目的探寻一种简便的恶性肿瘤初筛自查方法。方法将人体肿瘤分为三类:体表肿瘤约占15%;空腔脏器肿瘤占65%;深层实体脏器肿瘤20%。通过科普宣传教育,让群众学会自查体表部位肿瘤,每年用自查盒助查各空腔脏器有无微量出血,隐血阳性时,去医院进一步精查。北京部分大学选40~70岁居民1万人,分成试验组、对照组各5千人。结果试验四年后:试验组共查出79例癌,年平均癌检出率482.5人/10万,癌死亡率为12.2/10万;对照组癌死亡率206.2人/10万,两组有显著性差异。在癌症高发区,同时作扩大普查试验:广东四会市肿瘤所,在门诊普查1669人,检出25例癌,鼻咽癌占24例。1999~2000年,江苏省食管胃癌高发区,普查近8万人,检出480例癌。加上“九五”以前的普查统计,用秦氏自查盒已筛查431075人,检出1272例癌,癌前病变1万多例。结论试验证明该方案设计合理,在当前是一种最简便的恶性肿瘤初筛自查方法。 相似文献

金属蛋白酶解离素28(a disintegrin and metallo2 proteinase 28,ADAM28)是ADAM家族最近发现的一种分泌性糖蛋白,它同样具有ADAM家族的生理功能:参与细胞增殖,分化,胞外基质重建,裂解基质蛋白和细胞迁移等。研究显示,ADAM28基因参与了牙胚的发育过程,并在牙源性间充质细胞的增殖、分化和凋亡中发挥重要的调控作用。同时ADAM28基因具有金属蛋白酶活性,因而它对于细胞外基质的形成和蛋白的外功能区脱落有重要的调节作用。本文着重回顾了近年来国内外关于ADAM28生物学功能的研究及其在牙齿发育中的作用机制。 相似文献

OBJECTIVE: The aims of this study were to investigate the colonisation of Streptococcus mutans and to determine the possibility of horizontal transmission in Chinese nursery children. DESIGN: Fifty-six children aged between 3 and 4 years old from a nursery were included in this study. Twenty-four children were from the day and night nursery class, the others were from the day nursery class. Dental plaque samples were collected with sterile toothpicks and cultured on S. mutans-selective tryptone-yeast-cystine agar supplemented with 0.2U/ml bacitracin and 15% sucrose. The typical isolates of S. mutans were identified by classical microbiological methods and genotyped by arbitrarily primed PCR (AP-PCR) and restriction fragment length polymorphism analysis. RESULTS: S. mutans was isolated from 41 of the 56 children. The prevalence of S. mutans was higher in the children from day nursery class, compared with the children from day and night nursery class (P<0.05). A total of 140 S. mutans isolates from 41 children were analysed by AP-PCR, and 88 different amplitypes were identified. Of 41 children with S. mutans isolates, 82.9% carried two or more genotypes. Two genotypes of S. mutans were repeatedly isolated among 13 children in the day and night nursery class, and one genotype was isolated from two children in the day nursery class. CONCLUSION: The presence of matching genotypes of S. mutans among children attending the same nursery suggests the possibility of horizontal transmission. 相似文献

目的:分离培养来源于人成体牙髓组织的干细胞(dental pulp stem cells,DPSC),分析研究其在未分化状态及在矿化诱导液中基因表型的变化:方法:采用胶原酶消化法获得人牙髓组织的单细胞悬液,14 d后挑取细胞克隆,分别在普通培养基和矿化诱导培养基中进行培养,利用RT-PCR,图像分析检测细胞基因表型变化。结果。未诱导的DPSC不表达牙本质特异性蛋白(dentin phosphprotein,DSPP)及骨涎蛋白(Bone sialoprotein,BSP),表达部分成骨(osteocalcin,OCN alkaline phsphatase,ALP)相关表型,低表达转录因子核心结合因子(Cbfa-1);与末诱导的DPSCs相比较,在成骨诱导中DPSCs表达DSPP和BSP,骨相关基因表达也相应上调。结论:DPsCs在矿化诱导液的作用下向成牙本质样细胞分化,其过程有可能是通过谱系特异表犁的表达或表达上调实现的。 相似文献

