Renal disease has not been considered a major late complication of bone marrow transplantation. Of 31 evaluable pediatric patients undergoing allogeneic or autologous bone marrow transplantation for neuroblastoma or acute lymphoblastic leukemia, 14 developed a hemolytic anemia, microscopic hematuria, and renal insufficiency at a median of 5 months (range, 3 to 7 months) posttransplant. Renal biopsies were performed in two patients at the onset of kidney disease and showed mesangiolysis with intraglomerular capillary aneurysm formation, mesangial proliferation, and focal thickening and splitting of the glomerular basement membranes. The clinical presentation, time to onset of renal disease, and biopsy material are consistent with a diagnosis of radiation nephritis, a previously uncommon finding in this patient group. The high incidence of this syndrome in the current report may have been due to the combination of intensive chemotherapy and total- body irradiation in the conditioning regimens. 相似文献

With change in the imaging technique and magnetic field strength used in magnetic resonance imaging, wide variations in the delineation of pathologic features occur. Using imaging data from patients with known pathologic conditions, we evaluated the intensity images in spin-echo and inversion-recovery imaging at varying repetition times, echo times, and inversion times over broad ranges and changing magnetic field strengths. Differences in conspicuity and the apparent size of the lesions are important to consider in diagnosing and evaluating pathologic conditions, especially when different imagers and techniques are employed. 相似文献

BACKGROUND: Intramedullary fixation of midclavicular fractures may be a better option than non-operative treatment for high performance/professional athletes because of the potential reduction in recovery time. OBJECTIVES: To evaluate the effectiveness of intramedullary fixation in high performance athletes and the time required to return to sporting activity. METHODS: Data were taken from a prospective study on intramedullary fixation techniques using the elastic stable Ti nail (TEN, Synthes) for the treatment of displaced midclavicular fractures, initiated in 1996. The patients in 12 cases were classified as high performance/professional athletes. These cases were used to evaluate the technique specifically in this population. Fractures were classified according to the Orthopaedic Trauma Association (OTA) system. Patients were evaluated before and after surgery for shoulder function and subjective pain. After the operation, radiological assessments documented fracture healing, and clinical outcomes scores were obtained. Time required to return to training and competition was documented. RESULTS: All fractures were transverse or oblique. Mean (SD) shoulder abduction increased from 36.3 (8) degrees before surgery to 154.2 (17) degrees afterwards (p<0.001). Mean subjective pain score using a visual analogue scale (0-100) decreased from 71.7 (18) points before surgery to 19.2 (6) points (p<0.001) three days after. There were no complications. Hospital stay averaged 2.9 (1) days. Mean delay to resumption of training was 5.9 (1) days, and to resumption of competition it was 16.8 (5) days. The mean Constant clinical outcomes score one year after hardware removal was 98.3 (2) points. CONCLUSIONS: Intramedullary fixation of displaced midclavicular fracture was successful in terms of clinical outcome and rapid resumption of sporting activities. Arterial stiffness normally increases with age and has been established as a precursor of cardiovascular disease. Interleukin-6 is a pleiotropic inflammatory cytokine with an important role in the inflammatory cascade, such as up-regulation of C-reactive protein (CRP). The interleukin-6 –174-G/C promoter polymorphism appears to influence levels of inflammatory markers, which have been shown to be associated with arterial stiffness. We studied the association of this polymorphism with levels of interleukin-6 and CRP and with arterial stiffness. The study (n = 3849) was embedded in the Rotterdam Study, a prospective, population-based study. Analyses on the association between the –174-G/C polymorphism and pulse wave velocity, distensibility coefficient, and pulse pressure were performed using analyses of variance. Analyses on the levels of inflammatory markers and arterial stiffness were performed using linear regression analyses. Analyses were adjusted for age, sex, mean arterial pressure, heart rate, known cardiovascular risk factors, and atherosclerosis. We found pulse wave velocity to be 0.35 m/s higher for CC-homozygotes vs. wildtype GG-homozygotes (p = 0.018) with evidence for an allele-dose effect (p trend = 0.013), and a similar pattern for pulse pressure (p trend = 0.041). No apparent consistent association with the distensibility coefficient was found. CRP levels were associated with pulse wave velocity (p = 0.007). Bronchogenic carcinomas arise from bronchial epithelial cells (BECs).
Inhalation exposure of BECs to nitrosamines in cigarette smoke is an
important exogenous risk factor for malignant transformation of BECs. Thus,
an important endogenous risk factor is likely to be the capacity of BECs to
metabolize nitrosamines. Among the cytochrome P450 enzymes capable of
metabolizing nitrosamines, CYP2A6, CYP2E1 and CYP2B6 are expressed in BECs.
In this study, we used quantitative RT-PCR to evaluate expression of CYP2A6
and CYP2E1 in primary human BECs from 12 non-smokers and eight smokers.
CYP2A6 was expressed in 20/20 cases and quantifiable in 18/20 cases, with a
mean level of 580 mRNA/10(6) beta- actin mRNA. CYP2E1 expression was
observed in 9/20 cases, but in all cases it was expressed at levels below
our limit of quantification (10 mRNA/10(6) beta-actin mRNA). There was
significant (P < 0.05) 20-fold inter-individual variation in expression
of CYP2A6. Further, the mean level of CYP2A6 among smokers (260 mRNA/10(6)
beta-actin mRNA) was significantly lower than among non-smokers (740
mRNA/10(6) beta-actin mRNA). It is hypothesized that: (i) inter-individual
variation in CYP2A6 gene expression may contribute to inter-individual
variation in risk for bronchogenic carcinoma; (ii) smoking may reduce the
level of expression of CYP2A6 in the BECs of some individuals; and (iii)
CYP2A6 is more important than CYP2E1 for metabolic activation of
nitrosamines in bronchial epithelial cells.
