Five Cases of Malignant Lymphoma Associated With Early Gastric Cancer

Five cases of malignant lymphoma associated with early gastriccancer were reported. Two of them were diffuse large lymphoidtypes (DLL) of malignant lymphoma and the rest were Hodgkin'sdisease (HD). Malignant lymphomas were antecedent or coincidentalin all four cases except for one case of DLL. Histological diagnosisof the early gastric cancer was well differentiated adenocarcinomain three cases and mucocellular adenocarcinoma in two cases.All three cases of HD and one case of DLL were over 60 yearsold. The response to purified protein derivative (PPD) was negativein three out of four cases examined. Malignant tumors were foundin relatives of three patients.     

Radionuclide assessment of left ventricular performance on exercise after external conduit operation

Only limited information is available concerning left ventricular (LV) response to exercise after an external conduit operation for cyanotic congenital heart disease. Sixteen patients who had undergone external conduit repair (EC group) were studied with multi-gated cardiac pool imaging using a supine bicycle ergometer on 20 occasions. Six patients with a history of Kawasaki disease without coronary artery stenosis served as controls (control group). Myocardial imaging and cardiac catheterization were also performed in the EC group. There was no significant difference in left ventricular ejection fraction (LVEF) at rest between the groups. However, on exercise, LVEF of the EC group was significantly lower than that of the control group. Nine patients in the EC group showed a perfusion defect (PD) on 12 occasions. LVEF on exercise of the patients with PD was significantly lower than that of the patients without PD. Furthermore, only the patients with PD showed a LVEF decrease of 5% or more in response to exercise. In the EC group, a significant inverse relationship was demonstrated between right ventricular systolic pressure (RVP) and LVEF response to exercise. However, two out of four patients who underwent external conduit replacement improved their LVEF response to exercise with successful reduction of RVP. These findings indicate that an impaired left ventricular response to exercise was common in patients after external conduit operations. Myocardial damage and right ventricular outflow tract obstruction could be the causes of this left ventricular dysfunction.     

Primary Adenocarcinoma of the Esophagus Report of Six Cases

Six primary adenocarcinomas were found among 1,346 cases ofesophageal carcinoma, an incidence of 0.45%. Three of the adenocarcinomaswere the "type ordinaire" of Lortat-Jacob, two were adenoidcystic carcinoma, and one was mucoepidermoid carcinoma. Theinitial symptom was dysphagia in all six cases. All six lesionswere located in the middle and/or lower third of the esophagus.Esophagograms showed tumorous-type lesions in four cases andspiral-type in two cases. Although endoscopical examinationwas performed in five cases, the biopsied specimen showed adenocarcinomasin only two. Four patients were operated and on two receivedradiation therapy. Four patients died within one year, and twoare still alive more than 21 months after surgery.     

COMPARATIVE STUDY OF ABNORMALITY IN GLYCOGEN STORING CAPACITY AND OTHER HISTOCHEMICAL PHENOTYPIC CHANGES IN CARCINOGEN-INDUCED HEPATOCELLULAR PRENEOPLASTIC LESIONS IN RATS

A sequential comparison was made between abnormal glycogen storage and other histochemical phenotypic changes in hepatocellular precancerous lesions (altered foci and neoplastic nodules) during various stages in the process of development of cancer in rat liver. N-2-fluorenylacetamide was fed to male rats for 8 weeks and groups of rats were killed at the end of carcinogen feeding and at 12 and 24 weeks on control diet. Foci rich in glycogen storage accounted for a majority of all foci over the course of experiment, while foci devoid of glycogen storage, which were absent at the end of carcinogen feeding, gradually increased in number during maintenance. Glycogen-deficient lesions that might appear to arise from glycogen-rich lesions displayed hyper-basophilia demonstrated by toluidine blue reaction, but often lacked gamma-glutamyl transpeptidase activity. Resistance to iron accumulation was consistently shown in all precursor lesions for hepatocellular carcinoma in the siderotic liver regardless of abundance or absence of cellular glycogen. It was suggested that properties such as loss of glycogen storing capacity, hyperbaso-philia, and some cellular atypicality resembling those of carcinoma cells might be essential elements for malignant progression.     

Ganglion cells in Ewing's sarcoma following chemotherapy: A case report

A case of Ewing's sarcoma of the bone, arising in the right radius of a 12-year-old girl, which showed unique histologic features after pre-operative treatment, is reported. The light microscopic features of a biopsy sample were those of a small round cell tumor showing positive immunoreaction with antibodies against the product of the MIC 2 gene (O13), neuron-specific enolase, neurofilament, and synaptophysin, but no morphological differentiation. The patient received combined intensive multi drug chemotherapy and radiation before surgery. Examination of the surgical specimen showed that the tumor was less cellular than that in the biopsy specimen, and was composed mainly of loosely textured large cells mimicking ganglion cells, occasionally forming Homer-Wright rosettes. An lmmunohistochemical study revealed that neural differentiation was enhanced. Immunoreactivity for Leu-7 also became positive. Although the patient underwent postoperative chemotherapy, she died of multiple lung and bone metastases 30 months alter the diagnosis. Autopsy showed that metastatic foci were made up of densely packed small round cells like those seen In the biopsy samples, but associated with prominent Homer-Wright rosettes. To the authors' knowledge, this is the first report of a tumor being replaced almost entirely by ganglion cells after preoperative chemotherapy and radiotherapy.     