Maleic acid has been used as an etchant or non-rinse conditioner in adhesive dentistry. However, the inherent mechanisms of the interaction of maleic acid with hydroxyapatite/enamel have never been fully elucidated. The purpose of this study was to provide evidence for the chemisorption of maleic acid onto hydroxyapatite/enamel, and to identify the reaction products obtained following the interaction of maleic acid with hydroxyapatite. Hydroxyapatite particles were dissolved in a 15% (w/v) aqueous solution of maleic acid (pH = 0.98). Half of the solution was dried to obtain a desiccated mixture. This mixture, hydroxyapatite, maleic acid and self-prepared calcium maleate were analysed by X-ray diffraction (XRD). Acetone was added to the other half of the solution to obtain a precipitate. This precipitate, hydroxyapatite, maleic acid, unetched enamel and maleic acid-etched enamel were analysed by X-ray photoelectron spectroscopy (XPS). The precipitate was also analysed by (1)H NMR. A new binding energy, indicating carboxylate groups, was detected by XPS on the precipitate and maleic acid-etched enamel surface. XRD data indicated the formation of calcium maleate and calcium hydrogen phosphate after the reaction. NMR data revealed that one carboxylic group of maleic acid reacted with hydroxyapatite. Hence, maleic acid can chemisorb to hydroxyapatite and enamel via ionic interactions. 相似文献

The purpose of this study was to investigate the effect of image superimposition from surrounding alveolar bone and intraradicular contrast medium on radiographic interpretation of a C-shaped canal system in mandibular second molars. Thirty mandibular second molars with fused roots were collected and stored in 10% formalin solution. After being scanned by microcomputed tomography and reconstructed, the buccal-lingual radiographs of each tooth were taken with or without intraradicular contrast medium and mandibular bony plates. The number of mesial and distal canals in the coronal-to-middle canal portion and the canal number in the apical 4-mm area were recorded. The buccal-lingual and three-dimensional view of reconstructed canal images were used to evaluate the accuracy of radiograph reading. The results were subjected to the Friedman and chi-square test. The results showed that without intraradicular contrast medium, the bone superimposition could decrease the accuracy of canal recognition significantly (p < 0.001), especially in the apical area. When contrast medium was in canals, the canal reading seemed not to be affected by the bone superimposition (p > 0.05). Based on these findings, the intraradicular contrast medium could increase the accuracy of canal recognition of a C-shaped canal system in mandibular second molars despite the bone image superimposition, especially in the detection of continuous C-shaped canals and the canals in the apical area. 相似文献

Introduction
The purpose of this study was to visualize the stresses and strain distribution patterns in ProTaper Universal F2 files (Dentsply Maillefer, Ballaigues, Switzerland) and to establish the stress- and strain-curvature relationship for this instrument under various conditions by using a dynamic, three-dimensional finite-element model.Methods
An accurate geometric model of a ProTaper Universal F2 instrument was created. Two short, straight tubes were also modeled to represent the parts of root canal apical and coronal to the curvature. Then, the file was constrained to a curve of varying degree, curve length, and position. The maximum von Mises stress and strain on the tension side of the instrument was measured at 5-degree intervals in a numerical simulation package (LS-DYNA; Livermore Software Technology, Livermore, CA).Results
The mechanical performance of the ProTaper F2 file under various conditions was simulated. A long curvature length produced lower values of stress and strain under the same angle of curvature. An increase in the curvature angle generally induces higher stress and strain. For the same degree and curve length, the stress and strain increased if the curved portion was situated further up the shaft of the instrument (ie, with a larger diameter).Conclusions
The dynamic, numerical model may be used to evaluate and compare the effect of various root canal curvatures on the behavior of different designs of root canal instrument. A new framework design is presented for resin-bonded, acid-etched, fixed partial dentures. This innovative design divided the restoration into two parts, each with a separate path of insertion. The preparation of abutment teeth required minimal reduction and the pontic could not be displaced during function.
