Probiotic bacteria are proposed to alleviate intestinal inflammation in infants with atopic eczema/dermatitis syndrome (AEDS) and food allergy. In such infants we investigated effects of probiotic bacteria on faecal IgA, and on the intestinal inflammation markers tumour necrosis factor-alpha (TNF-alpha), alpha1-antitrypsin (AT), and eosinophil cationic protein (ECP). A total of 230 infants with AEDS and suspected cow's milk allergy (CMA) received in a randomized double-blinded manner, concomitant with elimination diet, Lactobacillus GG (LGG), a mixture of four probiotic strains (MIX), or placebo for 4 wk. Four weeks after treatment, CMA was diagnosed with a double-blind placebo-controlled milk challenge. Faecal samples of 102 infants, randomly chosen for analysis, were collected before treatment, after 4-wk treatment, and on the first day of milk challenge. After treatment, IgA levels tended to be higher in probiotic groups than in the placebo group (LGG vs. placebo, p=0.064; MIX vs. placebo, p=0.064), and AT decreased in the LGG group, but not in other treatment groups. After challenge in IgE-associated CMA infants, faecal IgA was higher for LGG than for placebo (p=0.014), and TNF-alpha was lower for LGG than for placebo, but non-significantly (p=0.111). In conclusion, 4-wk treatment with LGG may alleviate intestinal inflammation in infants with AEDS and CMA. 相似文献

The hph-1 ENU-mutant mouse provides a model of tetrahydrobiopterin deficiency for studying hyperphenylalaninaemia, dopa-response dystonia, and vascular dysfunction. We have successively localized the hph-1 mutation to a congenic interval of 1.6-2.8 Mb, containing the GCH gene encoding GTP cyclohydrolase I (GTP-CH I). We used these data to establish a PCR method for genotyping wild type, hph-1 and heterozygote mice, and found that heterozygote animals have partial tetrahydrobiopterin deficiency. The selenium concentration of maternal and umbilical cord whole blood was determined by atomic absorption spectrophotometry in 21 parturients at term. Six placental and amniotic membrane tissue specimens were also investigated. The mean selenium concentrations in the maternal (0.73 +/- 0.15 mumol/l) and umbilical cord blood (0.77 +/- 0.18 mumol/l) were similar and without significant correlation. Placental (2.24 +/- 0.20 mumol/kg wet weight) and amniotic membrane tissue specimens (2.32 +/- 0.54 mumol/kg wet weight) also contained similar concentrations of selenium which were about 3 times higher than those in the maternal and umbilical cord blood. Low whole blood selenium concentration in Finnish parturients may be a sign of deficient nutritional intake of selenium during pregnancy. The relatively high concentration of selenium in the placenta and amniotic membranes on the other hand suggest that metabolically active organs are being provided primarily with this essential trace element. 相似文献

Background Irritable bowel syndrome is the most common diagnosis in gastroenterology. Trials suggest certain probiotics to be beneficial. Aim To investigate the effects of multispecies probiotic supplementation (Lactobacillus rhamnosus GG, L. rhamnosus Lc705, Propionibacterium freudenreichii ssp. shermanii JS and Bifidobacterium animalis ssp. lactis Bb12) on abdominal symptoms, quality of life, intestinal microbiota and inflammatory markers in irritable bowel syndrome. Methods Eighty‐six irritable bowel syndrome patients (Rome II criteria) participated in this randomized, placebo‐controlled 5‐month intervention. Patients were randomized to receive daily either multispecies probiotic supplementation or placebo. Irritable bowel syndrome symptoms, quality of life, microarray‐based intestinal microbiota stability (n = 20), serum cytokines and sensitive C‐reactive protein were monitored. Results The composite irritable bowel syndrome score had at 5 months decreased 14 points (95% CI: ?19 to ?9) from baseline with the multispecies probiotic vs. three points (95% CI: ?8 to 1) with placebo (P = 0.0083). Especially, distension and abdominal pain were affected. A stabilization of the microbiota was observed, as the microbiota similarity index increased with the probiotic supplementation (1.9 ± 3.1), while it decreased with placebo (?2.9 ± 1.7). No differences were seen in C‐reactive protein. Conclusions This multispecies probiotic seems to be an effective and safe option to alleviate symptoms of irritable bowel syndrome, and to stabilize the intestinal microbiota. 相似文献

