During a recent 3-year period, 17 consecutive patients were seen with advanced fibrotic esophageal strictures secondary to alkaline-acid-pepsin reflux. From detailed preoperative evaluations alone it was impossible to determine whether therapy should consist of excisional surgery, esophagogastroplasty or intra-operative dilatation with correction of reflux. Only at operation could the length, extent, degree and severity of the stricture be fully determined. Each of the 17 patients was treated by controlled dilatation, coupled with an antireflux procedure. This simplified approach proved successful on strictures thought preoperatively to be undilatable. It appears that this conservative approach is applicable to many advanced strictures and excisional and plastic procedures should be reserved for those cases that prove unyielding to intraoperative dilatation. The true appraisal of a reflux stricture and the choice of surgical procedure is best determined at the operating table. 相似文献

We describe a middle-aged Chinese systemic lupus erythematosus (SLE) patient developing steroid refractory and transfusion dependent red cell aplasia. BACKGROUND: The aim of this study was to develop models that allow serial, noninvasive imaging of human prostate cancer cells in immunodeficient mice using a dedicated small animal positron emission tomography scanner (microPET). METHODS: LNCaP tumor cells were stably transduced ex-vivo with the mutant herpes simplex virus type 1 thymidine kinase (HSV-sr39tk) PET reporter gene and green fluorescent protein (GFP). The stably transduced LNCaP cells were then enriched via fluorescent cell sorting and implanted into SCID mice. Beginning 2 weeks after tumor cell inoculation, mice were repeatedly scanned by microPET performed 1 hr after tail-vein injection of approximately 200 muCi Fluorine-18 labeled penciclovir ((18)F-FHBG). PET-images were correlated to tumor size, % injected dose (ID)/g tumor tissue, PSA levels, autoradiography, and histology. RESULTS: Monitoring LNCaP xenografts using microPET and our reporter gene approaches is feasible. MicroPET was capable of detecting subcutaneous tumors as small as 3 mm in diameter (approximately 0.2% ID/g). The magnitude of (18)F-FHBG-uptake in PET-images correlated with the tumor volumes and the serum PSA levels. Other non-HSV1-TK-specific tracers were also studied. While (18)F-flurodeoxyglucose ((18)F-FDG) gave poor imaging results in LNCaP cells, (11)C-acetate gave satisfactory images. CONCLUSIONS: We demonstrated the feasibility of monitoring prostate cancer xenografts in a mouse model using microPET and the HSV1-sr39tk PET reporter gene/(18)F-FHBG reporter probe system. Increased Myc gene copy number is observed in human prostate cancer. To define Myc's functional role, we generated transgenic mice expressing human c-Myc in the mouse prostate. All mice developed murine prostatic intraepithelial neoplasia followed by invasive adenocarcinoma. Microarray-based expression profiling identified a Myc prostate cancer expression signature, which included the putative human tumor suppressor NXK3.1. Human prostate tumor databases revealed modules of human genes that varied in concert with the Myc prostate cancer signature. This module includes the Pim-1 kinase, a gene known to cooperate with Myc in tumorigenesis, and defines a subset of human, "Myc-like" human cancers. This approach illustrates how genomic technologies can be applied to mouse cancer models to guide evaluation of human tumor databases. 相似文献

Diet can play a major role in cancer prevention. The international
differences in cancer incidence are largely accounted for by lifestyle
practices that include nutrition, exercise, and alcohol and tobacco use.
About 50% of cancer incidence and 35% of cancer mortality in the U.S.,
represented by cancers of the breast, prostate, pancreas, ovary,
endometrium, and colon, are associated with Western dietary habits. Cancer
of the stomach, currently a major disease in the Far East, relates to
distinct, specific nutritional elements such as excessive salt intake. For
these cancers, information is available on possible initiating genotoxic
factors, promoting elements, and prophylactic agents. In general, the
typical diet in the United States contains low levels of the potent
carcinogenic agents, heterocyclic amines, formed during the cooking of
meats. It provides only about half the potent appropriate fiber intake and
is high in calories. About twice as many calories as would be desirable
come from fat, certain kinds of which enhance the development of cancers.
Other foods with functional properties, such as soy products and tea, can
be beneficial. To achieve reduction in risk of certain cancers, diet must
be optimized, primarily to reduce caloric intake and the fat component. The
latter should be 20% or less of total caloric intake and fiber should be
increased to 25- 35 g per day for adults. One approach to achieving these
goals is the Fiber First Diet, a diet designed around adequate fiber intake
from grains, especially cereals, vegetables, legumes, and fruits, which
thereby reduces both calorie and fat intake. Such dietary improvements will
not only reduce cancer and other chronic disease risks, but will contribute
to a healthy life to an advanced age. A corollary benefit is a lower cost
of medical care.
