BACKGROUND: During shoulder replacement surgery, the normal height of the proximal part of the humerus relative to the tuberosities frequently is not restored because of differences in prosthetic geometry or problems with surgical technique. The purpose of the present study was to determine the effect of humeral prosthesis height on range of motion and on the moment arms of the rotator cuff muscles during glenohumeral abduction. METHODS: Tendon excursions and abduction angles were recorded simultaneously in six cadaveric specimens during passive glenohumeral abduction in the scapular plane. Moment arms were calculated for each muscle by computing the slope of the tendon excursion-versus-glenohumeral abduction angle relationship. The experiments were carried out with the intact joint and after replacement of the humeral head with a prosthesis that was inserted in an anatomically correct position as well as 5 and 10 mm too high. RESULTS: Insertion of the prosthesis in positions that were 5 and 10 mm too high resulted in significant and marked reductions of the maximum abduction angle of 10 degrees (range, 5 degrees to 18 degrees ) and 16 degrees (range, 12 degrees to 20 degrees ), respectively. In addition, the moment arms of the infraspinatus and subscapularis decreased by 4 to 10 mm. This corresponded to a 20% to 50% decrease of the abduction moment arms of the infraspinatus and an approximately 50% to 100% decrease of the abduction moment arms of the subscapularis, depending on the abduction angle and the part of the muscle being considered. CONCLUSIONS: If a humeral head prosthesis is placed too high relative to the tuberosities, shoulder function is impaired by two potential mechanisms: (1) the inferior capsule becomes tight at lower abduction angles and limits abduction, and (2) the center of rotation is displaced upward in relation to the line of action of the rotator cuff muscles, resulting in smaller moment arms and decreased abduction moments of the respective muscles. Clinical Relevance: In patients managed with shoulder replacement surgery, limitation of range of motion, loss of abduction strength, and overload with long-term failure of the supraspinatus tendon are potential consequences of positioning the humeral head of the prosthesis proximal to the anatomic position.  相似文献

This study compares remifentanil/propofol (remi/prop) with isoflurane/fentanyl (iso/fen) anesthesia to determine which provides the greater hemodynamic stability, lesser myocardial ischemia, and morbidity with better postoperative outcomes after carotid endarterectomy. Sixty patients undergoing unilateral carotid endarterectomy were randomized to receive either a remi/prop or iso/fen anesthetic. Hemodynamic variables were recorded during the surgical procedure. In addition, transesophageal echocardiography was used to assess evidence of intraoperative regional wall motion abnormalities suggestive of cardiac ischemia. Emergence and extubation times, recovery from anesthesia, hemodynamic instability, nausea, vomiting, and pain in post anesthesia recovery, discharge delays, ICU admittance, hospital discharge, and preoperative and postoperative troponin levels were compared using appropriate statistical methods with P < 0.05 considered significant. The groups were demographically alike. Hemodynamic variables were similar during intubation and throughout surgery. Twenty-two percent of patients receiving iso/fen developed intraoperative regional wall motion abnormalities suggestive of ischemia, whereas no remi/prop patients had changes (P < 0.05). There was no difference in ST-T wave changes after surgery, and no patient had an elevation in troponin I levels. Postoperative variables were similar except that patients who received iso/fen had lower Stewart recovery scores during the first 15 minutes after post anesthesia care unit admission and a higher incidence of nausea and vomiting the day after surgery, whereas patients receiving remi/prop had discharge delays secondary to hypertension. ICU admittance, time to first void, oral intake, and time to hospital discharge were similar between the groups. At 9 times the cost of an iso/fen anesthesia technique, remi/prop offers little advantage over inhalational anesthesia for carotid endarterectomy.  相似文献

There is a need for a noninvasive method that measures wound angiogenesis. Hyperoxia is known to increase the appearance of new blood vessels in wounds, yet no study has confirmed increases in wound bed perfusion with periodic hyperbaric oxygen (HBO) exposure. This study investigates whether laser Doppler imaging is able to detect and quantify the enhancement of wound angiogenesis that is known to occur with intermittent HBO treatments. Full-thickness dorsal dermal wounds were created on mice randomized to hyperoxic (n = 14) and control (n = 15) groups. Hyperbaric oxygen was administered twice daily for 90 minutes each at 2.1 atmospheres for 7 days. Wound bed perfusion was measured by laser Doppler imaging on days 0, 7, and 10 postwounding. Wound blood flow increased significantly over baseline on day 7 and 10 in the hyperoxic group, but only on day 10 in the control group. Comparison between groups showed a 20% statistically significant increase in wound perfusion in HBO-treated animals compared to control on day 10 (p = 0.05). Laser Doppler imaging was able to detect and quantify the increase in wound bed perfusion resulting from intermittent HBO treatments and shows promise as a noninvasive measure of angiogenesis and wound healing.  相似文献

Malignant fibrous histiocytoma MFH is an aggressive spindle cell cancer and is the most common soft tissue tumor in the elderly, primarily affecting the extremities. It has high metastatic potential and can spread to various viscera including liver, lung, bone, and brain; however, cardiac metastasis is an extreme rarity. Here, we present a 50-year-old male, diagnosed as pleiomorhphic storiform MFH a of right arm who developed parenchymal pulmonary metastases and a mass lesion in left atrium. AimsSelf-expanding metal stents provide rapid improvement of dysphagia in oesophageal cancer but are associated with complications. The aim of the present study was to test the effectiveness of an alternative treatment of combining biodegradable stents with radiotherapy.Materials and methodsA Simon two-stage single-arm prospective phase II trial design was used to determine the efficacy of biodegradable stents plus radiotherapy in patients with dysphagia caused by oesophagus cancer who were unsuitable for radical treatment. Fourteen patients were recruited and data from 12 were included in the final analyses.ResultsFive of 12 patients met the primary end point: one stent-related patient death; four further interventions for dysphagia within 16 weeks of stenting (41.7%, 95% confidence interval 15.2–72.3%). The median time to a 10-point deterioration of quality of life was 2.7 weeks. Nine patients died within 52 weeks of registration. The median time to death from any cause was 15.0 weeks (95% confidence interval 9.6–not reached).ConclusionThe high re-intervention observed, which met the pre-defined early stopping criteria, meant that the suggested alternative treatment was not sufficiently effective to be considered for a larger scale trial design. Further work is needed to define the place of biodegradable stents in the management of malignant oesophageal strictures.  相似文献
