Background

Short-term experimental and animal studies have confirmed superior fixation of cementless implants inserted with compaction compared to broaching of the cancellous bone.

Methods

Forty-four hips in 42 patients (19 men) were randomly operated using cementless hydroxyapatite-coated Bi-Metric stems. Patients were followed with radiostereometric analysis at baseline, 6 and 12 weeks, 1, 2, and 5 years, and measurements of periprosthetic bone mineral density at baseline, 1, 2, and 5 years. Complications during the study period and clinical outcome measures of Harris Hip Score were recorded at mean 7 years (5-8.8) after surgery.

Results

Absolute migrations of medio/lateral translations between the broaching group and the compaction group of mean 0.14 mm (standard deviation [SD] 0.10) vs mean 0.30 mm (SD 0.27) (P = .01) at 1 year, and of mean 0.13 mm (SD 0.10) vs 0.34 mm (0.31) (P = .01) at 5 years were different. Absolute valgus/varus rotations of mean 0.12° (SD 0.13°) in the broaching group were less than mean 0.35° (0.45°) in the compaction group (P < .01) at 1 year, but at 5 years no difference was observed (P = .19). Subsidence and retroversion were similar between groups at all follow-ups (P > .13). The compaction group had significantly less bone loss than the broaching group in Gruen zone 3 (distal-lateral to the stem) at 1 and 5 years. No further differences in bone mineral density changes were found between groups up to 5 years after surgery. Complications throughout the period and clinical outcome measures of Harris Hip Score were similar at 7 years (5-8.8) after surgery.

Conclusion

We found increased migration when preparing the bone with compaction compared with broaching in cementless femoral stems.  相似文献

Background and purpose — Following a hip fracture, most patients will encounter poorer functional outcomes and an increased risk of death. Treatment-monitoring of hip fracture patients is in many countries done by national audits. However, they do not allow for a deeper understanding of treatment limitations. We performed a local evaluation study to investigate adherence to 7 best-practice indicators, and to investigate patient groups at risk of suboptimal treatment.Patients and methods — 2,804 patients were surgically treated for a hip fracture from 2011 to 2017 at our institution. Data regarding admission, hospital stay, and discharge was prospectively collected, and adherence to the 7 best practice indicators (nerve block, surgical delay, antibiotics, implant choice, thromboprophylaxis, mobilization, and blood transfusions) was analyzed. Patient groups with lower adherence were identified.Results — 34% of patients received all 7 best practice indicators after considering contraindications; in particular, nerve blocks and thromboprophylaxis displayed low adherence at 61% and 91% respectively. Nursing home residents and patients with cognitive impairment, multiple comorbidities, or low functional levels were at risk of having a lower adherence.Interpretation — The most dependent patients with cognitive impairment, comorbidities, or low functional levels had lower guideline adherence. This large patient subgroup needs a higher treatment focus and more resources. Our findings are likely similar to those in other national and international institutions.

Hip fractures are a leading cause of disability and mortality among seniors worldwide, with 1-year mortality surpassing 20%. Survivors often experience diminished walking ability, reduced activities of daily living, and loss of independence (Bentler et al. 2009, Dyer et al. 2016). Recent years have seen only minimal improvements in outcomes, such as mortality, which suggest that hip fracture treatment needs improvement (Rogmark 2020). However, patients with hip fracture represent a heterogeneous and fragile patient group with multiple comorbidities, which complicates treatment.Evidence-based treatment is fundamental to modern medicine, and previous research has demonstrated improved outcomes for patients receiving best practice indicators (Nielsen et al. 2009, Kristensen et al. 2016, Oakley et al. 2017, Farrow et al. 2018). However, most studies are based on process indicators, which give no information on the actual treatment provided; this includes national audits (Sweden’s National Quality Register 2018, Danish Multidisciplinary Hip Fracture Registry 2019, Royal College of Physicians 2019, Australian & New Zealand Hip Fracture Registry 2019). To our knowledge, only a few studies have evaluated direct local adherence to guidelines for patients with hip fracture (Seys et al. 2018, Mcglynn et al. 2003, Sunol et al. 2015). Continuous monitoring through national audits and local studies might detect gaps in the treatment of patients with hip fracture and hopefully secure improvement.We assessed the degree of adherence to 7 best practice indicators in a local evidence-based guideline for treatment of hip fractures. We expected adherence to increase during the study period as the guideline was incorporated better over time. Furthermore, the study aimed to clarify whether particular patient groups are at risk of significantly lower guideline adherence and hence suboptimal treatment at our institution.  相似文献

