BACKGROUND: Malondialdehyde (MDA) in plasma is regarded as an indicator for increased lipid peroxidation. METHOD: Measurements of MDA concentrations in plasma were compared among healthy children (n = 31), patients with neurological disorders or epileptic syndromes (n = 15), and children with pontocerebellar structural defects (n = 31), where the cause or genetic defect remained unknown. RESULTS: In healthy children the median MDA value was 5.86 nmol/ml (mean (SD) value: 6.25 (1.97), range: 3.76-11.19). For the group with various neurological disorders or epilepsy, the values were similar with the median value at 5.66 nmol/ml (range 0.22-10.86). Compared with healthy controls and the neurological/ epileptic group, the 31 children with pontocerebellar structural defects had significantly increased MDA values with a median value at 11.29 nmol/ml (mean (SD) value: 11.62 (3.27), range 3.65-19.22). IMPLICATION: These findings of increased plasma MDA in the majority of children with pontocerebellar structural defects of unknown origin raised the question whether increased lipid peroxidation leads to prenatal and postnatal pontocerebellar maldevelopment or degeneration.  相似文献

Inappropriate therapy remains an important limitation of implantable cardioverter defibrillators (ICD). PARAD+ was developed to increase the specificity conferred by the original PARAD detection algorithm in the detection of atrial fibrillation (AF). To compare the performances of the two different algorithms, we retrospectively analyzed all spontaneous and sustained episodes of AF and ventricular tachycardia (VT) documented by state-of-the-art ICDs programmed with PARAD or PARAD+ at the physicians' discretion. The results were stratified according to tachycardia rates <150 versus > or =150 beats/min. The study included 329 men and 48 women (64 +/- 10 years of age). PARAD was programmed in 263, and PARAD+ in 84 devices. During a mean follow-up of 11 +/- 3 months, 1,019 VT and 315 AF episodes were documented among 338 devices. For tachycardias with ventricular rates <150 beats/min, the sensitivity of PARAD versus PARAD+ was 96% versus 99% (NS), specificity 80% versus 93% (P < 0.002), positive predictive value (PPV) 94% versus 91% (NS), and negative predictive value (NPV) 86% versus 99% (P < 0.0001). In contrast, in the fast VT zone, the specificity and PPV of PARAD (95% versus 84% and 100% versus 96%) were higher than those of PARAD+ (NS, P < 0.001). Among 23 AF episodes treated in 16 patients, 3 episodes triggered an inappropriate shock in 3 patients, all in the PARAD population. PARAD+ significantly increased the ICD algorithm diagnostic specificity and NPV for AF in the slow VT zone without compromising patient safety.  相似文献

Heteromorpha arborescens belongs to the family Apiaceae. It is commonly known as the parsley tree. One of its uses in the Eastern Cape Province of South Africa is for the treatment of abdominal pains. The therapeutic effects of the methanolic and aqueous root extracts of H. arborescens were investigated at two dose levels respectively on experimental models of pain and inflammation in rodents. The antinociceptive activity was evaluated using the hot-plate, abdominal constriction and formalin tests. The anti-inflammatory properties of these extracts were assessed using albumin and carrageenan as phlogistic agents. Both extracts produced significant (P<0.05, P<0.01) inhibition of thermal nociception induced by a hot plate. On chemical nociception induced by intraperitoneal acetic acid and subplantar formalin injection, both extracts significantly (P<0.05, P<0.01) decreased the number of writhing episodes and the licking time in a dose dependent manner. Treatment with the extracts at the same doses produced a significant (P<0.05, P<0.01) pain inhibition of the carrageenan induced inflammatory pain. Similarly, both extracts produced a significant (P<0.05, P<0.01) reduction of edema induced by albumin and carrageenan. These results suggest that both extracts of H. arborescens may act by inhibition of the mediators of inflammation. These findings seem to justify the use of the plant in traditional medicine in the management of pain and inflammation related diseases.  相似文献

β-Hydroxy-β-methylbutyrate (HMβ) supplementation is used to treat cancer, sepsis and exercise-induced muscle damage. However, its effects on animal and human health and the consequences of this treatment in other tissues (e.g., fat and liver) have not been examined. The purpose of this study was to evaluate the effects of HMβ supplementation on skeletal muscle hypertrophy and the expression of proteins involved in insulin signalling. Rats were treated with HMβ (320 mg/kg body weight) or saline for one month. The skeletal muscle hypertrophy and insulin signalling were evaluated by western blotting, and hormonal concentrations were evaluated using ELISAs. HMβ supplementation induced muscle hypertrophy in the extensor digitorum longus (EDL) and soleus muscles and increased serum insulin levels, the expression of the mammalian target of rapamycin (mTOR) and phosphorylation of p70S6K in the EDL muscle. Expression of the insulin receptor was increased only in liver. Thus, our results suggest that HMβ supplementation can be used to increase muscle mass without adverse health effects.  相似文献

The HIV epidemic presents challenges including orphans and a large mass of children rendered vulnerable by the epidemic and other societal forces. Focus on orphaned and vulnerable children (OVC) is important, but needs accurate definition. Twelve focus group interviews of service providers, leaders in these communities, OVC and their caretakers were conducted at six project sites across Botswana, South Africa and Zimbabwe to extend this definition. The loss of a parent through death or desertion is an important aspect of vulnerability. Additional factors leading to vulnerability included severe chronic illness of a parent or caregiver, poverty, hunger, lack of access to services, inadequate clothing or shelter, overcrowding, deficient caretakers, and factors specific to the child, including disability, direct experience of physical or sexual violence, or severe chronic illness. Important questions raised in this research include the long-term implications for the child and community, and the contribution of culture systems.  相似文献
