长沙市小学生焦虑障碍现状调查

目的　初步了解小学儿童焦虑障碍的患病现况。方法　使用焦虑性情绪障碍筛查表 (SCARED)调查长沙市某小学 2～ 6年级的 5 6 5名小学生 ,并对量表总分≥划界分 (2 3分 )的学生进行面谈。结果　在 5 6 5名小学生 (男生2 90名 ,女生 2 75名 )中 ,SCARED筛查阳性的有 14 0名 ,占总人数的 2 4 78%。使用中国精神障碍分类与诊断标准 (第三版 ) (CCMD 3)对筛查阳性者进行面谈 ,发现符合焦虑障碍诊断标准的 32例 (男 15 ,女 17) ,患病率为 5 6 6 % ,其中儿童分离性焦虑症 7例 ,患病率为 1 2 4 % ;儿童广泛焦虑症 11例 (1 95 % ) ;儿童恐惧症 10例 (1 77% ) ;儿童社交恐惧症 14例(2 4 8% ) ;有 8例存在 2～ 3种焦虑障碍共病 ;无 1例诊断为学校恐怖症、选择性缄默症和惊恐发作。结论　在儿童中焦虑情绪存在较普遍 ,这对儿童的学习、行为、自我意识造成不良影响 ,应予以积极干预。     

Taylor-type focal cortical dysplasia: is the epilepsy always resistant to medical treatment?

Patients with Taylor-type focal cortical dysplasia (TTFCD) generally present with medically intractable epilepsy and impaired neurological and/or intellectual functioning. Surgery usually proves to be the only treatment approach leading to control of seizures. We describe a 17-year-old girl with TTFCD who exhibited a very long period of seizure remission. Combined clinical and neuroimaging findings were compatible with a diagnosis of a balloon cell-subtype TTFCD. As for the clinical course, partial motor seizures began at one year of age and ceased at five: our patient has had no seizure recurrence over a 12-year-follow-up. Moreover, throughout the 15-year follow-up, neurological examinations and cognitive abilities always remained within normal limits. Neuropsychological assessment clearly showed no impairments in executive functions: planning abilities, working memory, attention and impulse control, or constructive aspects of motor coordination. The predominant deficits pertained to verbal abilities in the context of borderline intellectual performances. To our knowledge, this case report documents the longest duration of seizure remission in a patient with TTFCD, thus emphasizing the possible benign course of such dysplastic lesions which usually have a poor prognosis, leading to early surgical treatment.     

左主干起源于右冠状动脉窦（附2例报告并文献复习）

目的:分析左主干起源于右冠状动脉窦的临床特征,提高对左主干起源于右冠状动脉窦的认识和诊断.方法:回顾分析2例左主干起源于右冠状动脉窦的临床病例并对文献进行回顾分析.结果:左主干起源于右冠状动脉窦是一种罕见的先天性冠脉畸形,这种畸形可分为良性或恶性,主要根据异常左冠状动脉与主动脉和肺动脉之间的关系.对于可能引起猝死或严重心肌缺血患者,应进行冠状动脉旁路移植术.2例患者左主干起源于右冠状动脉窦,其中1例左主干走行于主动脉与肺动脉之间,1例走行于主动脉前方,这2例未进行预防性外科手术,目前预后良好.结论:左主干起源于右冠状动脉窦是罕见的冠脉畸形,应注意鉴别是良性或是恶性,确定下一步治疗方案.     

