目的 探讨用Amplatzer房间隔缺损封堵器经导管堵闭部分边缘缺乏或不足(<5mm)的房间隔缺损的可行性。方法 1999年7月至2 0 0 1年9月对芝加哥大学儿童医院的2 3例边缘<5mm的房间隔缺损患儿试用Amplatzer封堵器进行堵闭,其中男10例,女13例,平均年龄10 7岁。2 0例经食管超声心动图或心内超声心动图测量的房间隔缺损前缘0~4mm ,2例下缘为2mm ,1例后缘为2mm。同期,4 8例房间隔缺损边缘>5mm的患儿作对照。结果 全部2 3例边缘<5mm的房间隔缺损患儿和4 8例对照儿均成功安装了封堵器。两组间年龄、缺损大小和所用封堵器大小差异无显著性意义(P >0 . 0 5 )。2 3例边缘<5mm的患儿中17例(73. 9% )即刻完全堵闭,对照组4 4例(91 .7% )即刻完全堵闭(P <0 .0 5 ) ;2 4h复查边缘<5mm患儿及对照组的完全堵闭率分别是91 3%和93 8% (P >0. 0 5 ) ;6个月复查两组完全堵闭率分别是95 7%和95 8% (P >0 . 0 5 )。缺损边缘<5mm患儿及对照组手术时间分别为(72 . 5±2 6 .5 )min和(6 1. 4±2 1 .9)min ;X线暴光时间分别为(12 .6±7 .0 )min和(9. 8±4 . 2 )min ,与对照组比较手术时间稍长。两组患儿术中和术后均未出现并发症。结论 前、后或下缘<5mm的房间隔缺损仍可以用Amplatzer房间隔缺损封堵器经导管堵闭,但长期手术  相似文献

患儿女 ,4岁 10个月。因心律失常安装室内除颤器入院。患儿第 1胎第 1产 ,足月顺产 ,产前胎儿超声检查发现心功能降低 ,疑及肌部室间隔缺损。生后 1d超声检查示左心室壁中至重度增厚 ,呈海绵样改变 ,轻度左室功能降低。给予地高辛口服 ,门诊随访。患儿 6个月时 ,无诱因哭闹、烦躁 ,进而青紫 12h ,在当地门诊心电图检查诊断为室性心动过速 ,急诊入院。入院时患儿面色青紫 ,心律不齐 ,血压 96 / 34mmHg (1mmHg =0 133kPa) ,心电图示间断心动过速 ,室率 2 5 0~ 2 70次 /min ,遂停用地高辛 ,改用利多卡因 7mg静脉滴注 (简称…  相似文献

RATIONALE AND OBJECTIVES: To determine and analyze the patterns of practice of nonvascular interventional procedures in academic centers in the United States. MATERIALS AND METHODS: A survey was administered via e-mail to the chief residents of 112 academic radiology departments in the United States. Recipients of the survey were asked to identify which sections within their respective radiology departments performed each of 43 types of nonvascular interventional procedures. An additional entry for performance of procedures by non-radiologists was provided. A total of 66 (59%) recipients responded. The statistical method used was the analysis of contingency tables. RESULTS: Percutaneous abdominal biopsies are performed mainly by abdominal and body imaging sections (43/66, 65%), followed by vascular/interventional sections (21/66, 32%). Percutaneous abdominal drainages are performed mostly by abdominal, body imaging, and computed tomography sections (40/66, 61%), followed by vascular/interventional sections. Fluoroscopically guided procedures were performed most commonly by vascular/interventional sections, including percutaneous gastrostomy (40/66, 61%), percutaneous nephrostomy (42/66, 64%), and biliary interventions such as percutaneous transhepatic cholangiography (47/66, 71%). Breast and musculoskeletal procedures are performed by their respective sections most frequently. Non-radiologists perform a significant portion of certain types of procedures: paracenteses, thoracenteses, biliary interventions (particularly stone extractions), enterostomies (particularly percutaneous jejunostomies and cecostomies), and certain biopsies (kidney and prostate). CONCLUSIONS: Academic US radiology sections perform nonvascular interventional radiology procedures in a complex and nonuniform manner. The vascular/interventional sections and organ system and modality (especially CT) sections perform the bulk of the procedures included in the survey. Breast imaging sections predominate in procedures in their disciplines. A substantial number and amount of interventional radiology procedures are performed by non-radiologists.  相似文献

