An evaluation of CA 549, a circulating marker of breast cancer using a procedure for comparison with CA 15.3.

A serum assay of CA 549 (Hybri-BREScan CA 549 degrees, Hybritech), a new tumor marker, was performed in 129 patients with breast cancer and 35 healthy women, in parallel with CA 15.3 (ELSA-CA 15.3 degrees, CIS Biointernational). Comparing 95 women with primary breast carcinoma and 35 controls, Relative (or Receiver) Operating Characteristic (ROC) analysis revealed that the Area Under ROC Curve (AUC) of CA 15.3 was significantly higher than that of CA 549, indicating that, for our population, the first marker was more effective. Parallel and series analyses were also performed using ROC AUC and revealed that the combination of these two tests did not give more information than the CA 15.3 test alone; however, they did not in any way constitute diagnostic tools. In our experience, the best field of application for CA 549 seems to be the therapeutic monitoring and early detection of breast cancer recurrences. However, further investigations on a larger scale are necessary to assess more precisely the place of CA 549 in following the clinical course of breast cancer patients.     

Diagnostic value of computer analysis of multipeaked EMG spikes.

Computer analysis of absolute number of peaks per second and the number of peaks in the multipeaked spikes/sec was carried out in the EMG interference recordings of 5 healthy, 6 myopathic and 8 neuropathic subjects. The purpose of the study was to detect diagnostically different patterns. A spike was considered multipeaked if it had 6 or more peaks. The amplitude of elimination (baseline) was examined at 1: 5, 1: 10 and 1: 15 of the average amplitude per second. Both the number of peaks and the number of peaks in multipeaked spikes in the neuropathic and myopathic muscles showed statistically significant differences when compared to healthy muscles. This technique could give an indication for the differential diagnosis of myopathic, neuropathic or healthy state of the muscle.     

Periodontal conditions in insulin-dependent diabetes mellitus.

In the present investigation, the frequency and severity of periodontal disease was assessed in a group of 71 patients with a mean duration of 16.5 years of insulin-dependent diabetes mellitus (IDD). The diabetics, aged 17-63 years, were under treatment at the diabetic outpatient clinic of the III Department of Medicine, University Central Hospital of Helsinki and at two clinics of the Helsinki Health Centre. Based upon their long-term medical records, 44 individuals were assessed to have a poorly controlled insulin-dependent diabetes mellitus (PIDD). At baseline of the present study, the PIDD group had a mean blood glucose level of 11.8 mmol/l and a mean glycosylated hemoglobin (HBA1) level of 10.7%. 27 subjects were classified as having a controlled insulin-dependent diabetes mellitus (CIDD). For each individual, site-specific recordings were made for the plaque index, gingival index, pocket depth, loss of attachment, bleeding after probing, gingival recession and radiographic loss of alveolar bone. Under similar plaque conditions, adult subjects with a long-term PIDD were found to have lost more approximal attachment and bone than subjects with a CIDD (P = 0.046, P = 0.019). These differences were not equally obvious when the subjects were classified according to the history of medical complications, such as retinopathies, neuropathies and nephropathies.     

Hemapheresis in pregnancy.

Plasma exchange in pregnancy has benefited those diseases that occur and are treated in the nonpregnant patient. In addition, special emphasis was given to diseases unique to pregnancy, such as hemolytic disease of the newborn, which is significantly modified by plasma exchange therapy. Fluid volumes, replacement solution, and techniques were discussed.     

Current methods in the diagnosis of adult-age cystic fibrosis]

An 19 year old male patient with cystic fibrosis is reported. The clinical, pathogenetic, anatomical features and literature data are reviewed. Attention is called to recent diagnostic facilities.     

The kappa statistic for establishing interrater reliability in the secondary analysis of qualitative clinical data.

Analysis of extant clinical records is receiving increased emphasis in nursing investigations. Appropriate use of this approach to patient research requires careful attention to data management, including assessment of reliability. Percent agreement, phi, and Kappa all serve as estimates of interrater reliability in the analysis of data. Kappa has particular merit as a measure of interrater reliability; it also has some peculiar problems in implementation and interpretation. The nature and computation of Kappa and its application in analysis of clinical data are discussed.     

The clinico-diagnostic significance of determining creatine kinase activity and isoenzymes in patients with systemic lupus erythematosus

Creatine kinase (CK) activity was assessed in the direct and reverse tests and CK isoenzymes in agar gel for 41 patients with systemic lupus erythematosus (SLE). With growing activity of the process, there was a decline of CK activity in the reverse test though in the direct test this activity was scarcely changeable allowing a shift to predominance of the direct response. Significant differences were found in CK reverse reaction which correlated with SLE activity and pattern. The more active the disease, the less content of CK-1 and CK-3 isoenzymes. Investigations of CK activity and isoenzymes during SLE treatment help objectivize the clinical control.     

Volar scaphotrapezial dislocation.

We present a case of volar luxation of the scaphotrapezial bone, in which radiographs suggested luxation of the scaphoid, but unclear relations to the other carpal bones. Computer tomographic reconstruction showed an isolated scaphotrapezial luxation. This was treated by closed reduction and a plaster cast, after which the patient regained normal function of her wrist.
