Closed reduction with CT-guided screw fixation for unstable sacroiliac joint fracture-dislocation 总被引:2,自引:0,他引:2
Background Unstable posterior pelvic ring fractures and dislocations are uncommon but potentially life-threatening injuries in children. Early definitive management reduces risk of immediate complications as well as chronic pain and gait dysfunction. Conventional operative therapy carries substantial risk of extensive blood loss and iatrogenic neurological and vascular injury. Minimally invasive image-guided intervention may further reduce immediate risk and improve long-term outcome.Objective To describe CT-guided closed reduction and internal fixation (CRIF) and review outcomes of unstable fracture-dislocation of the sacroiliac (SI) joint in children.Materials and methods Between 2000 and 2003, three children (two girls, one boy) age 8–14 years were referred to interventional radiology for treatment of unstable SI joint fracture-dislocation not adequately treated with anterior external fixation alone.Results The three affected SI joints (two left, one right) were treated in a combined approach by pediatric interventional radiologists and orthopedic surgeons, using a percutaneous approach under CT guidance. Over a threaded guiding pin, 7.3 mm cannulated screws were used to achieve stable reduction of the affected SI joints. One screw was removed after slight (2 mm) migration. No neurovascular or other complications occurred. All patients had satisfactory healing with near-anatomic reduction, although recovery of the youngest was delayed by associated spinal injury.Conclusions Compared to open surgical alternatives, CRIF under CT guidance reduces operating time, decreases blood loss, and allows early definitive fixation and immediate non-weight-bearing mobilization with a low rate of complication for unstable posterior pelvic ring fractures. In addition, CT-guided placement of the guide pin may allow safer screw positioning and may minimize the total number of screws needed to achieve pelvic stability. 相似文献

By asking the question 'What is the optimal medical management of infertility and minor endometriosis?', it is assumed that endometriosis has a detrimental effect on fertility. The published data suggest that oocyte dysfunction may contribute to infertility associated with endometriosis. This is expressed as a reduction in fertilization and implantation rates; implantation rates to a lesser extent, though still significant. Other evidence for oocyte dysfunction exists, not all of which is consistent. Suppression of ovulation and menstruation to treat endometriosis-associated infertility is not effective. However, ovulation induction, perhaps with intrauterine insemination, does result in pregnancy rates higher than in control cycles, while stimulated IVF success rates are equivalent to those of other diagnostic groups. For the future, angiogenesis is critical to the support of endometriotic deposits and targeted therapies are promised; their role in improving fertility has not yet been explored. 相似文献

Lung carcinoids occur sporadically and rarely in association with multiple
endocrine neoplasia type 1 (MEN1). There are no well defined genetic
abnormalities known to occur in these tumors. We studied 11 sporadic lung
carcinoids for loss of heterozygosity (LOH) at the locus of the MEN1 gene
on chromosome 11q13, and for mutations of the MEN1 gene using dideoxy
fingerprinting. Additionally, a lung carcinoid from a MEN1 patient was
studied. In four of 11 (36%) sporadic tumors, both copies of the MEN1 gene
were inactivated. All four tumors showed the presence of a MEN1 gene
mutation and loss of the other allele. Observed mutations included a 1 bp
insertion, a 1 bp deletion, a 13 bp deletion and a single nucleotide
substitution affecting a donor splice site. Each mutation predicts
truncation or potentially complete loss of menin. The remaining seven
tumors showed neither the presence of a MEN1 gene mutation nor 11q13 LOH.
The tumor from the MEN1 patient showed LOH at chromosome 11q13 and a
complex germline MEN1 gene mutation. The data implicate the MEN1 gene in
the pathogenesis of sporadic lung carcinoids, representing the first
defined genetic alteration in these tumors.

Fifty women with polycystic ovaries took part in a prospective randomized
study. All women required treatment by in-vitro fertilization (IVF) for
reasons other than anovulation. They had all previously undergone ovarian
stimulation with gonadotrophin therapy which had failed to result in
pregnancy or had been abandoned due to high risk of developing ovarian
hyperstimulation syndrome (OHSS). Twenty-five women were treated by
long-term pituitary desensitization followed by gonadotrophin therapy,
oocyte retrieval and embryo transfer (group 1). Twenty-five women underwent
laparoscopic ovarian electrocautery after pituitary desensitization
followed by gonadotrophin therapy, oocyte retrieval and embryo transfer
(group 2). A significantly higher number of women in group 1 had to have
the treatment cycle abandoned due to impending or actual OHSS, determined
by endocrine and clinical findings. In addition, the development of
moderate or severe OHSS in completed cycles was higher in group 1. The
pregnancy rate and miscarriage rates in the two treatment groups were
similar. The authors propose that laparoscopic ovarian electrocautery is a
potentially useful treatment for women who have previously had an IVF
treatment cycle cancelled due to risk of OHSS or who have suffered OHSS in
a previous treatment cycle.

