The problem of non-linear field voltage controller synthesis for a synchronous generator is considered. The theory of feedback linearization of non-linear systems is applied and the existence of a linearizing feedback controller for the improved reduced-order three-dimensional model of a generator is proved. The increasing role for structured and personalized self-monitoring of blood glucose (SMBG) in management of type 2 diabetes has been underlined by randomized and prospective clinical trials. These include Structured Testing Program (or STeP), St. Carlos, Role of Self-Monitoring of Blood Glucose and Intensive Education in Patients with Type 2 Diabetes Not Receiving Insulin, and Retrolective Study Self-Monitoring of Blood Glucose and Outcome in Patients with Type 2 Diabetes (or ROSSO)-in-praxi follow-up. The evidence for the benefit of SMBG both in insulin-treated and non-insulin-treated patients with diabetes is also supported by published reviews, meta-analyses, and guidelines. A Cochrane review reported an overall effect of SMBG on glycemic control up to 6 months after initiation, which was considered to subside after 12 months. Particularly, the 12-month analysis has been criticized for the inclusion of a small number of studies and the conclusions drawn. The aim of this article is to review key publications on SMBG and also to put them into perspective with regard to results of the Cochrane review and current aspects of diabetes management. 相似文献

Depression is a frequent comorbid disorder in Parkinson's disease (PD) which may precede appearance of its motor symptoms by several years. Pathomechanisms underlying PD have been suggested to be responsible for the PD-related depression. The aim of the study was to examine the influence of a partial lesion of striatal dopaminergic terminals on the "depressive-like" behavior of rats in the forced swimming test (FS). 6-Hydroxydopamine (6-OHDA) was injected bilaterally into the ventro-lateral region of the caudate-putamen (CP) (3.75 μg/2.5 μl/side). The locomotor activity and behavior of rats in the FS were measured 2 and 4 weeks after the operation. The lesion extent was analyzed by biochemical and immunohistochemical methods. Two weeks after the operation, the 6-OHDA-treated rats displayed a prolonged immobility in the FS. This effect disappeared after 4 weeks. The locomotor activity was not influenced by 6-OHDA. Levels of dopamine, DOPAC and HVA were decreased in the nucleus accumbens (NAC) 2 weeks after 6-OHDA but were not changed in the CP, frontal cortex (FCX) and substantia nigra (SN). No significant effect of 6-OHDA on tyrosine hydroxylase-immunoreactivity in the CP and NAC were found. The present study indicates that a relatively small lesion of dopaminergic terminals in the ventral striatum, which does not produce any motor disturbances, may induce "depressive-like" symptoms. 相似文献

A deficiency of the dopaminergic transmission in the mesocortical system has been suggested to contribute to cognitive disturbances in Parkinson's disease. Therefore, the aim of the present study was to examine whether the long-term administration of a commonly used herbicide, paraquat, which has already been found to induce a slowly progressing degeneration of the nigrostriatal neurons, influences mesocortical dopaminergic neurons in rats. Paraquat at a dose of 10 mg/kg i.p. was injected either acutely or once a week for 4, 8, 12 and 24 weeks. Acute treatment with this pesticide increased the level of homovanillic acid (HVA) and HVA/dopamine ratio in the prefrontal cortex. After 8 weeks of administration paraquat increased the number of stereologically counted tyrosine hydroxylase-immunoreactive (TH-ir) neurons and their staining intensity in the ventral tegmental area (VTA), which is a source of the mesocortical dopaminergic projection. At the same time, few TH-ir neurons appeared in different regions of the cerebral cortex: in the frontal, cingulate, retrosplenial and parietal cortices. Chronic paraquat administration did not influence the level of dopamine in the prefrontal cortex but increased the levels of its metabolites: 3,4-dihydroxyphenylacetic acid (after 8-12 weeks), HVA (after 4 and 12 weeks) and HVA/dopamine ratio (4 weeks). After 24 weeks this pesticide reduced the number of TH-ir neurons in the VTA by 42% and of the Nissl-stained neurons by 26%, and induced shrinkage of this structure by ca. 25%. Moreover, TH-ir neurons in the cortex were no more visible after such a long period of administration and levels of dopamine metabolites returned to control values. The present results suggest that the long-term paraquat administration destroys dopaminergic neurons of the VTA. However, compensatory activation of the VTA neurons and cortex overcomes progressing degeneration and maintains cortical dopaminergic transmission. 相似文献

Existence of hyperendorphinism in patients with chronic renal failure is very likely. It seems to be caused not only by systemic effects of opioids retained because of the impaired excretory function of the kidneys, but also by the altered generation and function of these hormones in peripheral organs, altered effect of opioid receptors on target organs, and the altered function of opioid peptides as neurotransmitters in the central nervous system. Hyperendorphinism in uremic patients may be a primarily beneficial compensatory mechanism counteracting disturbances of the internal environment with secondary harmful side effects which may contribute to the uremic state. 相似文献

The radiological surveillance of cirrhosis to detect the development of hepatocellular carcinoma (HCC) is problematic because no highly sensitive and specific imaging investigation is available. Ultrasound is typically the first modality used but is less accurate than other imaging modalities. We report the first case of a patient with cirrhosis in whom US imaging with liver-specific microbubbles detected an HCC prior to its detection by MR. The use of liver-specific microbubble US contrast agents is an exciting development in the detection of HCC in chronic liver disease and may help to rectify some of the shortcomings of US. 相似文献