The in vitro activity of isavuconazole against Mucorales isolates measured by EUCAST E.Def 9.2 and CLSI M38-A2 methodologies was investigated in comparison with those of amphotericin B, posaconazole, and voriconazole. Seventy-two isolates were included: 12 of Lichtheimia corymbifera, 5 of Lichtheimia ramosa, 5 of group I and 9 of group II of Mucor circinelloides, 9 of Rhizomucor pusillus, 26 of Rhizopus microsporus, and 6 of Rhizopus oryzae. Species identification was confirmed by internal transcribed spacer (ITS) sequencing. EUCAST MICs were read on day 1 (EUCAST-d1) and day 2 (EUCAST-d2), and CLSI MICs were read on day 2 (CLSI-d2). Isavuconazole MIC50s (range) (mg/liter) by EUCAST-d1, CLSI-d2, and EUCAST-d2 were 1 (0.125 to 16), 1 (0.125 to 2), and 4 (0.5 to >16), respectively, across all isolates. The similar values for comparator drugs were as follows: posaconazole, 0.25 (≤0.03 to >16), 0.25 (0.06 to >16), and 1 (0.06 to >16); amphotericin, 0.06 (≤0.03 to 0.5), 0.06 (≤0.03 to 0.25), and 0.125 (≤0.03 to 1); voriconazole, 16 (2 to >16), 8 (1 to >16), and >16 (8 to >16), respectively. Isavuconazole activity varied by species: Lichtheimia corymbifera, 1 (0.5 to 2), 1 (1 to 2), and 2 (1 to 4); Lichtheimia ramosa, 0.25 (0.125 to 0.5), 1 (0.5 to 2), and 2 (0.5 to 4); Rhizomucor pusillus, 0.5 (0.5 to 1), 1 (0.125 to 1), and 2 (1 to 2); Rhizopus microsporus, 1 (0.5 to 4), 0.5 (0.125 to 1), and 4 (1 to 8); and Rhizopus oryzae, 1 (0.5 to 4), 1 (0.125 to 2), and 4 (0.5 to 8), respectively, were more susceptible than Mucor circinelloides: group I, 8 (4 to 8), 4 (2 to 4), and 16 (2 to 16), respectively, and group II, 8 (1 to 16), 8 (1 to 8), and 16 (4 to >16), respectively. This was also observed for posaconazole. The essential agreement was best between EUCAST-d1 and CLSI-d2 (75% to 83%). Isavuconazole displayed in vitro activity against Mucorales isolates with the exception of Mucor circinelloides. The MICs were in general 1 to 3 steps higher than those for posaconazole. However, in the clinical setting this may be compensated for by the higher exposure at standard dosing.  相似文献

Purpose

Little is known about the anterolateral ligament’s (ALL) influence on knee laxity. The purpose of this study was to investigate rotational knee laxity against a pure axial rotational stress using radiostereometric analysis (RSA) after cutting and reconstructing both the anterior cruciate ligament (ACL) and the ALL.

Methods

Eight human donor legs were positioned and stereoradiographically recorded at 0°, 30° and 60° of knee flexion using a motorised fixture, while an internally rotating force of 4 Nm was applied to the foot. Anterior–posterior and rotational laxity were investigated for knees with intact ligaments and compared with those observed after successive ACL and ALL resection and reconstruction.

Results

After cutting the ALL in ACL-deficient knees, the internal rotation was increased in all three knee flexion angles, 0° (p?=?0.04), 30° (p?=?0.03) and 60° (p?<?0.01) by 1.0°, 1.6° and 2.5°, respectively. However, no decrease in laxity was found after reconstructing the ALL in ACL-reconstructed knees.

Conclusions

The ALL was confirmed as a stabiliser of internal rotation in ACL-deficient knees. However, reconstructing the ALL using a gracilis autograft tendon did not decrease the internal rotation laxity in the ACL-reconstructed knee. Based on the results of this study, we do not recommend reconstructing the ALL in ACL-reconstructed knees to decrease internal knee laxity.