The bias favoring deletion over inversion in DH-JH rearrangement has been
known for years, but the underlying mechanism has yet to be fully defined.
It has been suggested that the ratio of deletion/inversion is determined by
the combined effect of two factors: (i) the relative strengths of 5' and 3'
recombination signal sequences (RSS) of a DH segment, and (ii) the
efficiency with which the deletional product (one joint) forms relative to
the inversional product (two joints). In this study, we analyzed for the
first time the effect of factor 1 alone on the biased 3' RSS utilization in
DH-JH joining by using deletional plasmids in an extrachromosomal substrate
V(D)J recombination assay. It was found that the 3' RSS and associated
coding end (12 bp) mediate recombination more efficiently than the 5'
RSS/coding end DH-JH plasmids. These results demonstrate that the effect of
the RSS/coding end alone can account, at least partially, for the
predominant deletion in DH-JH recombination. The potential effect of the
relative strength of RSS and associated coding end on the ordered
rearrangement of DH-JH followed by VH to DH-JH was also assessed. When
recombination frequencies of D-->J (3' DH to J3) were compared with
frequencies of V-- >D (VHPJ14 to 3' DH or VHOX2 to 3' DH), it was found
that V-->D joining was, if anything, more efficient than D-->J
joining. Therefore, if all three segments were accessible, RSS/coding end
effects would not contribute to the ordered rearrangement of the IgH locus.

The purpose of this study was to determine if discrepancies exist between upright and inverted seated positions in isometric
maximal voluntary contraction (MVC) elbow flexor force, MVC force produced in the first 100 ms (F100), MVC rate of force development,
electromyographic (EMG) activity of the biceps and triceps as well as heart rate and blood pressure. The results showed significantly
(p < 0.01) higher MVC force (543.6 ± 29.6 vs. 486.5 ± 23.0 N), F100 (328.3 ± 94.5 vs. 274.6 ± 101.8 N), rate of force development
(p = 0.003) (1,851.9 ± 742.2 vs. 1,591.0 ± 719.6 N s−1) and biceps brachii EMG activity (48%, p < 0.01) in the upright versus inverted condition. There were relatively greater co-contractions with the inverted position
(p < 0.01) due to the lack of change in triceps’ EMG and the substantial decrease in biceps’ EMG. There were no significant
changes in trunk EMG activity. With inversion, there were significant decreases in heart rate (16.8%), systolic (11.6%) and
diastolic (12.1%) blood pressures (p < 0.0001). These results illustrate decrements in neuromuscular performance with an inverted seated posture which may be

Ruminant γδ T cells are concentrated at epithelial surfaces and share many features in common with species such as mice and humans which contain relatively fewer γδ T cells than ruminants. To date no γδ T cells with invariant TcR have been found in sheep and the generation of γδ TcR diversity which is thymic dependent follows a developmentally regulated sequence. Analysis of thymic export of γδ T cells shows that emigration of γδ T-cell subsets increases markedly during fetal life and after birth suggesting intrathymic processes leading to mature γδ T cells may change during development. Skin homing γδ T cells acquire their tissue tropism inside the thymus and pathways of recirculation of γδ T cells to skin are laid down during fetal development independent of antigen and remain stable through into adult life. 相似文献

Widespread use of zebrafish (Danio rerio) in genetic analysis of embryonic development has led to rapid advances in the technology required to generate, map and clone mutated genes. To identify genes involved in the generation and regulation of vertebrate circadian rhythmicity, we screened for dominant mutations that affect the circadian periodicity of larval zebrafish locomotor behavior. In a screen of 6,500 genomes, we recovered 8 homozygous viable, semi-dominant mutants, and describe one of them here. The circadian period of the lager and lime (lag(dg2)) mutant is shortened by 0.7 h in heterozygotes,and 1.3 h in homozygotes. This mutation also shortens the period of the melatonin production rhythm measured from cultured pineal glands, indicating that the mutant gene product affects circadian rhythmicity at the tissue level, as well as at the behavioral level. This mutation also alters the sensitivity of pineal circadian period to temperature, but does not affect phase shifting responses to light. Linkage mapping with microsatellite markers indicates that the lag mutation is on chromosome 7. A zebrafish homolog of period1(per1) is the only known clock gene homolog that maps near the lag locus. However, all sequence variants found in per1 cDNA from lag(dg2) mutants are also present in wild type lines, and we were unable to detect any defect in per1 mRNA splicing, so this mutation may identify a novel clock gene. 相似文献

This paper explores the use of selective laser sintering (SLS) for the generation of bone tissue engineering scaffolds from polycaprolactone (PCL) and PCL/tricalcium phosphate (TCP). Different scaffold designs are generated, and assessed from the point of view of manufacturability, porosity and mechanical performance. Large scaffold specimens are produced, with a preferred design, and are assessed through an in vivo study of the critical size bone defect in sheep tibia with subsequent microscopic, histological and mechanical evaluation. Further explorations are performed to generate scaffolds with increasing TCP content. Scaffold fabrication from PCL and PCL/TCP mixtures with up to 50 mass% TCP is shown to be possible. With increasing macroporosity the stiffness of the scaffolds is seen to drop; however, the stiffness can be increased by minor geometrical changes, such as the addition of a cage around the scaffold. In the animal study the selected scaffold for implantation did not perform as well as the TCP control in terms of new bone formation and the resulting mechanical performance of the defect area.
