We show here that T cell cross-reactivity between heterologous viruses influences the immunodominance of virus-specific CD8(+) T cells by two mechanisms. First, T cells specific for cross-reactive epitopes dominate acute responses to viral infections; second, within the memory pool, T cells specific for cross-reactive epitopes are maintained while those specific for non-cross-reactive epitopes are selectively lost. These findings suggest an immunological paradigm in which viral infections shape the available T cell repertoire, causing alterations in the hierarchies of both the primary and memory CD8(+) T cell responses elicited by subsequent viral infections. Thus, immunodominance is a function of the host's previous exposure to unrelated pathogens, and this may have an impact on protective immunity and immunopathology.  相似文献

BACKGROUND: In the absence of a US Food and Drug Administration (FDA)-cleared latex skin testing reagent, in vitro tests remain important for the diagnosis of latex allergy. OBJECTIVE: To evaluate the performance characteristics of IMMULITE 2000 3gAllergy (Immulite), a third-generation, FDA-cleared, continuous random-access immunoanalyzer, for the quantification of latex specific IgE. METHODS: Stored serum samples (N = 201) from patients classified as having positive or negative latex puncture skin test results were measured for latex specific IgE levels using Immulite, and these data were compared with historical results from 3 second-generation, FDA-cleared IgE antilatex assays (AlaSTAT [Ala], AutoCAP [CAP], and HY*TEC enzyme immunoassay [HT]). RESULTS: The diagnostic performances of the CAP, Ala, and Immulite assays (> or = 0.35 kU/L cutoff value) were equivalent in sensitivity and specificity (P > .05). The HT assay (> or = 0.05 kU/L cutoff value) was more sensitive and less specific (P < .05). Immulite (> or = 0.10 kU/L cutoff value) had greater sensitivity than Ala and CAP and greater specificity than HT (P < .05 for both). Diagnostic efficiency was greater for Immulite than for CAP, Ala, and HT (P < .05). CONCLUSIONS: The Immulite system is superior in diagnostic performance, especially at the 0.10 kU/L or greater cutoff level, for the diagnosis of latex allergy compared with older, second-generation assays. Immulite still misclassifies 15.5% of puncture skin test-positive individuals as negative for latex specific IgE. Compared with second-generation assays, Immulite represents a technological advance, with enhanced speed and less operator intervention.  相似文献

Peripheral T-cell lymphomas (PTCLs) are a heterogeneous group of non-Hodgkin lymphomas with a wide spectrum of clinicopathologic features, and apoptosis mechanisms may have a role in lymphomagenesis. We assessed apoptotic rate (AR) in 112 PTCLs using a tissue microarray developed in our laboratory and a modified terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling assay. The mean AR was 1.47% +/- 1.38% for the entire group of PTCLs (range, 0.06%-5.15%), and AR varied significantly among different tumor types. In mycosis fungoides, the mean AR was 0.74%; angioimmunoblastic T-cell lymphoma, 1.02%; PTCL, not otherwise specified, 1.38%; cutaneous anaplastic large cell lymphoma (ALCL), 1.41%; anaplastic lymphoma kinase protein (ALK)-negative ALCL, 1.43%; extranodal natural killer/T-cell lymphoma of nasal type, 2.04%; ALK-positive ALCL, 2.95%; and enteropathy-type T-cell lymphoma, 3.06%. Mean AR was higher in PTCL with large cell vs small/medium cell morphologic features (1.66% +/- 1.1% vs 0.99% +/- 1.0%). In a subset of 33 PTCLs, the tissue microarray results comparedfavorably with those obtained in full tissue sections. We conclude that the highest ARs in PTCLs are found in enteropathy-type T-cell lymphoma and ALK-positive ALCL, and that AR can be assessed reliably by using a tissue microarray.  相似文献

Fifty children diagnosed with medulloblastoma completed 188 psychological evaluations using the Wechsler Intelligence Scales for Children (D. Wechsler, 1974, 1991) over a 7-year study period following 35-40 Gy postoperative craniospinal irradiation. Random coefficient models were used to predict the trend in the children's intellectual performance as a function of time since diagnosis, with both patient and treatment variables as parameters of this function. A quadratic model demonstrated a delay prior to decline in performance for older patients, whereas the younger patients showed an immediate loss of performance with a plateau at approximately 6 years postdiagnosis. A steeper decline was found for those with higher baseline performance. Clinicians may use the proposed predictive model to identify those patients who are at risk of significant intellectual decline.  相似文献

