Reconstructive microsurgery is a clinical technique used to transfer large amounts of a patient''s tissue from one location used to another in order to restore physical deformities caused by trauma, tumors, or congenital abnormalities. The trend in this field is to transfer tissue using increasingly smaller blood vessels, which decreases problems associated with tissue harvest but increases the possibility that blood supply to the transferred tissue may not be adequate for healing. It would thus be helpful to surgeons to understand the relationship between the tissue volume and blood vessel diameter to ensure success in these operations. As a first step towards addressing this question, we present a simple mathematical model that might be used to predict successful tissue transfer based on blood vessel diameter, tissue volume, and oxygen delivery.  相似文献

Porokeratosis is a disorder of clonal hyperproliferation of keratinocytes with several different clinical manifestations. Cutaneous lesions vary in their appearance and distribution. Study Objective

To evaluate the effect of pneumoperitoneum and head position during laparoscopic surgery on intracranial pressures (ICPs) using sonographic measurements of optic nerve sheath diameter (ONSD).

Design

Prospective observational study (Canadian Task Force classification II-1).

Setting

A tertiary-level hospital.

Patients

Sixty-one women aged 15 to 50 years with American Society of Anesthesiologists grade 1 risk and body mass index?≤?29?kg/m2 were admitted to the hospital between November 2015 and October 2016 for elective laparoscopic surgery and were included in this study.

Intervention

Patients were placed in the Trendelenburg position with head down (group I; n?=?33) and reverse Trendelenburg position with head up (group II; n?=?28).

Measurements and Main Results

ONSD was measured via sonography at 4 time points: at baseline before pneumoperitoneum, after pneumoperitoneum, after patient was placed in respective position, and once pneumoperitoneum was released. Patient demographics were comparable in all respects. ICP as indicated by ONSD showed a significant increase after pneumoperitoneum (p?=?.0001 in group I and p?=?.0011 in group II). When patients were placed in either head position, ONSD showed a further increase in ICP. This increase was more pronounced in patients assuming the head-down Trendelenburg position compared with patients in reverse Trendelenburg (head-up) position. Baseline and preoperative ONSD measurements were not reached even after 5 minutes of desufflation.

Conclusions

Pneumoperitoneum causes an increase in ICP. The patient position, either head up or head down as in gynecologic laparoscopic procedures, further worsens ICP. ONSD does not revert back to baseline until 5 minutes after desufflation.  相似文献

Aim

To analyze tear cytokines levels and their correlation to ocular surface parameters in allogenic hematopoietic stem cell transplants (allo-HSCT) patients.

Methods

Prospective longitudinal study of allo-HSCT patients and controls for ocular surface evaluation (OSDI, TBUT, Schirmer's test, staining scores), tear biochemical analysis for protein, cytokines [IL-10, IL-12, IL-2, IL-4, IL-6, IL-17, interferon (IFN)-gamma, tumor necrosis factor (TNF)-alpha, VEGF], MMPs [MMP 2, 9, 7, 13, 10 and chemokine (IL-8)], & VEGF on three consecutive follow up visits (at three monthly interval) was done.

Results

Of 24 post allo-HSCT patients (19 males, 5 females) & 12 controls (mean age 34.3 + 5.8 years) enrolled, 20 patients [mean age 33.4 + 7.77 years; mean time of recruitment of 5.2 + 2.12 months following alloHSCT] who completed three consecutive follow up visits were included for analysis. Ocular GVHD (oGVHD) was seen in 8 patients (33.3%). Tears biochemical analysis showed elevated levels of interferon γ, IL 6, IL 8, IL 10, IL 12AP70, IL 17A, MMP 9 and VEGF in oGVHD eyes as compared to non-oGVHD & control eyes. Non-oGVHD eyes showed elevated tear MMP 7 and MMP 9 as compared to healthy controls. Tear protein levels were significantly decreased in oGVHD eyes and were equivocal in nonGVHD and control eyes. TBUT and ocular staining scores to correlate best with tear interleukins and MMPs.

Conclusion

Evaluation of levels of tear VEGF, total protein & MMP 9 can be of significance in identifying oGVHD in post alloHSCT patients.  相似文献

A detailed screening of bacterial isolates from the Central Himalayan region for plant growth promotion and antimycelial activity against Pythium and Phytophthora strains afforded seven isolates, of which three were particularly effective against the incidence of damping-off in field trials on chilli and tomato. In this investigation an initial spectroscopic survey of the methanolic extracts of the seven bacterial isolates showed complex mixtures except for Pseudomonas sp. GRP3, one of the most promising isolates on the basis of field studies. Strain GRP3 was selected for structural characterization of its secondary metabolites, particularly glycolipids. The extracellular secondary metabolites were enriched by Amberlite XAD-16 adsorber resin followed by separation and structural analysis using TLC, LC-MS, MS-MS, and NMR spectroscopy. Acquired data show the presence of a number of mono- and dirhamnolipids and include rhamnose (Rha)-C8-C10, Rha-C10-C8, Rha-C10-C10, Rha-C10-C12:1, Rha-C10-C12, Rha-Rha-C8-C10, Rha-Rha-C10-C10, Rha-Rha-C10-C10:1, Rha-Rha-C10-C12, Rha-Rha-C10-C12:1, Rha-Rha-C12-C12:1, and Rha-Rha-C12-C12 in strain GRP3. Since rhamnolipids are involved in the lysis of the plasma membrane of zoospores of fungi, application of such rhamnolipid-producing rhizobacteria could facilitate control of damping-off plant pathogens.  相似文献
