Factors affecting the outcome of distal realignment for patellofemoral disorders of the knee 总被引:1,自引:0,他引:1
This study correlated the risk factors with the clinical outcome of distal realignment for patellofemoral disorders in 48 patients with 53 knees with 25 to 96 months follow-up. The indications for surgery included pain and disability due to patellofemoral disorders with failure of at least 6 months of conservative treatments. The evaluations included pain scores, Lysholm functional scores and radiographs of the knee. The overall results were satisfactory in 47 knees (88.7%) and unsatisfactory in six knees (11.3%). There was no correlation of the clinical results with age, sex, body weight and body height, preoperative pain scores and Lysholm scores. However, the clinical outcome correlated with the severity of articular damage and the correction of patellar malalignment. Error in patient selection and inadequate surgical technique were attributable to poor outcomes. 相似文献

We sought to determine whether identification of poor-risk subgroups of diffuse large B-cell lymphoma (DLBCL) using immunohistochemical stains would have practical utility with regard to prognosis and therapeutic decisions. Tissue microarray blocks were created using replicate samples of formalin-fixed, paraffin-embedded tissue from 200 cases of de novo DLBCL. The sections were stained with antibodies to proteins that are expressed by activated or proliferating B cells including MUM1, FOXP1, bcl-2, survivin, protein kinase C-beta (PKC-beta), cyclin D2, cyclin D3, and Ki-67. In univariate analysis, tumor expression of cyclin D2 (P = 0.025) or PKC-beta (P = 0.015) was associated with a worse overall survival, whereas none of the other markers was predictive of overall survival. Patients with DLBCL that expressed either cyclin D2 or PKC-beta had a 5-year overall survival of only 30% as compared to 52% for those who were negative for both markers (P = 0.0019). In multivariate analysis, the expression of cyclin D2 or PKC-beta was an independent predictor of poor overall survival (P = 0.035). Cyclin D2 and PKC-beta expression will be useful in designing a 'biological prognostic index' for patients with DLBCL. 相似文献

Neopterin and C-reactive protein (CRP) concentrations were determined in serum samples from 129 severe acute respiratory syndrome (SARS) patients and 156 healthy blood donors. In the patients with confirmed SARS, an early neopterin elevation was detected already at the day of onset of symptoms and rose to a maximum level of 45.0 nmol/L 3 days after the onset. All SARS patients had elevated neopterin concentrations (>10 nmol/L) within 9 days after the onset. The mean neopterin concentrations were 34.2 nmol/L in acute sera of SARS patients, 5.1 nmol/L in convalescent sera, and 6.7 nmol/L in healthy controls. In contrast, the mean CRP concentrations in both acute and convalescent sera of SARS patients were in the normal range (<10 mg/L). Serum neopterin level in SARS patients was associated with fever period and thus the clinical progression of the disease, while there was no significant correlation between the CRP level and the fever period. Serum neopterin may allow early assessment of the severity of SARS. The decrease of neopterin level was found after steroid treatment, which indicates that blood samples should be collected before steroid treatment for the neopterin measurement. 相似文献

We describe the specific silencing of reporter gene lacZ in FHM cells (muscle cells of fathead minnow, a fish cell line) by either expressing small hairpin RNAs (shRNAs) from plasmids or transfecting small interfering RNAs (siRNAs) transcribed in vitro. Two types of dsRNAs could inhibit reporter gene expression, and siRNAs were more effective, while both of them worked very well in HeLa cells. siRNAs were tested for silencing expression of the major capsid protein (MCP) encoded by tiger frog virus (TFV), an iridovirus causing severe disease in fish. siRNAs targeting mcp gene effectively inhibited TFV replication in fish cells as demonstrated by reduced mcp RNA level, postponed emergence of cytopathogenic effect, as well as reduced TFV titer and particles in cells. The results suggest that the siRNA method suppressed TFV efficiently in fish cells, providing a potential approach to the therapy of aquaculture viral diseases. 相似文献