THE ROLE OF MONOCYTE CHEMOATTRACTANT PROTEIN-1 (MCP-1) IN THE PATHOGENESIS OF COLLAGEN-INDUCED ARTHRITIS IN RATS

Collagen-induced arthritis was produced in rats by intradermal immunization with type II collagen and the expression and production of monocyte chemoattractant protein-1 (MCP-1) were examined by immunohistochemistry, enzyme-linked immunosorbent assay (ELISA), and Northern blot analysis. Two to three weeks after the immunization, the hindfeet showed swelling and redness, followed by the development of severe arthritis, particularly in the ankle joints. During this period, prominent infiltration of neutrophils and macrophages was observed. Sandwich ELISA and Northern blot analysis revealed that MCP-1 concentrations in the joint lavages and MCP-1 mRNA levels in the joint tissues both peaked at 2 weeks after the immunization. By immunohistochemistry, various types of cells, particularly neutrophils, macrophages, synovial cells, and vascular endothelial cells, stained positively for MCP-1. Finally, injection of a neutralizing monoclonal antibody against rat MCP-1 significantly decreased the number of exudate macrophages in the lesions and reduced the ankle swelling by about 30 per cent compared with controls. These results suggest that MCP-1 plays a critical role in this model in the recruitment of monocytes and in the development of arthritis. © 1997 John Wiley & Sons, Ltd.     

AM-3K,A NOVEL MONOCLONAL ANTIBODY SPECIFIC FOR TISSUE MACROPHAGES AND ITS APPLICATION TO PATHOLOGICAL INVESTIGATION

An anti-human macrophage monoclonal antibody, AM-3K, was produced using human alveolar macrophages as antigen. The molecular weights of the antigen recognized by AM-3K were 120 and 70 kD. Immunohistochemically, AM-3K reacted intensely with most macrophages in lymphoreticular organs and in many other organs and tissues. In the spleen, AM-3K reacted with red pulp macrophages, some white pulp macrophages, and tingible body macrophages in lymphoid follicles. In the lymph nodes, many macrophages distributed in the outer cortex, paracortical area, medulla, capsule, or within lymphoid follicles showed an intense reaction for AM-3K. Kupffer cells of the liver, macrophages in the connective tissues, and interstitial macrophages of the kidneys, pancreas, testis, and many other organs were also strongly labelled. AM-3K also reacted with macrophages in many pathological conditions. This antibody, however, did not react with dendritic cell populations, such as epidermal Langerhans cells, interdigitating cells in the paracortex of the lymph nodes, and follicular dendritic cells within the lymphoid follicles, nor with cells other than macrophages, including epithelial cells, vascular endothelial cells, lymphocytes, and granulocytes. Reaction products for AM-3K were found on the cytoplasmic membrane of macrophages by immunoelectron microscopy. In both cryostat sections and formalin-fixed paraffin sections, this monoclonal antibody recognized the antigen present on the cell surface membrane of tissue macrophages, but not monocytes or dendritic cells.     

Brain processing of proper names

Proper names are of practical importance for verbal communication. For instance, they are indispensable for transmission of autobiographical information concerning a person. Recent studies have revealed that proper names are neuropsychologically and anatomically processed in a manner that differs from the processing of common nouns. The current review described proper names, which are labels attached to referents, as being without semantic meaning. The clinical data are presented regarding comprehension and retrieval of people's names, and memory of their unique properties. The validity of a cognitive neuropsychological model of people's name processing is also evaluated. The review suggests that after the initial memory consolidation of a name and a person by the left hemisphere's hippo/parahippocampus, the memory become established in the adjoining fusiform gyrus. Consequently, it is hypothesized that the fusiform gyrus forms a processing network for people's names with the temporal pole and postero temporo/occipital lobe. The research on geographical names, topographical names, and therapeutic approaches for deficits of proper names is also reviewed.     

Further investigation of the epitope recognized by the new monoclonal antibody 2C9

OBJECTIVE: We established a new monoclonal antibody (2C9) that reacted with prostate tissue. The immunohistochemical reactivity of this antibody is similar to anti-prostate-specific membrane antigen (PSMA). Herein, we report the antigenic determinant of 2C9 antibody. METHODS: The reactivity of the antibody was characterized by immunohistochemical staining and the antigen target was characterized by amino acid sequencing after immuno-affinity purification from an LNCaP cell lysate and cloning of a cDNA using a mammalian expression cDNA cloning system. RESULTS: The amino acid and nucleotide sequences for the antigen molecule recognized with 2C9 monoclonal antibody demonstrated identity with PSMA. CONCLUSION: The target molecule of the 2C9 monoclonal antibody is PSMA, pointing to future diagnostic and therapeutic applications.
