目的 :比较NP(NVB DDP)与EP(DDP Vp 16 )方案治疗晚期非小细胞肺癌 (NSCLC)的近期疗效与不良反应。方法 :通过前瞻性对照研究 ,将 72例初治的中晚期NSCLC随机分为治疗组和对照组 ,分别给以NP与EP方案化疗 2个周期 ,按WHO标准评定疗效和不良反应。结果 :两组有效率 (CR PR)各为 50 0 %和 2 7 8%。治疗组高于对照组 (P <0 0 5) ,全身状况治疗组也有明显改善 (P <0 0 5)。不良反应主要表现为骨髓抑制和静脉炎 ,重度白细胞下降和静脉炎发生率各为 55 6 %和2 5 0 %及 16 7%和 9 1% ,治疗组重于对照组 (P <0 0 5)。结论 :NVB DDP是一种安全有效的化疗方案 ,可作为中晚期NSCLC的有效治疗方案。  相似文献

Surface-enhanced Raman spectroscopy (SERS) can be regarded as a powerful tool for probing chemical molecules by effectively enhancing Raman signals. However, the enhancement factors depend on the SERS template, the probed molecular structures, and the excitation laser wavelength. Herein, we proposed a simple and easily fabricated nanostructured template for SERS and analyzed the wavelength-dependent factors. Three types of golden nanopillar arrays on silicon wafers were designed and manufactured. The SERS signals of the Rhodamine 6G (R6G) molecules were extracted. Three laser sources, a blue 17 mW 458 nm diode laser, a green 20 mW 532 nm laser, and a red 6 mW 633 nm laser, were employed as the excitation laser sources. The 458 nm laser was located far from the resonate spectrum of R6G. The optical intensity distributions for the different SERS templates excited by three laser beams were also simulated. The enhancement factors (EFs) of R6G on the three nanostructured templates were measured and compared. The photoluminescence spectrum of the nanostructured templates and SERS signals of R6G were also measured. In addition, the experimental results concerned optical simulations. The analysis tool that was used was a convolution profile of multiple Lorentzian line shapes with a Gaussian profile. It is helpful to understand the SERS signals when the excitation laser wavelength is located out of the resonance region of molecules. It can also provide a new design approach to fabricate an SERS Template with a nanopillar array for different excitation wavelengths.  相似文献

Shiga toxin (stx) is the principal virulence factor of the foodborne pathogen, Shiga toxin-producing Escherichia coli (STEC) O157:H7 and is associated with various lambdoid bacterio (phages). A comparative genomic analysis was performed on STEC O157 isolates from cattle (n = 125) and clinical (n = 127) samples to characterize virulence genes, stx-phage insertion sites and antimicrobial resistance genes that may segregate strains circulating in the same geographic region. In silico analyses revealed that O157 isolates harboured the toxin subtypes stx1a and stx2a. Most cattle (76.0%) and clinical (76.4%) isolates carried the virulence gene combination of stx1, stx2, eae and hlyA. Characterization of stx1 and stx2-carrying phages in assembled contigs revealed that they were associated with mlrA and wrbA insertion sites, respectively. In cattle isolates, mlrA and wrbA insertion sites were occupied more often (77% and 79% isolates respectively) than in clinical isolates (38% and 1.6% isolates, respectively). Profiling of antimicrobial resistance genes (ARGs) in the assembled contigs revealed that 8.8% of cattle (11/125) and 8.7% of clinical (11/127) isolates harboured ARGs. Eight antimicrobial resistance genes cassettes (ARCs) were identified in 14 isolates (cattle, n = 8 and clinical, n = 6) with streptomycin (aadA1, aadA2, ant(3’’)-Ia and aph(3’’)-Ib) being the most prevalent gene in ARCs. The profound disparity between the cattle and clinical strains in occupancy of the wrbA locus suggests that this trait may serve to differentiate cattle from human clinical STEC O157:H7. These findings are important for stx screening and stx-phage insertion site genotyping as well as monitoring ARGs in isolates from cattle and clinical samples.  相似文献

目的 研究HIF-1α及VEGF在哮喘大鼠的表达情况及布地奈德的干预作用.方法 60只雌性SD大鼠随机分为对照组、干预组及哮喘组,以卵清白蛋白激发法制备哮喘模型;HE染色观察各组气道炎症发生及气道结构改变情况;免疫组化测定三个因子的表达.结果 ①哮喘组气道壁平滑肌厚度较对照组和干预组显著增加(P<0.05).②哮喘组HIF-1α及VEGF在大鼠肺组织中的表达较对照组和干预组显著增加(P<0.05).③直线相关性分析显示,气道壁平滑肌厚度与大鼠肺组织中HIF-1α及VEGF的表达水平呈正相关(1分别为0.787,0.776;P<0.05);HIF-1α与VEGF的表达也呈正相关.结论 哮喘大鼠气道HIF-1及VEGF表达均高于正常组,布地奈德可能通过上调这些因子的表达,降低血管通透性从而抑制哮喘气道重塑.  相似文献

