Quantitative ultrasound tissue characterization in shoulder and thigh muscles – a new approach

Background  

The echogenicity patterns of ultrasound scans contain information of tissue composition in muscles. The aim was: (1) to develop a quantitative ultrasound image analysis to characterize tissue composition in terms of intensity and structure of the ultrasound images, and (2) to use the method for characterization of ultrasound images of the supraspinatus muscle, and the vastus lateralis muscle.     

An overview of psychiatric treatment approaches to three offender groups

The chances for successful management of three groups of mentally ill offenders--insanity acquittees, sexual offenders, and offenders with alcohol problems--can be maximized by using specific diagnostic and therapeutic techniques for each group. The recommended model for treatment of insanity acquittees is based on a single administrative organization to oversee treatment programs, which include clearly defined inpatient and outpatient components and careful monitoring of conditional release to the community. Techniques for the diagnosis of sexual offenders include bioimpedance and physiological measures of sex hormone levels and penile tumescence. Medication with antiandrogens and cognitive behavioral treatment using covert sensitization and operant aversion are among the appropriate therapeutic modalities. Treatment of persons charged with alcohol-related crimes should be matched to the severity of the alcohol problem. Legal intervention supplemented by brief treatment programs may be effective for offenders with lesser degrees of alcohol abuse. For offenders with more serious alcohol problems, longer group treatment or detoxification may be necessary. Treatment of alcoholic criminals may be most effective in peer groups in institutional settings, where legal coercion can be used to encourage compliance.     

Sociobiology and antisocial personality. An alternative perspective

The clinical characteristics of antisocial personality disorder are well defined, but poorly explained in developmental or etiological terms. After reviewing basic tenets of sociobiology, a theory of the etiology of antisocial personality is developed. Predictions that flow from this etiological theory are compared with published data for the disorder. These comparisons suggest that sociobiological theory provides a powerful and predictive etiological hypothesis for antisocial personality disorder. Social and clinical implications and avenues for future study of antisocial personality are suggested, as are potential relationships of this theoretical approach to other psychopathological conditions.     

Transcutaneous reduction and external fixation of displaced fractures of the proximal humerus. A controlled clinical trial

A consecutive series of 31 displaced fractures of the proximal humerus were randomly selected for treatment either by closed manipulation or by transcutaneous reduction and external fixation. Follow-up assessed the quality of reduction and healing as well as the functional outcome. The external fixation method gave better reduction, safer healing and superior function.     

Preparative purification of the rat mast cell chymase. Characterization and Interaction with granule components

The rat mast cell granule chymotrypsinlike enzyme was purified to homogeneity from 1 M NaCl solubilized membrane and granule-rich fractions of concentrated rat peritoneal mast cells by a preparative technique utilizing chromatography on Dowex 1, filtration on Sephadex G-75, and affinity chromatography with D-tryptophan methyl ester. Acid disk gel electrophoresis of the purified chymase disclosed a single stained band with activity being eluted from a replicate sliced gel in the same region. SDS-polyacrylamide gel electrophoresis of purified protein gave a single stained band that did not change in position with reduction and alkylation. Mast cell chymase is thus a cationic protein of 25,000 mol wt composed of a single polypeptide chain. The apparent K(m) of the chymase for BTEE was 1.5 x 10(-3) M and the V(max) was 67.8 μmol/min per mg. The enzyme was inhibited by TPCK and not by TLCK. The chymase complexed with native macromolecular rat mast cell heparin in molar ratios of 12:1 and 16:1, and complete heparin uptake occurred at a 40:1 ratio of chymase to heparin. Chymase activity was partially masked by combination with heparin in the isolated granule or experimental chymase-heparin complex, and soluble purified chymase was inhibited by concentrations of 5-HT comparable to those present in mast cells. It is therefore possible that the active site of chymase in the mast cell granule is largely masked by the combined effects of macromolecular heparin and 5-HT.     

Bone marrow circulation after osteotomy. Blood flow, pO2, pCO2, and pressure studied in dogs

We investigated the immediate effect of osteotomy of the femoral shaft in adult dogs on intramedullary pO2, pCO2, relative bone blood flow (RBF) and intraosseous pressure (IOP). Single osteotomy resulted in a pO2 decrease from 49 mm Hg to 30 mm Hg. Double osteotomy further reduced pO2 to 21 mm Hg. Following double osteotomy, pCO2 increased from 42 mm Hg to 55 mm Hg. IOP was decreased by single osteotomy from 29 mm Hg to 15 mm Hg, and to 4 mm Hg by the second osteotomy. Linear regression analysis showed a negative correlation between pCO2 and RBF. We concluded that osteotomy introduces immediate haemodynamic and metabolic changes in the bone marrow canal. A periosteal supply of the endosteal bone seems possible provided this system is intact.
