Disinhibition is a strong correlate of alcohol use, yet limited alcohol research has examined the facets of this personality construct. Recent work suggests that sensation seeking and impulsivity show differential relations with alcohol outcomes, indicating unique mechanisms of risks associated with each of these dimensions of disinhibition. The goal of the study was to examine sensation seeking and impulsivity as unique predictors of alcohol use and problems, and to test a broad range of drinking motives as potential mediators of these relations. Self-reported data from college students (N=310) were utilized for the study. Results suggested that sensation seeking and impulsivity were associated with alcohol use and problems through different mediational pathways. There was some evidence for gender moderating these pathways. The findings indicate that alcohol prevention and intervention programs should be tailored to specifically target individuals elevated on impulsivity versus sensation seeking.  相似文献

Despite the success of the aromatase inhibitors (AIs) in treating estrogen receptor positive breast cancer, 15-20 % of patients receiving adjuvant AIs will relapse within 5-10 years of treatment initiation. Long-term estrogen deprivation (LTED) of breast cancer cells in culture mimics AI-induced estrogen depletion to dissect mechanisms of AI resistance. However, we hypothesized that a subset of patients receiving AI therapy may maintain low circulating concentrations of estrogens that influence the development of endocrine resistance. We expanded established LTED models to account for incomplete suppression of estrogen synthesis during AI therapy. MCF-7 cells were grown in medium with charcoal-stripped serum supplemented with defined concentrations of 17β-estradiol (E2) or the estrogenic androgen metabolite 5α-androstane-3β,17β-diol (3βAdiol), an endogenous selective estrogen receptor modulator. Cells were selected in concentrations of E2 or 3βAdiol that induce 10 or 90 percent of maximal proliferation (EC(10) and EC(90), respectively), or estrogen deprived. Estrogen independence was evaluated during selection by assessing cell growth in the absence or presence of E2 or 3βAdiol. Following >7 months of selection, estrogen independence developed in estrogen-deprived cells and EC(10)-selected cells. Functional analyses demonstrated that estrogen-deprived and EC(10)-selected cells developed estrogen independence via unique mechanisms, ERα-independent and dependent, respectively. Estrogen-independent proliferation in EC(10)-selected cells could be blocked by kinase inhibitors. However, these cells were resistant to kinase inhibition in the presence of low steroid concentrations. These data demonstrate that further understanding of the total estrogen environment in patients on AI therapy who experience recurrence is necessary to effectively treat endocrine-resistant disease.  相似文献

This article presents a synopsis of the current data on the mechanisms of blood--brain barrier (BBB) alteration and autoimmune response in acute ischemic stroke. Most researchers confirm the relationship between the severity of immunobiochemical changes and clinical outcome of acute ischemic stroke. Ischemic stroke is accompanied by aseptic inflammation, which alters the brain tissue and exposes the co-stimulatory molecules of the immune system and the neuronal antigens. To date, BBB is not considered the border between the immune system and central nervous system, and the local immune subsystems are found within and behind the BBB. BBB disruption contributes to the leakage of brain autoantigens and induction of secondary autoimmune response to neuronal antigens and long-term inflammation. Glymphatic system function is altered and jeopardized both in hemorrhagic and ischemic stroke types. The receptors of innate immunity (toll-like receptor-2 and toll-like receptor-4) are also involved in acute ischemia--reperfusion injury. Immune response is related to the key processes of blood clotting and fibrinolysis. At the same time, the stroke-induced immune activation may promote reparation phenomena in the brain. Subsequent research on the reduction of the acute ischemic brain injury through the target regulation of the immune response is promising.  相似文献

Purpose/aim:Spatial neglect is caused by damage to neural networks critical for spatial attention. Spatial neglect without proper treatment impedes rehabilitation outcomes. Prism adaptation treatment, a visuomotor protocol, has been used with stroke survivors with spatial neglect to improve function. This case report explored the feasibility and potential effects of prism adaptation treatment in an individual with spatial neglect after glioblastoma removal.MethodsFeasibility was designed and tested to include acceptability and implementation. Exploratory aim on the effectiveness of the intervention was determined using the Catherine Bergego Scale via the Kessler Foundation Neglect Assessment Process, star cancellation, line bisection and scene copying.ResultsThe patient reported favouring the treatment. Eight sessions, one session a day, were completed over two weeks. The patient’s spatial neglect symptoms reduced on all assessments.ConclusionPrism adaptation treatment was feasible and effective; however, further research is needed to understand the complete benefits of prism adaptation treatment in this population.  相似文献

The aim of this study is to establish factors affecting total number of imaging studies performed for acute cholecystitis (AC) prior to surgery. The study included subjects with cholecystectomy and pathologic diagnosis of AC 1/1/05–1/1/14 and imaging studies (computed tomography (CT), ultrasound (US), and/or cholescintigraphy) within 7 days of surgery. The subjects were separated into groups based on modality of the first study. For each subject, report of the first study was reviewed for report’s confidence in diagnosis of AC (scored 1–5 on Likert scale: 5?=?definitely AC, 1?=?definitely no AC), recommendation of additional study, clinical history concerning for AC (history of right upper quadrant pain, cholelithiasis, and/or “rule out AC”). There were 219, 339, and 38 subjects in CT, US, and cholescintigraphy groups, respectively, with mean confidence scores of 3.7 (±1.2), 3.7 (±1.1), and 4.7 (±0.9), respectively (p?<?0.001). Prior to surgery, only one study was performed in 21.9 % (48/219) of CT group, 70.2 % (238/339) of US group, and 71.1 % (27/38) of cholescintigraphy group (p?<?0.0001). Compared to the US group, the odds of undergoing additional study were 11.8 times higher (p?<?0.001) in CT group and 1.7 times higher (p?=?0.229) in cholescintigraphy group, adjusting for age, sex, time interval between first study and the surgery, confidence score, recommendation of follow-up study, and clinical history concerning for AC. Patients with AC and CT as the first study are more likely to undergo additional imaging studies prior to surgery as compared to US or cholescintigraphy.  相似文献

MEAP, the moving ensemble analysis pipeline, is a new open‐source tool designed to perform multisubject preprocessing and analysis of cardiovascular data, including electrocardiogram (ECG), impedance cardiogram (ICG), and continuous blood pressure (BP). In addition to traditional ensemble averaging, MEAP implements a moving ensemble averaging method that allows for the continuous estimation of indices related to cardiovascular state, including cardiac output, preejection period, heart rate variability, and total peripheral resistance, among others. Here, we define the moving ensemble technique mathematically, highlighting its differences from fixed‐window ensemble averaging. We describe MEAP's interface and features for signal processing, artifact correction, and cardiovascular‐based fMRI analysis. We demonstrate the accuracy of MEAP's novel B point detection algorithm on a large collection of hand‐labeled ICG waveforms. As a proof of concept, two subjects completed a series of four physical and cognitive tasks (cold pressor, Valsalva maneuver, video game, random dot kinetogram) on 3 separate days while ECG, ICG, and BP were recorded. Critically, the moving ensemble method reliably captures the rapid cyclical cardiovascular changes related to the baroreflex during the Valsalva maneuver and the classic cold pressor response. Cardiovascular measures were seen to vary considerably within repetitions of the same cognitive task for each individual, suggesting that a carefully designed paradigm could be used to capture fast‐acting event‐related changes in cardiovascular state.  相似文献