Ethylene oxide (EtO) is widely used by the health industry for sterilizing heat-sensitive devices. About 100 000 workers are regularly exposed to this chemical, used in an estimated 10 000 sterilizing units in U.S. health care facilities. Based on recent findings. NIOSh has recommended that EtO be regarded as a potential carcinogen and has urged OSHA to reexamine its present standard of 50 ppm. The ACGIH Notice of Intended Changes for 1982 has proposed reducing the recommended limit from 10 ppm TWA to 1 ppm and considers EtO a suspect carcinogen for man. Because of this increasing concern for its toxicity, it is imperative that the exposures to EtO be assessed, and that all reasonable steps to reduce these exposures be taken. This study demonstrates the effects of local exhaust ventilation, sterilizer chamber temperature, and sterilizer relative humidity in relation to their influence on EtO plume directionality, concentration and duration. Using two Foxboro/Wilks Miran 1A General Purpose Gas Analyzers, EtO concentrations were simultaneously monitored both immediately above and below the sterilizer door during various process runs, at three different types of sterilizers. Sterilizer operating conditions were representative of temperatures (38-54 degrees C (100-130 degrees F)), and relative humidities (30-50%) commonly employed in the sterilization process. The effects of varying local exhaust parameters were observed. Results clearly demonstrated that EtO tends to flow upward when the sterilizer door is opened. In all cases, the highest concentrations were observed at the upper location. Local exhaust ventilation significantly reduced the concentration of EtO observed and lessened the duration for which these levels persisted. Changes in temperature and relative humidity, within the range cited, have not shown any significant differences in EtO evolution patterns, concentrations or duration. 相似文献

Background
High dietary sugar intake may compromise protein and micronutrient intakes in people with low energy intakes. The results of micronutrient dilution studies in older people have been few and conflicting. We examined the nutritional status and nutrient intakes associated with nonmilk extrinsic sugars (NMES) intakes in older people representing a broad spectrum of both healthy and vulnerable older populations.Design and participants
This cross-sectional study combined five Finnish data sets covering home-dwelling (n = 526) and institutionalized (n = 374) older people. Their nutritional status was assessed using Mini Nutritional Assessment (MNA) and nutrient intakes retrieved from 1- to 3-day food records. The participants were divided into quartiles corresponding to the proportions of energy received from NMES. Energy, nutrient, and fiber intakes were classified according to the NMES quartiles, and the participants were divided according to their places of residence (home, institution).Results
High NMES intakes were associated with older age, female sex, poor cognition, low MNA scores, immobility, and institutionalization. In all, 90% of the participants in the highest NMES quartile (Q4) were institutionalized. In the institutionalized individuals, low protein and micronutrient intakes were observed in both those with low energy intake (Q1) and in those with very high NMES intakes (Q4). In home-dwelling individuals, the nutrient intakes tended to decline linearly with increasing NMES intakes in protein and most micronutrients.Conclusions
Institutionalized older people consumed diets high in NMES, compared with those living at home, and their low energy and high NMES intakes were associated with low protein and micronutrient intakes. 相似文献29.

The purpose of this investigation was to evaluate the efficacy and toxicity of 6 months' treatment with the combination of epirubicin and docetaxel in metastatic breast cancer. Thirty-eight women (mean age 51 years, range 35-72) with metastatic breast cancer were treated with a regimen of epirubicin 75 mg/m and docetaxel 75 mg/m every 3 weeks, given 4 times if progression was seen upon evaluation after 4 courses or 8 times in responding/stable patients. The patients received 285 cycles of combination treatment and two treatments with docetaxel or epirubicin alone. When neutropenia with fever was observed, further cycles were given with dose reduction. The median cumulative docetaxel dose was 462 mg/m (range 199-600) and that of epirubicin 476 mg/m (range 199-740). The overall response rate was 54% (95% CI 37-71), with a median duration of response of 14.8 months (95% CI 8.8-27.8). Median time to progression was 12 months, median survival 26 months. Neutropenia below 0.5 x 10 /l occurred following 113 (39%) of the total of 285 cycles given; 21 patients (55%) were hospitalized for febrile neutropenia. We conclude that dose tailoring is required in treatment with an epirubicin and docetaxel regimen to avoid grade 3/4 adverse effects in a significant number of patients treated for metastatic breast cancer. 相似文献