An association between functional polymorphisms of genes resulting in decreased detoxification of carcinogens or DNA repair and aberrant promoter methylation is an attractive hypothesis in lung carcinogenesis. The genotypes at polymorphic sites of the glutathione S-transferase (GST) M1 (null/wildtype) and P1 (nucleotide 2627 A/G), myeloperoxidase (MPO) (nucleotide -463 G/A), X-ray repair cross-complementing group 1 (XRCC1) (nucleotides 26304 C/T; 28152 G/A), and NADPH quinine oxidoreductase (NQO1) (nucleotide 609 C/T) genes in 75 Chinese patients with non-small cell lung cancer (NSCLC) were characterized with polymerase chain reaction-restriction fragment length polymorphism. Results were correlated with aberrant methylation of the CDKN2A (alias p16(INK4A)), retinoic acid receptor beta (RARB), methylguanine-DNA methyltransferase (MGMT), and death-associated-protein (DAP) kinase genes in the tumors. In comparison with an age-matched control, none of the polymorphisms were associated with increased lung cancer risks. In male patients, however, the MPO -463 GG homozygous state was associated with CDKN2A (alias p16(INK4A)) methylation (odds ratio OR=3.63, 95% confidence interval CI=1.26-10.51), and the XRCC1 26304 T allele in the heterozygous/homozygous state was associated with methylation of CDKN2A (OR=6.13, 95% CI=1.55-24.16) and RARB (OR=7.67, 95% CI=1.62-36.18). In female patients, the GSTP1 G allele in the heterozygous/homozygous state was associated with RARB methylation (OR=18.0, 95% CI=0.76-427.29). These results showed that functional deficiencies in metabolic pathways that protect cells from carcinogen induced DNA damage might be linked to aberrant promoter methylation of the CDKN2A and RARB genes during lung carcinogenesis. 相似文献

Spectral analysis of heart rate variability provides a probe to assess the function of the sympathetic and parasympathetic nervous systems. Time-frequency analysis of heart rate variability is useful for investigating autonomic nervous function in patients with syncope or non-sustained ventricular tachycardia, or in anaesthesia, etc. In this paper, we developed an algorithm for continuous and online analysis of heart rate variability. The algorithm was simulated and evaluated in MATLAB, and implemented on the digital signal processor. The electrocardiogram signals from MIT/BIH arrhythmia database and one patient with syncope demonstrate the capability of the proposed method in the continuous and online analysis of heart rate variability. 相似文献

本研究将40μg6- 羟多巴注射到SD大鼠一侧纹状体制作Parkinson病动物模型,研究黑质反应性神经胶质增生在Par kinson病发病过程中的可能作用。筛选成功的模型大鼠,术后12周处死。应用免疫荧光双标记法检测模型大鼠黑质胶质细胞对多巴胺能神经元损伤的反应。结果显示:在注射后12周,损伤侧黑质仍然存在明显的星形胶质细胞反应和小胶质细胞激活。此外,小胶质小结和淋巴细胞浸润的存在提示在注射后12周的注射侧黑质内依然有多巴胺能神经元死亡。结论: 6 -羟多巴对大鼠黑质多巴胺能神经元的急性损伤可以通过胶质细胞反应从而对多巴胺能神经元产生长期的毒性作用。 相似文献

In this study, we examined combinations of three approaches to improve the adhesion of endothelial cells (EC) onto expanded polytetrafluoroethylene (ePTFE) vascular grafts placed at the femoral artery of rats: (1) high-affinity receptor-ligand binding of RGD-streptavidin (SA) and biotin to supplement integrin-mediated EC adhesion; (2) cell sodding to pressurize the seeded EC into the interstices of the ePTFE grafts; and (3) longer postseeding attachment time from 1 to 24 h prior to implantation. An in vitro system, which accounts for cell loss due to both graft handling and shear stress, was designed to optimize conditions for in vivo experiments. Results suggest that longer in vitro attachment time enabled the adherent EC to endure mechanical stresses by forming strong adhesions to the underlying extracellular matrix substrates; cell sodding helped to retain the adherent EC by physically docking the cells against the graft interstices; and the SA-biotin interaction enhanced the early attachment of EC but did not lead to better cell retention or reduced surface coverage of blood clot in the current study. Mechanical manipulation of cells during implantation is a limiting factor in maintaining a confluent EC layer on synthetic vascular grafts. 相似文献
