Minor and major complications of arthroscopic synovectomy of the knee joint performed by rheumatologist

The aim of the study was to analyze the type and incidence of major and minor complications resulting from arthroscopy with arthroscopic synovectomy of the knee joint performed by rheumatologist in different rheumatological diseases. METHODS: In a prospective study the incidence of complications in arthroscopic subtotal synovectomy with subsequent tidal flow lavage was analyzed in 201 rheumatic patients. The arthroscopic synovectomies were performed by one team of rheumatologists in the operation room of the University Clinic of Rheumatology in the town of Plovdiv for a period of 8-9 years. RESULTS: The major complications related to the arthroscopic synovectomy included septic arthritis (0.5%) and rupture of the joint capsule with edema of the thigh and leg (1.5%). The minor postarthroscopic complications were infection of the operative skin incision (2.0%), hemarthrosis (3.5%), severe postoperative pain (1.5%) and gout relapse (0.5%). Complete recovery was achieved after complications were treated and the result of the synovectomy was not compromised. CONCLUSION: Arthroscopic synovectomy of the knee joint performed by rheumatologist in in-patients involves low risk of complications among which minor operative and postoperative ones are prevalent. These do not compromise near and late results of the arthroscopic synovectomy.     

双链断裂修复蛋白hKu70缺陷细胞株的建立及其生物学特性

目的 建立并鉴定DNA双链断裂(DSB)修复蛋白hKu70缺陷细胞株,并观察该缺陷细胞的某些生物学效应,用于AKu70基因功能及职业有害因素对DNA双链断裂修复影响的研究。方法 用构建的AKu70基因反义RNA绿色荧光蛋白真核表达载体(pEGFP—CI—K)转染人胚肺成纤维细胞(HLF),用蛋白兔疫印迹法鉴定转染细胞中AKu70基因的表达水平。同时观察转染细胞生长形态,绘制生长曲线,软琼脂培养法鉴定恶性程度。结果 pEGFP—CI—K载体在转染细胞内可较稳定表达,hKu70蛋白缺陷细胞株AKu70基因的蛋白表达水平下降了42％,转染后hKu70蛋白缺陷细胞生长形态、生长速度无明显变化,软琼脂培养未见细胞集落。结论 成功建立和鉴定了hKu70蛋白缺陷细胞株,该缺陷不足以单独引起可观察的某些生物学效应。     

Methods of hormone replacement therapy in hypertensive women

A comparative retrospective study was made to examine influence of replacement therapy by estradiol valeriate and climonorm on perimenopausal hypertension. The test group (25 hypertensive women) received climonorm for 6 months. Control women received hypotensive therapy. Lipid metabolism and risk of vascular events were assessed. The replacement therapy had a positive effect on lipid metabolism. Criteria of Mann-Whitney, Friedmann, Wilkinson and of chi 2 were made use of. The target arterial pressure was achieved only with hypotensive therapy which also diminished intensity of menopausal disorders. To determine individual policy, it is necessary to stratify risk of cardiovascular events. The best effect is achieved at combination of hypotensive therapy with replacement hormone treatment.     

Diagnosis and surgical treatment of mediastinal neoplasms

The authors share their experiences with examination and surgical treatment of 163 patients with various neoplasms of the mediastinum. The first place among the neoplasms is occupied by tumors of the lymphatic apparatus (33%), the second--by tumors of the thymus (21%). The video-thoracoscopic technique was used in 17 of 131 operations performed. Complications after the operations took place in 6 patients (4.5%), two patients died (1.5%). A conclusion is made that the patients of this category must be treated not only by thoracic surgeons but also by hematologists, neurologists, neurosurgeons and specialists in radiation therapy.

头孢克肟胶囊人体药动学研究及生物等效性评价

目的：研究头孢克肟胶囊在健康人体内的药动学及生物等效性。方法：以18名健康志愿者为试验对象，采用同体交叉试验方法，分别单剂量口服受试制剂或参比制剂各400mg，采用HPLC法测定血浆中药物浓度。结果：参比制剂和受试制剂的tmax分别为（3．39±0．50）和（3．39±0．50）h，cmax分别为（2．33±0．33）和（2．27±0．25）μg／mL，t1／2分别为（3．92±0．85）和（4．41±0．68）h，AUC0～16分别为（14．61±2．67）和（14．82±2．02）μg·h·mL^-1，AUC0～∞分别为（16．00±3．13）和（16．81±2．62）μg·h·mL^-1。结论：受试制剂与参比制剂生物等效，受试制剂的相对生物利用度为（98．69±13．13）％。     

高强度聚焦超声治疗不能手术胰腺癌的1年生存分析：中国和保加利亚的多中心临床研究

目的:通过多中心临床研究分析高强度聚焦超声治疗不能手术胰腺癌的1年生存情况。方法:2013年1月到2014年1月,中国和保加利亚的不能手术的胰腺癌患者32例,男/女=20/12,年龄41~81(59.8±9.1)岁。胰腺癌病灶最大径20~60(39.3±9.6)mm,远处转移/无转移的患者18/14例。接受HIFU和/或化疗。记录术后并发症,疼痛变化和生存情况。使用Kaplan-Meier法计算总生存率和中位生存时间,比较化疗与否、是否发生远处转移的患者生存率有无差异(Log-Rank检验)。结果:术后1月,皮肤浅Ⅱ°烧伤者2例,经保守处理后1~2周愈合。伴有胰腺癌相关性疼痛症状的患者30例与术前比较,减轻的24例(80.0%),不变的5例(16.7%),加重的1例(3.3%)。所有患者的1年生存率为38.4%,中位生存时间为12个月。接受辅助化疗的患者1年生存率为57.4%,中位生存时间12月;未接受辅助化疗的患者1年生存率为20.8%,中位生存时间为6月;二者间比较有显著性差异。伴有远处转移的患者1年生存率为0%,中位生存时间为7月;无远处转移的患者1年生存率为49.2%,中位生存时间为12月;二者间比较无统计学差异。结论:中国和保加利亚的不能手术胰腺癌患者均能安全完成HIFU治疗,1年生存率和中位生存时间优于其它非手术治疗手段。辅助化疗能增加生存受益,远处转移是预后不良因素。     

In vivo testing of Enterogenin for a mutagenic activity of bone marrow hemopoietic tissue of mice.

The purpose of the present study was to test Enterogenin for genotoxicity. The micronuclearic test of Schmid W. was used. Enterogenin was introduced intraperitoneally, once and/or twice in two doses on male and female mice of the BDF 1 hybrid line. The positive controls were injected with clastogenic agent. Bone marrow smear preparations were made, determining the ratio between the normochromic erythrocytes (NCE) and polychromic erythrocytes (PCE), and the number of micronuclearic erythrocytes (MNE). Enterogenin has no genotoxic effect on the hemopoietic tissue of bone marrow of mice the ratio NCE/PCE being within the reference values. The preparation showed no clastogenic effect (p > 0.05), compared to the controls, as the number of PCE with micronuclei did not change 24, 48 and 72 hours after treatment with the two studied doses.
