Cervical metastases of microcystic adnexal carcinoma in an otherwise healthy woman

Microcystic adnexal carcinoma is a rare cutaneous neoplasm characterized by slow but locally aggressive growth, which normally does not lead to systemic metastasis. Frequent local recurrences are reported, which are most likely due to insufficient operative technique. We present the fourth case of cervical ipsilateral metastatic microcystic adnexal carcinoma in an otherwise healthy woman. The patient presented with a previously diagnosed but not completely resected microcystic adnexal carcinoma in the area of the right posterior scalp and two palpable ipsilateral lymph nodes. The tumor was resected using intraoperative snap frozen histological evaluation of the resection borders. In the same procedure two lymph nodes were resected from the right neck. The lymph nodes were histologically assessed and showed infiltration by small strains of tumor cells. After exclusion of a second primary tumor, e.g., mammary carcinoma, as the cause for cervical lymph node metastases, we performed a modified radical neck dissection with resection of the sternocleidomastoid muscle and the accessory nerve, which was histologically proven to be perineurally infiltrated by tumor cells. In this second procedure the histological evaluation of the specimen showed no sign of remaining tumor infiltration. After exclusion of distant metastasis the patient was irradiated with 60 Gy. The patient is well 1 year after the initial treatment without signs of recurrence.     

Acoustic biast trauma

A 50-year old male presented with swollen right eyelids, bleeding per nostrils and a vague left post-auricular swelling for 4 months. Posterior rhinoscopy revealed one pinkish polypoidal mass in the posterior nare and roof of nasopharynx. FNAC from the post-auricular swelling suggested metastatic undifferentiated carcinoma. Incisional biopsy was done form the nasopharynx and histopathological examination proved it to be a malignant paraganglioma. The case is reported for its rarity.     

Hepatitis C virus outside the liver

Observational and some experimental data suggest that hepatitis C virus can manifest in a variety of disease processes outside the liver. The links between hepatitis C virus and membranoproliferative glomerulonephropathy, porphyria cutanea tarda, lichen planus, cryoglobulinemia, diabetes mellitus, B-cell non-Hodgkin’s lymphoma, and thyroiditis are critically reviewed. Based on the available evidence, recommendations for the clinical approach to patients with extrahepatic diseases are discussed.     

Bilateral complete discoid medial meniscus combined with posterior cyst formation

Bilateral discoid medial menisci is an extremely rare condition of the knee and it can be associated to other pathological findings, including anterior portion cyst formation. We report on the clinical features, radiographic findings, treatment and results of one patient who presented a bilateral medial discoid meniscus combined with posterior portion cyst of the left knee. To the best of the author’s knowledge, this is the first case of bilateral medial discoid meniscus associated with posterior portion cyst formation.
