Background
The under-5 mortality rate (U5MR) is an important metric of child health and survival. Country-level estimates of U5MR are readily available, but efforts to estimate U5MR subnationally have been limited, in part, due to spatial misalignment of available data sources (e.g., use of different administrative levels, or as a result of historical boundary changes).Methods
We analyzed all available complete and summary birth history data in surveys and censuses in six countries (Bangladesh, Cameroon, Chad, Mozambique, Uganda, and Zambia) at the finest geographic level available in each data source. We then developed small area estimation models capable of incorporating spatially misaligned data. These small area estimation models were applied to the birth history data in order to estimate trends in U5MR from 1980 to 2015 at the second administrative level in Cameroon, Chad, Mozambique, Uganda, and Zambia and at the third administrative level in Bangladesh.Results
We found substantial variation in U5MR in all six countries: there was more than a two-fold difference in U5MR between the area with the highest rate and the area with the lowest rate in every country. All areas in all countries experienced declines in U5MR between 1980 and 2015, but the degree varied both within and between countries. In Cameroon, Chad, Mozambique, and Zambia we found areas with U5MRs in 2015 that were higher than in other parts of the same country in 1980. Comparing subnational U5MR to country-level targets for the Millennium Development Goals (MDG), we find that 12.8% of areas in Bangladesh did not meet the country-level target, although the country as whole did. A minority of areas in Chad, Mozambique, Uganda, and Zambia met the country-level MDG targets while these countries as a whole did not.Conclusions
Subnational estimates of U5MR reveal significant within-country variation. These estimates could be used for identifying high-need areas and positive deviants, tracking trends in geographic inequalities, and evaluating progress towards international development targets such as the Sustainable Development Goals.32.

Investigations of the changes in the mechanics of the craniomandibular system as a result of treatment have been limited by the lack of validated models of this system. The aims of this project were to (1) validate numerical model predictions of temporomandibular joint (TMJ) eminence morphology and muscle forces produced during molar biting and (2) use the validated models to calculate the changes in TMJ and muscle forces as a consequence of treatment involving orthognathic surgery. Ten volunteers participated; their combined orthodontic and orthognathic surgical treatments were completed. Three-dimensional anatomical data from each subject were used in computer models to predict the sagittal TMJ eminence morphology and joint and muscle forces for each subject, consistent with the neuromuscular objectives of minimizing joint loads and muscle effort. The actual shape of the eminence in each subject was measured with jaw tracking. Surface electromyographic recordings were a measure of the muscle forces involved in static molar biting. Model predictions were compared with measured data from the subjects for eminence shape (R(2) = 0.96) and for muscle activity ratios (R(2) = 0.98). The strength of these relationships validated the models for use in calculating changes in joint loads and muscle forces after treatment. The results suggested that the mechanics of the masticatory system are affected by the combined treatments. The TMJ loads increased in 8 subjects. The average increases in condylar and muscle forces were 4% relative to the applied bite force, but in 1 case the increases were up to 20%. Therefore, although average increases in the forces were small, some persons may experience biologically significant increases in joint and muscle forces as a result of treatment. 相似文献