The natural history of renal function following orthotopic heart transplant   总被引:2,自引:0,他引:2  
BACKGROUND: The outcome of solid organ transplantation has dramatically improved after the introduction of the calcineurin inhibitor cyclosporine. With the increasing longevity of heart transplant recipients, the long-term effects of cyclosporine on renal function have become more evident. The natural history of kidney function following orthotopic heart transplant is not well defined and long-term follow up studies are scant. METHODS: We conducted an observational study on patients who received a heart transplant at Saint Louis University Hospital between January 1, 1983 and December 31, 1988. Patients were followed up for 15 yr or until death whichever occurred first. In order to assess the effect of heart transplantation and cyclosporine exposure on long-term renal function we restricted the statistical analysis to patients who survived the first year post-transplantation. RESULTS: A total of 68 patients received orthotopic heart transplants at Saint Louis University Hospital between 1983 and 1988. Forty-eight (71%) patients survived for more than 1 yr. All patients were treated with cyclosporine based triple immunosuppressive regimen, with gradual cyclosporine dose reduction over time. The mean duration of follow-up was 8 yr. The estimated GFR at 5 and 10 yr post-transplant were significantly lower than estimated GFR at baseline and 1 yr post-transplant. There was no significant difference between estimated GFR at 15 yr and estimated GFR at baseline or 1 yr post-transplant. The cumulative incidence of chronic renal failure (GFR < or = 29 mL/min/1.73 m2) at 5, 10 and 15 yr was 4.2, 10.4 and 12.5%, respectively (p < 0.05). The cumulative incidence of severe chronic renal failure (GFR < or = 15 mL/min/1.73 m2) at 5, 10 and 15 yr was 2.1, 8.3 and 8.3%, respectively. The mortality rate was 8, 37, and 52% at 5, 10, and 15 yr, respectively. The 10 and 15 yr survivors had an estimated GFR at 1 yr post-transplant that was significantly higher than the non-survivors. Age, pre-transplantation estimated GFR, pre-transplantation diabetes and pre-transplantation hypertension are risk factors associated with > or = 10 mL/min/1.73 m2 decrement in estimated GFR. CONCLUSION: Heart transplant survivors beyond the first year post-transplant have a significant decrease in renal function and significant mortality observed over time. Age, pre-transplant GFR, pre-transplant diabetes and pre-transplant hypertension are important risk factors for decrement in renal function.  相似文献

BACKGROUND: The National Kidney Foundation has recently published the Kidney Disease Outcomes Quality Initiative (K/DOQI) Clinical Practice Guidelines for Bone Metabolism and Disease in Chronic Kidney Disease (CKD). According to these guidelines, in patients with stage 5 CKD, the adjusted calcium level should be 8.4- 9.5 mg/dl, the serum phosphate should be 3.5-5.5 mg/dl, the calcium phosphorous product should be <55 mg(2)/dl(2) and the intact parathyroid hormone (PTH) level should be 150-300 pg/ml. METHODS: In order to evaluate our ability to meet these targets, we reviewed laboratory parameters of bone and mineral metabolism of 140 patients over a 6-month period in an inner city hemodialysis unit. Serum calcium and phosphate levels were determined using standard assays and PTH levels were determined using the Nichols Intact PTH assay. RESULTS: We found that the levels of serum calcium and serum phosphorus fell within the range recommended by the K/DOQI guidelines 49 and 36% of the time respectively. 57% of the determinations for calcium x phosphorus product were <55 mg(2)/dl(2). PTH levels were within the recommended values in 20% of the determinations. Only 7% of the determinations met all four criteria simultaneously in spite of meeting other K/DOQI targets such as hematocrit and dialysis adequacy. CONCLUSION: These data indicate that current practice for the management of bone and mineral metabolism in hemodialysis falls far short of meeting the K/DOQI guidelines.  相似文献