人胎盘微绒毛膜的制备及其转铁蛋白受体研究

作者分别用差速离心法及蔗糖梯度离心法制备胎盘微绒毛膜(PMM),用受体放射分析法研究了PMM的转铁蛋白受体(TfB),结果表明:两种方法制备的PMM均可用于受体分析,但差速离心法更为简便。测定60例产妇PMM的TfR,其数目为3.53±1.98×10~(12)个位点/mg膜蛋白,最大结合容量为6.33±4.21×10~(-12)mol/mg膜蛋白,Kd值为4.95±3.39×10~(-9)mol/L。受体结合反应体系最适实验条件为,膜蛋白浓度每管50μg,标记物浓度每管50 000cpm,孵育30分钟,B/F分离的聚乙二醇浓度为12％(W/V)。对TfR的特性研究表明:TfR与转铁蛋白的结合具有高度亲和力、高度特异性及可饱和性。     

营养性驱铅饮料的研究

本文研究营养性驱铅饮料的动物和人群试验。饮料与铅料动物组间比较,饮料组动物的尿、粪排铅量高,分别是对照组的1.5倍和1.1倍;血、肝、肾中含量低;体内 Zn、Cu、Fe 等微量元素基本稳定。其体重增长与对照组相一致。人群试验:试验组尿铅排出明显,试验末(2月后)仅为试验初的21.7%;血铅平均减少0.39μmol/l;血中 Hb、δ-ALAD 和尿中δ-ALA 等均恢复良好;不影响体内Zn、Cu 等微量元素,无不良反应。     

备用根大鼠脊髓后角提取液中神经营养活性组分的分离及生物活性测定

制作备用根大鼠脊髓后角提取液，取其分子量大于10kD的组分进行SDS-聚丙烯酰胺凝胶电泳分析，发现手术侧和非手术侧的电泳区带数目大致相同，但手术侧样品的第四条蛋白质区带扫描的吸收峰面积百分比大于非手术侧样品的第四条区带，两者在量上可能有差别。将手术侧样品经交联葡聚精G-75凝胶层析，得到两个洗脱峰．第一峰洗脱液有促进体外培养的背根节神经元存活及其突起生长的作用，其电泳分析显示4条主带，分子量在40～80kD之间．实验结果提示部分去传入纤维支配的脊髓后角组织含有神经营养活性物质，该物质的分子量约在40～78kD之间．     

BGM细胞,Hela细胞,HEL细胞与A549细胞对HSV—1敏感性的比较

用单纯疱疹病毒－１型（ＨＳＶ－１）感染ＢＧＭ细胞、Ｈｅｌａ细胞、ＨＥＬ细胞及Ａ５４９细胞，利用空斑试验，比较四种细胞对ＨＳＶ－１的敏感性。结果示，ＨＳＶ－１在ＢＧＭ细胞培养中空斑形成单位（ＰＦＵ）数最多，出现细胞病变的时间早。提示ＢＧＭ细胞可为ＨＳＶ－１的分离培养提供一种新的、敏感细胞。     

Safety of GM-CSF in Patients with AIDS: A Review of the Literature

We performed a literature search for all clinical studies reporting outcomes in patients with the acquired immunodeficiency syndrome (AIDS) receiving granulocyte-macrophage colony-stimulating factor (GM-CSF) for any indication. Safety outcomes included human immunodeficiency virus replication, immune status, and frequency of opportunistic infections and neoplasms. Data were synthesized qualitatively. We identified 22 studies (274 patients): 12 addressed AIDS neutropenia, 8 AIDS cancer therapy, and 2 opportunistic infections. Viral burden was assessed by serum p24Ag in 15 studies. Nine reported no change in levels, three net decreases, and three net increases. All studies showing net increases involved patients receiving GM-CSF without a concurrent antiretroviral. The CD4 counts were unchanged in 5 studies, increased in 3, and not reported in 14. The incidence of neoplasms or new opportunistic infections was low. The literature suggests no increased risk of viral replication or clinical deterioration in patients with AIDS who take GM-CSF concurrently with zidovudine.     

Alzheimer disease

In an attempt to determine the risks for and kinds of dementia most prevalent among the African American population, the Alzheimer's Disease Research Center at the University of Pittsburgh developed a community satellite program specifically targeting the African American and historically medically underserved communities in Allegheny County, PA. The primary mission of the Alzheimer Outreach Center (AOC) was to increase the awareness of AD among the targeted population. The number of nonwhite patients participating in the studies at ADRC increased from 2 to 16% in the first year of the program. In May 1995, the AOC became a permanent ADRC program.
