OBJECTIVE: To study skin diseases and hypersensitivity to female sex hormones in patients with the premenstrual syndrome (PMS). STUDY DESIGN: Thirty women answered a questionnaire related to PMS and underwent gynecologic, dermatologic and laboratory examinations. Intradermal testing was performed with estradiol valerate, progesterone and placebo. Desensitization treatment was instituted in 15 patients. RESULTS: Ten patients were diagnosed with PMS and concomitant skin disease, including pruritus vulvae, hyperpigmentation, papular eruption and acne vulgaris (group A). Ten patients diagnosed with PMS but without skin disease served as the first control group (group B). The second control group consisted of 10 healthy women (group C). Immediate and delayed hypersensitivity reactions to sex hormones were observed in all patients with PMS and PMS-related skin diseases (groups A and B) but not in healthy women (group C). Desensitization produced a decrease in PMS symptoms and improvement in the skin disease related to PMS. CONCLUSION: Skin diseases may be a part of PMS. Demonstration of a delayed allergic reaction tofemale sex hormones may uncover a significant pathogenetic mechanism in patients with recurrent skin disease and PMS.  相似文献

The level of oxytocinase in the pregnant woman's blood is a sensitive marker of the efficiency of the hypothalamus/hypophisis/suprarenal glands in mother and fetus, as well as the quantity of hormone production, also in placenta, which determines the duration of pregnancy and the course of labor. In the study, the level of oxytocinase was measured before the labour in 485 pregnant women at risk of premature labour. After the mean duration of pregnancy of 264 +/- 12 days, 353 infants were born fully mature (group I), and 132 with the lowered maturity of 6-9 Klimek points (group II). In group II, the mean pregnancy duration (249 days), body mass (2700 g) and oxytocinase level (5.6 mumol/l/min) were significantly lower and tocolysis was necessary twice as often (41% vs. 21%). Independently of the level of newborn maturity in cases of tocolysis the mean level of oxytocinase was significantly 2.6 times lower (3 +/- 0.8 mumol/l/min) than in the other pregnant women (7.9 mumol/l/min). It proves, that low oxytocinasaemia becomes normal following the ACTH-depot therapy, which eliminates the necessity of tocolysis.  相似文献

The term hypertension in pregnancy stands either for a high blood pressure, which has already developed before pregnancy (i.e., chronic hypertension in pregnancy), or for a pregnancy-associated disease (i.e., pregnancy-induced hypertension). Each form of hypertension may be an isolated phenomenon or constitute a part of the syndrome of preeclampsia or eclampsia. This review focuses mainly on the risk factor assessment, prevention and treatment of hypertension developing during pregnancy. Despite a frequent occurrence of this disease its prevention and treatment is still a subject of debates, and only a limited number of studies, which fulfill the criteria of "evidence-based medicine" have so far been performed in this field. Although the impressive advances in treatment of hypertension in the general population have been done, the choice of drugs and control of hypertension developing during pregnancy is still far from being satisfactory.  相似文献

Anomalous origin of the left coronary artery from the pulmonary artery is a congenital coronary artery malformation most commonly present in infancy. A variety of surgical procedures have been described to achieve physiological correction of the coronary flow abnormalities. These techniques are effective as long as there is potential for myocardial recovery. However the sequelae of chronic myocardial ischemia that characterize this entity often irreversibly damage the heart and preclude correction and palliation of the native anomaly. In this type of setting, heart transplantation is a realistic option. Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) occasionally presents in adulthood. Anatomic repair with a two coronary artery system may not be optimal in patients presenting with ischemic cardiomyopathy. We report an adult patient with platelet factor 4 (PF4) antibodies who underwent orthotopic heart transplantation (OHT) for ALCAPA.  相似文献