Isolated reports of brucellosis among family members have been documented. The aim of this study is to determine if active serological screening of the households' members of acute brucellosis cases will detect additional unrecognized cases. From May 2000 to October 2001, patients with acute brucellosis were enrolled and their household members were serologically screened for brucellosis using the Standard Agglutination Test (SAT). Fifty-five index cases with acute brucellosis and 404 household members were enrolled. The majority of index cases (48%) were young adults, and 79% were illiterate. Ownership of animals and ingestion of unpasteurized raw milk were reported by 45 and 75% of the index cases respectively. Of the 55 families screened, 23 (42%) had two family members or more with serological evidence of brucellosis and 32 (58%) had only the index case. Households of > or = 5 members and a history of raw-milk ingestion by family members were risk factors associated with the seropositives (P < 0.05). Of the 404 household members screened, 53 (13%) were seropositive; of these 39 (74%) were symptomatic, and 9 (35%) had brucella bacteraemia. Symptomatic seropositives tended to have bacteraemia and higher brucella antibody titres compared to asymptomatic seropositives (P < or = 0.05). Screening family members of an index case of acute brucellosis will detect additional cases.  相似文献

BACKGROUND: Cardiac dysfunction after congenital heart surgery is a major cause of morbidity and mortality. Cardiac resynchronization through multisite ventricular pacing (MSVP) improves cardiac index and ventricular function, and lowers systemic vascular resistance (SVR) in adults with heart failure and interventricular conduction delay. METHODS: The acute hemodynamic effects of MSVP after congenital heart surgery were assessed. Twenty-nine patients (aged 1 week to 17 years) with prolonged QRS interval had atrial and ventricular unipolar epicardial temporary pacing leads placed at surgery. Group 1 consisted of patients with a single ventricle (n = 14); group 2 included patients with two-ventricle anatomy (tetralogy of Fallot, ventricular septal defect) undergoing ventricular surgery (n = 10); and group 3 included patients with two-ventricle anatomy undergoing other cardiac surgery (n = 5). At a mean postoperative day 1 (range, 0 to 6), blood pressure, systemic and mixed venous oxygen saturations, electrocardiograms, and echocardiograms were obtained before and after 20 minutes of MSVP. RESULTS: The QRS duration decreased with MSVP in all patients (mean, 23%, p < 0.005). Systolic blood pressure improved in all patients (mean, 9.7%, p < 0.005). Cardiac index improved in 19 of 21 patients tested, with no change in 2 patients (mean, 15.1%, p = 0.0001). In 2 patients, MSVP facilitated weaning from cardiopulmonary bypass. Echocardiographic mitral or tricuspid valve inflow was not significantly different with MSVP. CONCLUSIONS: Multisite ventricular pacing results in improved cardiac index and increased systolic blood pressure, and it can also facilitate weaning from cardiopulmonary bypass. Multisite ventricular pacing may be used as adjunct to standard postoperative treatment of cardiac dysfunction after congenital heart surgery.  相似文献

OBJECTIVE: To study the components of occipital seizures using independent component analysis (ICA) of subdurally recorded electroencephalogram (EEG) data. METHODS: Twenty-seven subdurally recorded occipital seizures from eight patients were studied. ICA was performed, and the resulting independent components (ICs) were compared with respect to their power, frequency, degree of spread within the epileptogenic zone, and propagation patterns. The ictal ICs were further subdivided into propagating and non-propagating types. RESULTS: ICA provided information over and above that supplied by standard visual EEG analysis, confirming that all the seizures originated in the occipital lobe, and affording additional knowledge regarding the nature of the seizure onset zone. Each seizure was composed of multiple ICs, some of which propagated while others remained within the epileptogenic zone. There was no statistical difference between the propagating and non-propagating ICs with respect to power or frequency. However, propagating ICs involved a significantly greater number of recording electrodes at their onset when compared to the non-propagating ICs. CONCLUSIONS: The propagation likelihood of ICs in occipital seizures is independent of their power or frequency, but it is dependent on the volume of brain giving rise to that signal; ICs that are generated by a greater volume of brain within the epileptogenic zone are more likely to propagate. SIGNIFICANCE: This study shows that ICA has the potential to be used to redefine the epileptogenic zone and guide the extent of cortisectomy for the treatment of patients with medically intractable occipital epilepsy.  相似文献