目的:对静息状态网络的研究方法、初步的研究成果等作以介绍,并结合静息状态网络在阿尔茨海默病早期预警中的应用,介绍静息状态脑网络的应用。资料来源:应用计算机检索PubMed1980-01/2006-12与静息状态网络相关的文献,检索词“restingstate,functional connectivity”,并限定文献语言种类为“English”;同时计算机检索万方数据库1995-01/2006-12有关方面的文献,检索词为“静息,功能连接,阿尔茨海默病”,并限定语言种类为中文。资料选择:对资料进行初审,选取包括静息状态的相关文献,开始查找原文。纳入标准:①有关静息状态脑网络和功能连接的研究。②有关阿尔茨海默病的研究。排除标准:重复研究。资料提炼:共收集到53篇有关静息状态网络方面的研究,排除23篇重复性研究,30篇符合要求。资料综合:近年来,研究者发现大脑处于无任务的静息状态时,仍然存在着某种功能活动。这些现象表明大脑在静息状态时可能存在有组织的网络。这有助于对人脑高级意识和某些认知疾病的研究,因此,有关这方面的工作越来越受到人们的重视。结论:对静息状态网络的本质和规律的研究还很有限,对这个网络所支持的精确的功能还有待于进一步研究。  相似文献

Objectives

Anti–programmed cell death 1 (PD-1)/programmed death ligand 1 (PD-L1) immunotherapy has demonstrated success in the treatment of advanced NSCLC. Recently, PD-1/PD-L1 blockade also has demonstrated interesting results in small trials of neoadjuvant treatment in stage IB to IIIA NSCLC. In addition, several clinical trials using anti–PD-1/PD-L1 immunotherapy as an adjuvant or neoadjuvant treatment in patients with resectable stage NSCLC are ongoing. However, few analyses of anti–PD-1/PD-L1 immunotherapy–related biomarkers in early-stage squamous cell lung carcinoma (SqCLC) have been reported. In this study, we evaluated PD-L1 protein expression, tumor mutation burden, and expression of an immune gene signature in early-stage SqCLC, providing data for identifying the potential role for patients with anti–PD-1/PD-L1 treatment in early-stage SqCLC.

Methods

A total of 255 specimens from patients with early-stage SqCLC were identified within participating centers of the Strategic Partnering to Evaluate Cancer Signatures program. PD-L1 protein expression by immunohistochemistry was evaluated by using the Dako PD-L1 22C3 pharmDx kit on the Dako Link 48 auto-stainer (Dako, Carpinteria, CA). Tumor mutation burden (TMB) was calculated on the basis of data from targeted genome sequencing. The T-effector and interferon gamma (IFN-γ) gene signature was determined from Affymetrix gene chip data (Affymetrix, Santa Clara, CA) from frozen specimens.

Results

The prevalence of PD-L1 expression was 9.8% at a tumor proportion score cutoff of at least 50%. PD-L1 mRNA and programmed cell death 1 ligand 2 mRNA positively correlated with PD-L1 protein expression on tumor cells (TCs) and tumor-infiltrating immune cells. PD-L1 protein expression on tumor-infiltrating immune cells was correlated with the T-effector and IFN-γ gene signature (p < 0.001), but not with TMB. For TCs, all of these biomarkers were independent of each other and neither PD-L1 protein expression, TMB, or T-effector and IFN-γ gene signatures were independently prognostic for patient outcomes.

Conclusions

Evaluation of PD-L1 expression, TMB, and T-effector and IFN-γ gene signatures in the cohort with early-stage SqCLC found them to be independent of each other, and none was associated with overall survival. Our results also support the hypothesis that PD-L1 expression is regulated by an intrinsic mechanism on TCs and an adaptive mechanism on immune cells.  相似文献

GOALS: This study characterizes cardiovascular autonomic function in women with irritable bowel syndrome (IBS), using standardized techniques. BACKGROUND: Autonomic dysfunction is believed to contribute to abnormal gastrointestinal motility and visceral hypersensitivity in IBS. There is mounting evidence of generalized impairment of autonomic activity in patients with IBS. STUDY: Thirty women aged 39 years (95% C.I. 25-53 years) diagnosed with IBS, and 30 age-matched healthy women were studied. The ratio of low frequency to high frequency heart rate variability domains (LF:HF ratio) was used to represent cardiac sympathovagal activity, and orthostatic testing and sustained isometric handgrip exercise were used as sympathetic stimuli. Parasympathetic activity was represented by the expiratory to inspiratory R-R interval (E:I) ratio during deep breathing at 6 minutes. RESULTS: LF:HF responses to handgrip exercise (316%, C.I. 134% to 498% vs. 107%, C.I. 15% to 153%; P < 0.05) and orthostatic testing (648%, C.I. 520% to 904% vs. 330%, C.I. 140% to 520%; P < 0.05) were higher in IBS patients than controls, and the E:I ratio was significantly lower (1.47, C.I. 1.33-1.61 vs. 1.20, C.I. 1.14-1.26; P < 0.01). CONCLUSIONS: Autonomic cardiovascular function is impaired in IBS, manifest as attenuated cardio-vagal tone, and relative sympathetic excess during stimulated conditions.  相似文献