We conducted a retrospective study of the clinical aspects of bacteremia caused by methicillin-resistant Staphylococcus aureus (MRSA) with heterogeneously reduced susceptibility to vancomycin. Bloodstream MRSA isolates were screened for reduced susceptibility by using brain-heart infusion agar, including 4 mg/L vancomycin with and without 4% NaCl. Patients whose isolates exhibited growth (case-patients) were compared with those whose isolates did not (controls) for demographics, coexisting chronic conditions, hospital events, antibiotic exposures, and outcomes. Sixty-one (41%) of 149 isolates exhibited growth. Subclones from 46 (75%) of these had a higher MIC of vancomycin than did their parent isolates. No isolates met criteria for vancomycin heteroresistance. No differences in potential predictors or in outcomes were found between case-patients and controls. These data show that patients with vancomycin-susceptible MRSA bacteremia have similar baseline clinical features and outcomes whether or not their bacterial isolates exhibit growth on screening media containing vancomycin.  相似文献

Rationale Latent inhibition (LI) describes a process by which repeated pre-exposure of a stimulus without any consequence retards the learning of subsequent conditioned associations with that stimulus. It is well established that LI is impaired in rats and in humans by injections of the indirect dopamine agonist amphetamine (AMPH), and that this disruption can be prevented by co-administration of either the typical neuroleptic haloperidol (HAL) or the atypical neuroleptic clozapine (CLZ).Objectives Most of what is known of the pharmacology of LI is derived from studies using either the conditioned emotional response or the conditioned active avoidance paradigm. The goal of the present study was to determine whether these results would generalize to the conditioned taste aversion assay.Methods We tested whether AMPH (0.5 mg/kg) pretreatment would disrupt LI of a conditioned aversion to sucrose, and if so, which stage of the procedure is critical for mediating the disruption; in addition, we tested whether HAL (0.2 mg/kg) or CLZ (5.0 mg/kg) could restore such an expected LI disruption.Results We determined that AMPH disrupted LI when it was injected before pre-exposure and prior to conditioning, but not if the rats were injected before either stage alone. When HAL or CLZ was given 40 min before AMPH (before both pre-exposure and conditioning), it blocked LI disruption.Conclusion These results are in line with the pharmacology of LI as derived from other conditioning paradigms. We conclude that the pharmacological regulation of LI in the CTA paradigm is similar to what has been observed previously in the conditioned emotional response and the conditioned active avoidance paradigms.  相似文献

The growth of children is an indicator of health and society's wellbeing. Growth references are useful in monitoring a child's growth, which is a very important part of child care. Poland's growth references are not updated regularly. Although several growth reference ranges have been developed in Poland over recent years, sampling was restricted to urban populations of major cities. Using magnetoencephalography, we investigated the potential of perilesional and contralesional activity to support language recovery in patients with poststroke aphasia. In healthy young controls, left‐lateralized ventral frontotemporal regions responded to semantic anomalies during sentence comprehension and bilateral dorsal frontoparietal regions responded to syntactic anomalies. Older adults showed more extensive bilateral responses to the syntactic anomalies and less lateralized responses to the semantic anomalies, with decreased activation in the left occipital and parietal regions for both semantic and syntactic anomalies. In aphasic participants, we observed compensatory recruitment in the right hemisphere (RH), which varied depending on the type of linguistic information that was processed. For semantic anomalies, aphasic patients activated some preserved left hemisphere regions adjacent to the lesion, as well as homologous parietal and temporal RH areas. Patients also recruited right inferior and dorsolateral frontal cortex that was not activated in the healthy participants. Responses for syntactic anomalies did not reach significance in patients. Correlation analyses indicated that recruitment of homologous temporoparietal RH areas is associated with better semantic performance, whereas higher accuracy on the syntactic task was related to bilateral superior temporoparietal and right frontal activity. The results suggest that better recovery of semantic processing is associated with a shift to ventral brain regions in the RH. In contrast, preservation of syntactic processing is mediated by dorsal areas, bilaterally, although recovery of syntactic processing tends to be poorer than semantic.
