Use of amniotic membrane transplantation in the treatment of venous leg ulcers

Amniotic membrane (AM), the most internal placental membrane, has unique properties including antiadhesive effects, bacteriostatic, wound protection and pain-reduction properties, as well as epithelialization initialization capacities. Furthermore, AM is widely available and less costly than other bioengineered skin substitutes. In a prospective pilot study, we evaluated the safety, feasibility, and the effects on healing of AM graft in 15 patients with chronic venous leg ulcers. AM grafts were prepared from placentas harvested during cesarean section. All grafted AM had adhered to the wound bed 7 days after being applied with a 100% engraftment rate. The percentage of granulation tissue increased significantly (from 17% on day 0 to 69% on day 14, p<0.0001), along with a significant decrease of fibrinous slough (from 36% at day 0 to 16% at day 14, p<0.001). A significant clinical response occurred in 12 patients (80%) including complete healing (20%) in three during the 3-month follow-up period. The ulcer surface area decreased significantly from a mean value (+/- standard deviation) of 4.59 +/- 2.49 cm(2) at baseline to 2.91+/-2.01 cm(2) on day 30 (p<0.001). All patients experienced a significant reduction of ulcer-related pain rapidly after AM transplantation. No adverse events were recorded. AM transplantation seems to function as a safe substrate, promoting proper epithelialization while suppressing excessive fibrosis. Further advantages of biotherapy with AM are its easy and low-cost production, and that it can be applied as an ambulatory treatment without immobilization. AM transplantation may thus be considered to be an alternative method for treating chronic leg ulcers.

自杀已经成为一个世界范围内的重大公共卫生问题。中国的自杀特征具有独特性。自杀行为的发生往往是内在因素与外部因素相互作用的结果，它与社会学、心理学、伦理学、生物学、精神病学等各方面密切相关。自杀原因虽然错综复杂，但我们要提高意识，认识到自杀是一种可以预防的公共卫生问题。     

基于变更模型的元数据在构件回归测试中的应用研究

本文针对Orso元数据方法的不足,对构件可能的变更进行了充分分析和分类,并按照分类情况对相关的构件变更处理方法进行了详尽的形式化描述,构造了构件变更模型,描述了每个变更的具体表现形式,论述了方法变更到构件接口变更的映射机制和算法,给出了算法实现的框架,并将这些理论模型应用到自主开发的构件RegisterStuGrade中,与没有元数据情况下的回归测试用例生成技术以及Orso方法进行回归测试用例数和回归测试运行时间两方面的分析和对比,给出了比较结果.     

Sleep length and quality,sleepiness and urinary melatonin among healthy Danish nurses with shift work during work and leisure time

Background  

Sleep problems are common effects of shift work. The aim of the present study was to evaluate how different types of shift affect sleep and sleepiness, and to relate sleepiness to urinary 6-sulfatoxymelatonin.     

Emphysematous pyelonephritis successfully treated with laparoscopic nephrectomy

Emphysematous pyelonephritis is a severe life-threatening infection which continues to carry significant morbidity and mortality. We present a case recently managed at our institution by laparoscopic nephrectomy. The patient survived, and in comparison to some of the more conventionally managed patients in the literature, made an extremely speedy recovery. We would advocate this option to be seriously considered when patients are suitable and appropriately trained surgeons are available.     

硫酸奈替米星葡萄糖注射液联用复方维生素注射液致呼吸困难伴腰痛1例

1临床资料 患者，女，43岁，因车祸致右胫腓骨骨折入住我院创伤骨科，一般情况良好，无食物、药物过敏史，入院后行腓骨复位固定术，3d后，伤口红肿并有脓液渗出，为预防和控制感染，静脉滴注硫酸奈替米星葡萄糖注射液每日200mL（每瓶100mL，硫酸奈替米星10万u，约相当于0．1g，葡萄糖5．0g，山东鲁抗辰欣药业有限公司，批号0803155209）分两次滴注，中间滴注含有4mL复方维生素注射液的10％葡萄糖注射液500mL（四川美大康佳乐有限公司，批号080125124）。     

在深静脉血栓—肺栓塞病程中腔静脉滤器的应用价值分析

目的：探讨腔静脉滤器(Vena cava filter VCF)在深静脉血栓(Deep venous thrombosis DVT)—肺栓塞(Pulmonary embolism PE)病程中的应用价值。方法：结合本组25例高危深静脉血栓一肺栓塞患者，对其中24例置放5种构型的永久型或临时型下腔静脉滤器(IVCF)26枚的临床疗效、影响病程与预后因素及病生理改变，复习文献分析讨论。结果：DVT有可能脱落导致PE或致死性肺栓塞；置放IVCF可有效预防肺栓塞，早期进行溶栓治疗，改变预后。结论：严格按适应证植入IVCF，对DVT—PE的早期治疗和预防致死性肺栓塞有重要意义，但其应用价值需进行综合评价和进一步探讨。
