BackgroundWear cannot be completely prevented after total hip arthroplasty. If severe polyethylene (PE) liner wear develops, the so-called catastrophic failure occurs and metallosis develops. We postulated that longevity of the new implant may be affected after revision surgery for metallosis following a catastrophic failure of a PE liner due to the substantial amount of PE wear particles and infiltration of the metal particles in this catastrophic condition.MethodsTwenty-three hips of 23 patients were identified because they showed metallosis during revision total hip arthroplasties performed in Seoul National University Hospital between January 1996 and August 2004. They were followed for at least 6.5 years after the index revision total hip arthroplasty. The clinical and radiological results of revision total hip arthroplasties in these patients were evaluated.ResultsThe median Harris hip score increased from 60 points before revision total hip arthroplasties to 90 points at the final follow-up. Osteolysis was detected at an average of 9.3 years after revision total hip arthroplasties in 13 hips and acetabular cup loosening at average 9.8 years after revision total hip arthroplasties in 9 hips. With radiographic evidence of osteolysis and loosening as the end points, the 15-year survival rates were 28.2% and 56.0%, respectively.ConclusionsThe survival rate of revision total hip arthroplasty in patients with metallosis following a catastrophic failure of a PE liner was low.  相似文献

OBJECTIVE: This study tested the hypothesis that triangulation of two anterior vertebral screws without penetration of the cortex offers more resistance to pullout than two screws placed in parallel and penetrated. DESIGN: The pullout strength for two parallel or two triangulated anterior vertebral screws fixation, with a uni-cortical or bi-cortical purchase, were tested and compared to the strength of a single-screw fixation with a bi-cortical purchase. Four porcine spines (six months old) were used for biomechanical test and bone mineral density was measured for each specimen before testing. BACKGROUND: The potential hazards from penetration by anterior vertebral cortex screws including neurovascular and organs injuries are well documented. However, bi-cortical screw penetration is widely recognized as necessary for good anterior spinal stabilization. The authors are not aware of any biomechanical study on the anterior placement of triangulated vertebral screws without penetration and its effect on the fixation strength of anterior vertebral device remains unclear. METHODS: In this study five modes of screw fixations in lateral vertebral bodies were performed: Group A, triangulated screws with one screw penetration; Group B, triangulated screws without penetration; Group C, parallel penetrating screws; Group D, parallel nonpenetrating screws; and Group E, a single-screw with bi-cortical purchase. Biomechanical analysis with a material testing system machine was performed to determine the pull out strength of each configuration. RESULTS: The results showed that the pullout strength in the various double-screw fixation modes were statistically increased as compared to that of the single-screw with bi-cortical purchase mode. There existed statistical differences (P<0.05) between Groups A and B, Groups C and D and Groups D and E, respectively. However, no significant difference was found between Groups B and C (P=0.144). CONCLUSIONS: Based on the current data, triangulation of two anterior vertebral screws without penetration of the cortex (Group B) achieved pullout strengths similar to that of two-parallel double-cortical screws (Group C). The authors believe that this is an attractive alternative in anterior spinal instrumentation avoiding the potential risks of cortical penetration. However, in the event of pullout failure, the triangulation configuration will produce a more disastrous consequence. RELEVANCE: Triangulation of two anterior vertebral screws without penetration of the cortex achieve pullout strengths similar to that of two-parallel double-cortical screws. This is an attractive alternative in anterior spinal instrumentation that avoids the potential risks of cortical penetration.  相似文献

Fibroblast growth factor (FGF) signaling has been implicated in the pathogenesis of pulmonary fibrosis. Mice lacking FGF2 have increased mortality and impaired epithelial recovery after bleomycin exposure, supporting a protective or reparative function following lung injury. To determine whether FGF2 overexpression reduces bleomycin‐induced injury, we developed an inducible genetic system to express FGF2 in type II pneumocytes. Double‐transgenic (DTG) mice with doxycycline‐inducible overexpression of human FGF2 (SPC‐rtTA;TRE‐hFGF2) or single‐transgenic controls were administered intratracheal bleomycin and fed doxycycline chow, starting at either day 0 or day 7. In addition, wild‐type mice received intratracheal or intravenous recombinant FGF2, starting at the time of bleomycin treatment. Compared to controls, doxycycline‐induced DTG mice had decreased pulmonary fibrosis 21 days after bleomycin, as assessed by gene expression and histology. This beneficial effect was seen when FGF2 overexpression was induced at day 0 or day 7 after bleomycin. FGF2 overexpression did not alter epithelial gene expression, bronchoalveolar lavage cellularity or total protein. In vitro studies using primary mouse and human lung fibroblasts showed that FGF2 strongly inhibited baseline and TGFβ1‐induced expression of alpha smooth muscle actin (αSMA), collagen, and connective tissue growth factor. While FGF2 did not suppress phosphorylation of Smad2 or Smad‐dependent gene expression, FGF2 inhibited TGFβ1‐induced stress fiber formation and serum response factor‐dependent gene expression. FGF2 inhibition of stress fiber formation and αSMA requires FGF receptor 1 (FGFR1) and downstream MEK/ERK, but not AKT signaling. In summary, overexpression of FGF2 protects against bleomycin‐induced pulmonary fibrosis in vivo and reverses TGFβ1‐induced collagen and αSMA expression and stress fiber formation in lung fibroblasts in vitro, without affecting either inflammation or epithelial gene expression. Our results suggest that in the lung, FGF2 is antifibrotic in part through decreased collagen expression and fibroblast to myofibroblast differentiation. Background

Whether osteoarthritic patients with mild varus deformity can be indicated for high tibial osteotomy (HTO) is not established. We examined the preoperative characteristics and postoperative outcomes of HTO in patients with mild genu varum compared to patients with greater varus deformity.

Methods

Seventy-one patients who underwent HTO were included in this retrospective study. Patients were divided into either mild varus (MV, mechanical femorotibial angle (mFTA) ≤ 4°, n?=?31 (44%)) and greater varus (GV, mFTA > 4°, n?=?40 (56%)) groups. Preoperative characteristics on single photon emission computed tomography–computed tomography (SPECT–CT), magnetic resonance image and radiograph were evaluated. Alignment parameters and functional outcomes were compared pre- and postoperatively between the groups.

Results

Preoperative characteristics were similar between the two groups, except the severity of arthritis and coronal alignment. There was no difference in the proportion of hot uptake in the medial compartment; medial meniscus posterior horn root tear, complex or radial tear; bone marrow edema. Full-thickness cartilage defect of medial compartment was more frequent and arthritis grade was also more severe in GV group. Coronal alignment of the MV group was corrected into more valgus than the GV group (4.5° vs. 2.8° in mFTA, P?=?0.012). Pre- and postoperative Knee Society knee and function scores were also comparable in the two groups.

Conclusions

Mild varus patients are similar to greater varus patients regarding preoperative features and achieve the comparable functional outcome. A selected subset of osteoarthritic patients with mild varus deformity might be indicated for HTO.