Oxygen transport imposes a possible constraint on the brain''s ability to sustain variable metabolic demands, but oxygen diffusion in the cerebral cortex has not yet been observed directly. We show that concurrent two-photon fluorescence imaging of endogenous nicotinamide adenine dinucleotide (NADH) and the cortical microcirculation exposes well-defined boundaries of tissue oxygen diffusion in the mouse cortex. The NADH fluorescence increases rapidly over a narrow, very low pO2 range with a p50 of 3.4±0.6 mm Hg, thereby establishing a nearly binary reporter of significant, metabolically limiting hypoxia. The transient cortical tissue boundaries of NADH fluorescence exhibit remarkably delineated geometrical patterns, which define the limits of tissue oxygen diffusion from the cortical microcirculation and bear a striking resemblance to the ideal Krogh tissue cylinder. The visualization of microvessels and their regional contribution to oxygen delivery establishes penetrating arterioles as major oxygen sources in addition to the capillary network and confirms the existence of cortical oxygen fields with steep microregional oxygen gradients. Thus, two-photon NADH imaging can be applied to expose vascular supply regions and to localize functionally relevant microregional cortical hypoxia with micrometer spatial resolution.  相似文献

Background and purpose

The quality and quantity of bone is important for the success of joint prostheses and may be monitored by dual energy X-ray absorptiometry (DXA). Available protocols suggest that the knee should be positioned in full extension. This is not possible for most patients in the first days after surgery; however, deficits in extension normalize with rehabilitation. Individual knee flexion between the baseline and follow-up investigations may therefore be different. We investigated the sensitivity of bone mineral density (BMD) measurements to knee flexion in a phantom study and in patients. We suggest a protocol for clinical use.

Methods

2 phantom tibial bones with tibia components were secured in a clamp and BMD measurements were repeated 5 times at every 5° change in flexion from 0° to 20°. For clinical use, a soft foam positioner was produced, in which the lower leg could be placed in neutral rotation and with the knee in approximately 25° of flexion. The clinical repeatability was tested with double examinations in 38 patients. We investigated 3 regions of interest (ROIs) below the tibial plateau.

Results

In the phantom study, just 5° of flexion was found to change the measured mean BMD. The reproducibility of clinical measurements (coefficient of variation) in the 3 ROIs assessed ranged from 1.8% to 3.7% for the anteroposterior scans, and from 3.4% to 6.2% for the lateral scans.

Interpretation

Knee flexion does affect the measured periprosthetic tibial BMD, and knee flexion should be the same at all clinical follow-ups. The protocol and soft foam positioner that we suggest permit precise and reliable assessment of BMD in the proximal tibia and they can be used in clinical work.  相似文献

Background and purpose

Radiostereometric analysis (RSA) is a highly accurate tool for assessment of polyethylene (PE) wear in total hip arthroplasty (THA); however, PE wear measurements in clinical studies are often limited to plain radiographs. We evaluated the agreement between PE wear measured with PolyWare software, which uses plain radiographs, and by model-based RSA, which uses stereo radiographs.

Methods

Measurements of PE wear postoperatively and at final follow-up (after mean 6 years) on plain radiographs of 12 patients after cementless THA were evaluated with PolyWare software and the results were compared with those from RSA as the gold standard (Model-based RSA using elementary geometrical shape models; EGS-RSA). With PolyWare, we either used the final radiographic follow-up (PW1) only or both the postoperative follow-up and the final follow-up (PW2).

Results

The 2D mean wear measured (in mm) was 0.80, 1.07, and 0.60 for the PW2, PW1, and RSA method. 2D intra-method repeatability was similar for PW1 and RSA with limits of agreement (LOAs, in mm) of ± 0.22, and ± 0.23, respectively. 2D inter-method concurrent validity was best between PW1 and EGS-RSA with LOAs of ± 0.55. For 2D linear wear measurements, the PW1 method had a clinical repeatability similar to that of RSA.