The present study compared the accuracy of three electronic apex locators (EALs) - Elements Diagnostic?, Root ZX? and Apex DSP? - in the presence of different irrigating solutions (0.9% saline solution and 1% sodium hypochlorite). The electronic measurements were carried out by three examiners, using twenty extracted human permanent maxillary central incisors. A size 10 K file was introduced into the root canals until reaching the 0.0 mark, and was subsequently retracted to the 1.0 mark. The gold standard (GS) measurement was obtained by combining visual and radiographic methods, and was set 1 mm short of the apical foramen. Electronic length values closer to the GS (± 0.5 mm) were considered as accurate measures. Intraclass correlation coefficients (ICCs) were used to verify inter-examiner agreement. The comparison among the EALs was performed using the McNemar and Kruskal-Wallis tests (p < 0.05). The ICCs were generally high, ranging from 0.8859 to 0.9657. Similar results were observed for the percentage of electronic measurements closer to the GS obtained with the Elements Diagnostic? and the Root ZX? EALs (p > 0.05), independent of the irrigating solutions used. The measurements taken with these two EALs were more accurate than those taken with Apex DSP?, regardless of the irrigating solution used (p < 0.05). It was concluded that Elements Diagnostic? and Root ZX? apex locators are able to locate the cementum-dentine junction more precisely than Apex DSP?. This study aimed to assess the capacity of a sodium hypochlorite and citric acid (CA) association (the latter at different concentrations) in removing coronal smear layer (SL) of primary teeth. For this purpose, the pulp chamber roof and floor of 28 primary molars were removed to obtain enamel and dentine disks. SL was produced on the internal walls of the disks using high-speed drills. The disks were irrigated with 1% sodium hypochlorite and citric acid at different concentrations (CA-4%, CA-6%, CA-8% and CA-10%), and with 0.9% sodium chloride. The samples were split and observed under SEM. Scores were attributed to the obtained photomicrographs, according to the amount of SL present. It was noted that all the tested concentrations of citric acid used after the sodium hypochlorite were capable of removing SL. The results were analyzed by the Kruskal-Wallis test, and there was no significant statistical difference among the scores of the groups tested. However, it was observed that CA-8% and CA-10% caused peritubular dentine destruction, and that CA-4% presented a larger number of samples with dense SL. Based on these results, 6.0% citric acid in association with 1% sodium hypochlorite is suggested as auxiliary chemical substances for primary teeth irrigation. 相似文献

BACKGROUND: Periodontitis has been linked to increased risk of cardiovascular diseases. Systemic reactions associated with cardiovascular events may depend on characteristics of the subgingival microflora in periodontitis. Our objectives were to compare the numbers of cultivable bacteria, composition of subgingival microflora and clonal distribution of Actinobacillus actinomycetemcomitans (A. actinomycetemcomitans) in two groups of patients with generalized chronic periodontitis (GCP), one with an acute myocardial infarction (AMI-GCP) and the other one without AMI (non-AMI-GCP). METHODS: In all, 150 dentate individuals were screened for suitability to this study. Subgingival bacterial samples were collected from 11 AMI-GCP and 11 non-AMI-GCP patients who had been selected using strict inclusion criteria in an attempt to exclude confounding factors and to increase comparability of periodontal conditions by matching for periodontal probing depths and attachment levels. Culture methods were used to determine the total viable counts and occurrence and proportions of six periodontal bacterial species and yeasts. Polymerase chain reaction (PCR) technique was used to detect A. actinomycetemcomitans and Porphyromonas gingivalis (P. gingivalis). Intraspecies characterization of A. actinomycetemcomitans included serotyping and genotyping. RESULTS: The mean proportions of P. gingivalis (P = 0.05) and Tannerella forsythensis (T. forsythensis) (P = 0.01) were significantly lower, but the numbers of Micromonas micros (M. micros) and A. actinomycetemcomitans were up to nine times higher and the mean total number of cultivable bacteria per sample higher (P <0.01) in AMI-GCP than in non-AMI-GCP. CONCLUSION: The findings that no target subgingival species were overrepresented but the total bacterial number was higher in AMI-GCP than non-AMI-GCP patients may provide support to the hypothesis that elevated numbers of bacteria in close vicinity to sterile parenteral area present a risk for systemic health. 相似文献

The human oral cavity contains a complex microbial community that, until recently, has not been well characterized. Studies using molecular tools have begun to enumerate and quantify the species residing in various niches of the oral cavity; yet, virtually every study has revealed additional new species, and little is known about the structural dynamics of the oral microbial community or how it changes with disease. Current estimates of bacterial diversity in the oral cavity range up to 700 species, although in any single individual this number is much lower. Oral microbes are responsible for common chronic diseases and are suggested to be sentinels of systemic human diseases. Microarrays are now being used to study oral microbiota in a systematic and robust manner. Although this technology is still relatively young, improvements have been made in all aspects of the technology, including advances that provide better discrimination between perfect-match hybridizations from non-specific (and closely-related) hybridizations. This review addresses a core technology using gel-based microarrays and the initial integration of this technology into a single device needed for system-wide studies of complex microbial community structure and for the development of oral diagnostic devices. 相似文献
