Background

Plasma removal by washing is an effective approach to prevent transfusion reactions by platelet concentrates (PCs). Recently, washed PCs were released by the Japanese Red Cross Society (JRCS).

Materials and methods

This retrospective multicenter study evaluated the efficacy and safety of released washed PCs (RWPCs) between September 2016 and January 2017 in Japan. The RWPCs were prepared by washing leukoreduced apheresis PCs with the platelet additive solution, BRS-A, using automated cell processors.

Results

Clinical data were obtained from 91 patients and 1210 RWPC transfusions at 50 institutions. The median number of RWPC transfusions per patient was 8 (range, 1–91). RWPCs were used in 94.5% of the patients with a history of recurrent or severe transfusion reactions for preventing such reactions. Responses of RWPCs were evaluated as complete response (91.6%), partial response (8.2%), no-change (0.2%), and progression (0%) and overall response was equal across subgroups divided by patients’ profiles. The median corrected count increment (CCI) at 1 and 24?h post-transfusion were 13.5 (range, 1.9–35.4)?×?109/L and 3.5 (range, ?13 to 53.6)?×?109/L, respectively, and median CCI at 24?h was 5.5 (range, ?13 to 53.6)?×?109/L in patients without risk factors associated with platelet transfusion refractoriness. Transfusion reactions to RWPCs were observed in only nine transfusions (0.7%), all of which were mild allergic reactions.

Conclusion

This study demonstrated that RWPCs were effective and safe in patients with a history of transfusion reactions. Further prospective studies on efficacy together with cost-benefit analysis in RWPCs are needed.  相似文献

Abdominal lymphangioma is usually diagnosed within the first 2 years of life and is extremely rare in adults. The most common location of abdominal lymphangioma is the mesentery, but there are sporadic reports of its development in the gallbladder. A 66-year-old woman was found to have a cystic lesion near the gallbladder. Preoperative studies, including endoscopic ultrasonography, computed tomography, and magnetic resonance imaging, showed a tumor with multilocular cystic structure, originating in the gallbladder fossa. The patient underwent exploratory laparotomy, and the mass was resected en bloc with the gallbladder, as there was no evidence of malignancy on intraoperative ultrasonography. Macroscopically, the tumor was a multilocular cystic mass, 6 × 3 × 2 cm in size, with a rough, sponge-like appearance. Histologically, the cystic tumor was diagnosed as a lymphangioma, originating in the gallbladder. To our knowledge, only three other cases of a cystic lymphangioma originating in the gallbladder have been reported in the medical literature of the world.  相似文献

Maturation of dendritic cells induced by Candida beta-D-glucan   总被引:1,自引:0,他引:1  
We investigated whether Candida beta-D-glucan (CSBG) alters the maturation of dendritic cells (DCs). DC phenotypes were analyzed using FACScan. The expression of surface molecules, including major histocompatibility complex (MHC) classes I and II, as well as CD80 and CD86, increased on DCs that were stimulated with lipopolysaccaride (DCs/LPS), in comparison with unstimulated bone marrow-derived DCs (BM-DCs). Furthermore, the level of surface molecule expression on DCs stimulated with CSBG (DCs/CSBG) was between that of DCs and DCs/LPS. Phagocytosis was assessed by the uptake of FITC-dextran. There were no differences in the uptake of dextran among DCs/LPS and DCs/CSBG. The ability of BM-DCs to uptake dextran was higher than that of DCs/LPS and DCs/CSBG. We analyzed the concentration of IL-12 secreted by DCs using ELISA. BM-DCs secreted a low concentration of IL-12, while DCs/LPS and DCs/CSBG secreted higher levels of IL-12 than BM-DCs. There were no remarkable differences in the concentrations of IL-12 produced by DCs/LPS and DCs/CSBG. This data suggests that CSBG may augment DC maturation.  相似文献

Spontaneous activities of the reticulospinal neurons in the reticular formation of the rostroventral medulla, of the ear sympathetic nerve (ESNA) and of the renal sympathetic nerve (RSNA) were analyzed with regard to cardiac cycle- and respiration-related rhythm in the anesthetized, vagotomized and immobilized rabbits. A reticulospinal neuron that was concurrently excited with increase in the ESNA and/or reduction of the blood flow of the ear skin by electrical stimulation of the dorsomedial hypothalamus was tentatively named as a cutaneous sympatho-excitatory neuron (Cu neuron). More than half of the Cu neurons (13/22) had a respiration-related rhythmic activity as well as the ESNA. Activity of most of the Cu neurons (19/22) was not modulated with the frequency of the heartbeat and the ESNA had little or no cardiac cycle-related activity. Simultaneous recording shows that the degree of modulation (relative power of the power spectrum of the post event time histogram at the frequency of the respiration) of activity of the Cu neurons correlated with that of the ESNA. On the other hand, most (13/18) of the barosensitive sympatho-excitatory reticulospinal neurons in the rostral ventrolateral medulla (RVLM neurons) had both cardiac cycle- and respiration-related activity as well as the RSNA had. The Cu neurons were located at the medial sites to the location of the RVLM neurons. These results further showed that the Cu neurons controlled the cutaneous vasoconstrictor fibers and that the sympatho-excitatory neurons were located at the different sites in the ventral medulla according to their function.  相似文献

Demyelination associated with HSV-1-induced facial paralysis   总被引:3,自引:0,他引:3  
In 1995, we developed an animal model of transient homolateral facial nerve paralysis by inoculating Herpes simplex virus type 1 (HSV-1) into the auricle of mice. This study examined the mechanism of facial nerve paralysis in this model histopathologically. Using the immunofluorescence technique with anti-HSV-1 antibody, the time course of viral spread and the site of viral replication were investigated over the entire course of the facial nerve. Furthermore, viral replication and nerve degeneration at the site of viral replication were observed by electron microscopy. On the 7th day after inoculation, facial paralysis was observed in more than 60% of mice. Immunofluorescence study revealed HSV-1 in the geniculate ganglion, the descending root, and the facial nucleus at this stage. On the 9th day, the descending root in the sections stained with osmium looked pale, because prominent demyelination had occurred in this region; electron micrographs showed many degenerated oligodendrocytes and large naked axons. In contrast, the facial nucleus neurons showed no remarkable degeneration, despite HSV-1 particles in their cytoplasm. From these findings, we concluded that facial nerve paralysis in this model is caused mainly by facial nerve demyelination in the descending root.  相似文献
