solubility of the silver salt in the hydrophobic resin system. Fourier transform infrared spectroscopy (FTIR) revealed that the silver ion reduction had no significant effect on the degree of vinyl conversion of the methacrylate. X-ray photoelectron spectroscopy (XPS) measurements showed an increased silver concentration at the composite surface compared to the expected concentration based on the total amount of silver salt added. Furthermore, the surface silver concentration leveled off when the silver salt mass fractions were 0.08% or greater. Composites with low concentrations of silver salt (< 0.08% by mass) exhibited comparable mechanical properties to those containing no silver. Transmission electron microscopy (TEM) confirmed that the silver nanoparticles formed within the polymer matrix were nanocrystalline in nature and primarily ≈ 3 nm in diameter, with some large particle aggregates. Composites containing silver nanoparticles were shown to reduce bacterial colonization with as little as 0.03% (by mass) silver salt, while additional amounts of silver salt did not further decrease their surface colonization. With a substantial effect on bacterial growth and minimal effects on mechanical properties, the in situ formation of silver nanoparticles within methacrylate materials is a promising technique for synthesizing antibacterial nanocomposites for biomedical applications.  相似文献

ObjectiveTo investigate the calcium and phosphorus contents of four domestic animals used as sources of meat in Nigeria.MethodsThe calcium contents of the body parts of the animals were determined using atomic absorption spectrometer. Their phosphorus contents were determined colorimetrically using the molybdenum blue method.ResultsThe calcium and phosphorus contents were significantly higher in the bone samples than in the other animal parts investigated (P<0.05). The calcium contents of all the edible parts were higher in chicken than in the other animals. High calcium and phosphorus contents were detected in the faeces of chicken and goat, and that of cattle and goat, respectively. Low calcium and phosphorus contents were detected in the urine samples. Calcium: phosphorus ratios calculated for the bones of chicken, cattle and goat were satisfactory.ConclusionsIn conclusion, calcium and phosphorus contents of the animal parts vary significantly and their relative contents may be related to the animal's diet. Chicken parts may be the preferred dietary source of these minerals. This study highlights the need for routine investigation of the mineral contents of food, which is necessary for proper nutritional guidelines.  相似文献

BACKGROUND: Ingestion of small amounts of an offending food can elicit adverse reactions in individuals with IgE-mediated food allergies. The threshold dose for provocation of such reactions is often considered to be zero. However, because of various practical limitations in food production and processing, foods may occasionally contain trace residues of the offending food. Are these very low, residual quantities hazardous to allergic consumers? How much of the offending food is too much? Very little quantitative information exists to allow any risk assessments to be conducted by the food industry. OBJECTIVE: We sought to determine whether the quality and quantity of existing clinical data on threshold doses for commonly allergenic foods were sufficient to allow consensus to be reached on establishment of threshold doses for specific foods. METHODS: In September 1999, 12 clinical allergists and other interested parties were invited to participate in a roundtable conference to share existing data on threshold doses and to discuss clinical approaches that would allow the acquisition of that information. RESULTS: Considerable data were identified in clinical files relating to the threshold doses for peanut, cows' milk, and egg; limited data were available for other foods, such as fish and mustard. CONCLUSIONS: Because these data were often obtained by means of different protocols, the estimation of a threshold dose was very difficult. Development of a standardized protocol for clinical experiments to allow determination of the threshold dose is needed.  相似文献

Aspergillus terreus (A. terreus) is of serious concern because of a high propensity to dissemination and in vitro and in vivo resistance to Amphotericin B (AmB). The underlying molecular mechanism of AmB is not known yet and here we want to explore whether fungal heat shock protein 90 (HSP90) is involved in polyene resistance in A. terreus. AmB-susceptible (ATS) and AmB-resistant (ATR) A. terreus and AmB-susceptible Aspergillus fumigatus (AFS) were investigated in response to AmB with a special focus on HSP90. HSP90 inhibitors resulted in significant improvement of AmB activity against ATR as minimum inhibitory concentrations (MIC) decreased from 32 to 0.38 mg/L. Gene expression profiling showed a greater basal amount of HSP90 levels in ATR and ATS when compared with AFS. HSP90 blockers in combination with AmB were evaluated in a murine model of disseminated aspergillosis. HSP90 inhibitors were not beneficial for mice infected with ATR, and neither mono- nor combination treatment with AmB yielded clinical improvement. HSP90 inhibition with 17-allylamino-17-demethoxygeldanamycin (17-AAG) was harmful. HSP90 seems to play a vital role in antifungal stress response in all aspergilli tested, whereas HSP90 does not substantiate the origin of AmB resistance in ATR.  相似文献