Interpretation

PW1 is sufficient for retrospective determination of 2D wear from medium-term wear measurements above 0.5 mm, It alleviates the need for baseline plain radiographs, has a clinical precision similar to that of RSA, and is easy and inexpensive to use.Wear of polyethylene (PE) components is widely regarded as the main factor limiting longevity of total hip arthroplasty (THA) (Cooper et al. 1992). Clinical studies have shown that periprosthetic osteolysis and aseptic loosening is strongly related to wear rates of above 0.2 mm/year (Sochart 1999, Dowd et al. 2000).Radiostereometric analysis (RSA) is the most accurate tool for in vivo assessment of PE wear (Kärrholm et al. 1997, von Schewelov et al. 2004, Bragdon et al. 2006), and it is regarded as the gold standard (Ilchmann et al. 1995). However, many radiographic in vivo studies of PE wear in THA are restricted to measurements on plain radiographs because the RSA set-up is expensive and not widely available. Measurement of PE wear on plain radiographs is often limited to 2D analysis because poor quality of cross-table lateral radiographs is a common problem (Sychterz et al. 1999b, 2001). Although PE wear is known to occur multidirectionally (Yamaguchi et al. 1997, Akisue et al. 1999), the bulk of the wear is detectable on the anterior-posterior radiographs alone (Sychterz et al. 1997, Hui et al. 2003, Martell et al. 2003). Based on the availability of radiographs and investigator preferences, some authors favor analysis of serial radiographs (Sychterz et al. 1997, Kim et al. 2001, Hernigou and Bahrami 2003) to describe the pattern of wear and the steady-state wear (Sychterz et al. 1999a, Bragdon et al. 2006), whereas others use 2 radiographic follow-ups (postoperative and latest) (Kraay et al. 2006), or only the latest radiographic follow-up with the assumption of zero wear at baseline (Norton et al. 2002)Little is known about the conformity between PE wear results measured with RSA and computerized methods using plain radiographs (Ilchmann et al. 1995, von Schewelov et al. 2004, Bragdon et al. 2006). Our group has questioned the conformity of 2D PE wear measurements based on serial, 2, or 1 radiographic follow-up (Stilling et al. 2009b). We determined that there was a statistically significant difference between all approaches, but we were unable to determine which strategy best reflected the true extent of wear (Stilling et al. 2009b). In addition, we recently showed that model-based RSA is an accurate tool for measurement of PE wear in good agreement with the true wear (Stilling 2009).We have now studied the intra-method repeatability and concurrent validity between 2 methods (PolyWare and EGS-RSA) for measurement of PE wear in THA, in a group of patients with an average follow-up of 6 years. We wanted to determine (1) whether there would be a difference in repeatability between the methods, (2) whether there would be a difference in wear measured using 1 or 2 radiographic follow-ups with the PolyWare method, and (3) whether either of the 2 PolyWare measurement strategies (1 or 2 radiographic follow-ups) would give results similar to the wear measured by RSA (concurrent validity).  相似文献

Accurate and fast yeast identification is important when treating patients with invasive fungal disease as susceptibility to antifungal agents is highly species related. Matrix‐assisted laser desorption‐time of flight mass spectrometry (MALDI‐TOF‐MS) provides a powerful tool with a clear potential to improve current diagnostic practice. Two MALDI‐TOF‐MS‐systems (BioTyper/Bruker and Saramis/AXIMA) were evaluated using: (i) A collection of 102 archived, well characterised yeast isolates representing 14 different species and (ii) Prospectively collected isolates obtained from clinical samples at two participating laboratories. Of the 102 archived isolates, 81 (79%) and 92 (90%) were correctly identified by Saramis/AXIMA and BioTyper/Bruker respectively. Saramis/AXIMA was unable to separate Candida albicans, C. africana and C. dubliniensis in 13 of 32 isolates. After manual interpretation of the mass spectra output, all 13 isolates were correctly identified, resulting in an overall identification performance of 92%. No misidentifications occurred with the two systems. Of the routine isolates one laboratory identified 99/99 (100%) and 90/99 (91%) to species level by Saramis/Axima and conventional identification, respectively, whereas the other laboratory identified 83/98 (85%) to species level by both BioTyper/Bruker and conventional identification. Both MALDI‐TOF‐MS systems are fast, have built‐in databases that cover the majority of clinically relevant Candida species, and have an accuracy that outperforms our conventional identification systems.  相似文献
