Cancer is often accompanied by worsening of the patient’s iron profile, and the resulting anemia could be a factor that negatively impacts antineoplastic treatment efficacy and patient survival. The first line of therapy is usually based on oral or intravenous iron supplementation; however, many patients remain anemic and do not respond. The key might lie in the pathogenesis of the anemia itself. Cancer-related anemia (CRA) is characterized by a decreased circulating serum iron concentration and transferrin saturation despite ample iron stores, pointing to a more complex problem related to iron homeostatic regulation and additional factors such as chronic inflammatory status. This review explores our current understanding of iron homeostasis in cancer, shedding light on the modulatory role of hepcidin in intestinal iron absorption, iron recycling, mobilization from liver deposits, and inducible regulators by infections and inflammation. The underlying relationship between CRA and systemic low-grade inflammation will be discussed, and an integrated multitarget approach based on nutrition and exercise to improve iron utilization by reducing low-grade inflammation, modulating the immune response, and supporting antioxidant mechanisms will also be proposed. Indeed, a Mediterranean-based diet, nutritional supplements and exercise are suggested as potential individualized strategies and as a complementary approach to conventional CRA therapy.  相似文献

A possible reason why levodopa induces a sustained, stable motor benefit during the first months to years of therapy may be its long duration of action. This long-duration effect may be due either to a presynaptic storage mechanism or to postsynaptic pharmacodynamic changes. We previously reported that the dopamine agonist ropinirole induced a long-duration response (LDR) in levodopa-naive patients with Parkinson's disease. In this study, we investigated motor responses to the short half-life dopamine agonist lisuride in a group of levodopa naive parkinsonian patients. Once lisuride reached its maximum effect, it was substituted, in randomized order, with placebo. Neither investigators nor patients knew when the active drug was switched to placebo. When patients were switched from lisuride to placebo, their Unified Parkinson's Disease Rating Scale (UPDRS) motor scores and tapping test and screw scores declined to baseline values within a mean 9.0 +/- 1.9 days. The results confirmed that, like ropinirole and levodopa, the short-acting dopamine agonist lisuride induces a long-duration response, probably due to postsynaptic changes.  相似文献

Cloacal dysgenesis sequence (CDS) is a severe hindgut malformation occurring in 1:50,000 to 250,000 live births (Qureshi et al. Prenatal diagnosis of cloacal dysgenesis sequence: differential diagnosis from other forms of fetal obstructive uropathy. Fetal Diagn Ther 1998;13:69-74; Bargaje et al. Cloacal dysgenesis sequence. Ann Diagn Pathol 2008;12:62-66). It is characterized by a smooth perineum with no urethral, vaginal, or anal openings, and lack of labioscrotal development. Typically, the bladder, vagina, and colon each end blindly, although persistent cloaca without perineal orifice can be seen. With no egress for urine, infants have renal insufficiency and pulmonary hypoplasia, usually making CDS lethal (Sahinoglu Z et al. The prenatal diagnosis of cloacal dysgenesis in six cases: can the termination of pregnancy always be the first choice? Prenat Diagn 2004;24:10-16). Reported survivors have had a persistent urachus or have been spared the effects of oligohydramnios by the presence of a twin (Liang X. The Valsalva maneuver is an autonomic test that evokes short sharp cardiovascular fluctuations mediated by the autonomic nervous system. Numerous spectral analysis methods have been proposed to analyze biological signals. When applied to heart rate (HR) variability, two major bands related to autonomic influence have been defined: LF (mainly sympathetic) and HF (parasympathetic). However, conventional spectral approaches are based on the assumption of stationarity, and most require at least five minutes of recording. These two requirements cannot be fulfilled when analysis of dynamic processes such as the regulatory action of the autonomic nervous system is required. Wavelet transform is a mathematical tool that, by determining the temporal localization of the changes, the frequencies involved and their contribution to the entire signal, overcomes the limitations imposed by conventional methods. In the present work, we use wavelets to evaluate autonomic influence through the LF and HF band powers on acute changes in systolic blood pressure (sBP) and RR intervals (RRI) during the Valsalva maneuver. Eighteen healthy volunteers performed the maneuver by blowing, after a deep inspiration and with a closed glottis, against a pressure of 40 mmHg for 15 seconds. Data were analyzed in three different periods: 1) the last minute just prior to the test (CTR); 2) the 15 seconds of the Valsalva maneuver (VM); 3) during the next 35 seconds after the maneuver (aVM). We observed that LF power increased in sBP and RRI in both VM and ower only increased after Valsalva. The data showed a marked increase in sympathetic activity during and after the maneuver and an increase in parasympathetic outflow after aVM. In conclusion, the ability of wavelets to analyze short non-stationary signals makes wavelet transform a promising tool to evaluate physiological and pathological autonomic conditions.  相似文献