Loss of residual renal function in patients on regular haemodialysis   总被引:3,自引:0,他引:3  
The literature offers scant data on loss of residual renal function in chronic haemodialysis patients. The present study was undertaken in 34 patients, to evaluate residual creatinine clearances (CCr) before the start of haemodialysis and after 3, 12 and 24 months. CCr progressively declined from 6.15 +/- 2.61 (before) to 1.40 +/- 1.29 ml.min-1 (after 24 months: p less than 0.01). The decrease was largest during the first three months of dialysis therapy (slope -0.99 +/- 1.01 ml.min-1.month-1 for the first three months vs. -0.23 +/- 0.12 ml.min-1.month-1 for the entire 24-month period: p less than 0.01). The decline in CCr during the first three months was significantly more pronounced in glomerular disease than in tubulo-interstitial disease (p less than 0.05). This could not be attributed to differences in blood pressure, body weight or hypotensive medications. Age and sex also had no influence. Our data indicate that there is a characteristic progressive loss of renal function in haemodialyzed patients and that the early decline is most pronounced in patients with glomerular disease. Regular assessment of residual renal function at least every three months is indicated in patients starting chronic haemodialysis treatment.  相似文献

Following administration of [3H]choline in the lateral semicircular canal of the cat labyrinth, bidirectional axoplasmic transport of [3H]choline and its derivatives was shown by radioautography in the vestibular system. Light-microscopic radioautographs exhibited various patterns of radioautographic labelling. First, a diffuse reaction was observed in vestibular nuclei representing anterograde-labelled, vestibular nerve endings. Second, a heavy labelling limited to perikarya was detected in efferent vestibular neurons and corresponded to retrograde transport.The anterograde migration of [3H]choline is known to be non-selective and is related to synthesis of phospholipids, non-diffusable molecules. In contrast, the retrograde perikaryal labelling seems highly selective and related to the cholinergic specificity of the transmitter. The selectivity of such labelling offers a further possibility of identifying cholinergic neurons and is additional evidence that cholinergic mechanisms are involved in the efferent vestibular control.  相似文献

The nucleotide sequences of the PorB proteins from 28 nonserotypeable serogroup C ET-15 meningococci recovered from invasive meningococcal disease cases were determined. PCR amplification of the porB genes responsible for encoding the serotype antigen was used for DNA sequence determination and identification of the nature of the serotype antigen. DNA sequencing revealed that three strains were of serotype 2a, and of the remaining 25 strains, 20 were found to have an identical single point mutation in the region of the VR3 gene, which encodes surface-exposed loop VI, where the serotype 2a epitope resides. This nonsynonymous mutation was confirmed by synthetic peptide immunochemical analysis to confer new serospecificity to these serotype 2a mutants. This finding of a potential novel mutational hot spot on the PorB proteins of meningococci may have implications for pathogenesis and vaccine development.  相似文献

Supraependymal cells in the third ventricle of neonatal male and female rats were examined with scanning electron microscopy (SEM) and transmission electron microscopy (TEM). Supraependymal cells in the third ventrical of adult male rats were also studied with SEM. Many neonate supraependymal cells were round to oval in shape and exhibited varying degrees of surface irregularity. Small finger-like processes or narrow pseudopodia projected from the cell bodies over the underlying ependymal cells. Some neonatal supraependymal cells exhibited flattened cell bodies with broad pseudopodia and few surface irregularities. TEM revealed a variety of cell profiles. Prominent within the cytoplasm of many supraependymal cells were lysosomes, smooth and coated vesicles suggesting pinocytosis, subplasmalemma vacuoles, and occasional lipid droplets. The morphological characteristics of neonatal supraependymal cells suggested they were mononuclear phagocytes. Adult supraependymal cells exhibited more pleomorphic cell shapes with numerous cell processes, varying widely in size and shape, and often extending over the ventricular surface for considerable distances. These observations, in combination with previous studies by other investigators, suggest that some adult supraependymal cells may also be phagocytic in nature. The differences in morphology between adult and neonatal supraependymal phagocytes may relate to the differing third ventricle environment between adult and neonates and/or differences in the origin of the phagocytes with age.  相似文献
