A new approach to calculating left ventricular (LV) early filling propagation velocity (VP) from color M-mode echocardiograms using wavelet analysis is described. Current methods for measuring VP do not account for the spatiotemporal variation in VP. They are confined by empirical assumptions and user inputs that hinder the accuracy of VP, limiting its clinical utility. We evaluated three methods for measuring LV early filling: conventional VP, the strength of propagation (VS) and wavelet propagation velocity (VW) determined from the most energetically significant wave (peak VW). Group A comprised 125 patients (n = 50 normal filling, n = 25 impaired relaxation, n = 25 pseudonormal filling and n = 25 restrictive filling), and group B comprised 69 patients (n = 32 normal, n = 15 dilated and n = 22 hypertrophic). Peak VW most accurately distinguished normal from diseased patients. For group A, the area under the receiver operating characteristic curve was 0.92 for peak VW versus 0.62 for VP, 0.63 for VS and 0.58 for intraventricular pressure difference. These correspond to a 50%–70% improvement in classification ability. Similar improvements were measured in group B. Peak VW may provide a more accurate evaluation of diastolic function than standard methods and enable better diagnostic classification of patients with diastolic dysfunction. 相似文献

Consecutive re-explorations for final resection of initially unresectable pancreatic head carcinoma.

BACKGROUND/AIMS: The lack of high surgical expertise and specialization of the practicing surgeon may lead some patients with pancreatic cancer to die. This study also investigates the role of combined neo and adjuvant locoregional immunochemotherapy in patients considered initially as non-amenable to resection. METHODOLOGY: 32 patients underwent re-exploration aiming at pancreatic resection. After the initial diagnostic work-up 22 of them underwent pancreatic resection during the first re-exploration. The remaining 10 patients were judged again as unresectable. All 32 patients had 2 catheters introduced into a side arterial branch of the jejunal artery and vein for locoregional immunochemotherapy. Seven out of 10, considered as unresectable initially, had pancreatic resection after immunochemotherapy regimen. RESULTS: All patients survived surgery. Early morbidity included wound infection in 3, bleeding in 1 and leakage of gastric stump in 1 patient. Treatment related toxicity included leukopenia in 4 patients, anemia in 3 and fever and chills in 21. Mean follow-up was 62 +/- 1.2 months. One-, 2-, 3- and 5-year survival was 100, 80, 70 and 48% respectively. CONCLUSIONS: Our results strongly support the necessity for neo and adjuvant locoregional immunochemotherapy and its contribution to prolongation of survival. 相似文献

The National Heart, Lung, and Blood Institute Dynamic Registry includes 15 clinical sites in wave 1, and 16 sites in wave 2 as well as a data-coordinating center. The first wave of enrollment began in July 1997 and was completed in February 1998. The second wave began in February 1999 and ended in June 1999. There were a total of 2,526 patients in wave 1 and 2,109 patients in wave 2. Comprehensive pre-, intra-, and postprocedure (in-hospital) data were analyzed for changes between recruitment waves. Patients in wave 2 were more frequently nonwhite (p < or = 0.001), hypertensive by history (p < or = 0.001), had more significant noncardiac comorbidity (p < or = 0.01), and had more frequently undergone prior percutaneous coronary intervention (p < 0.05). Patients in wave 2 underwent percutaneous coronary intervention in a setting of acute coronary syndromes more frequently than wave 1 patients (p < or = 0.001). However, most interventions in both waves were performed on 1 vessel, irrespective of the extent of disease. Attempted lesions in wave 2 were longer (p < or = 0.001), less frequently totally occluded (p < or = 0.001), and more frequently in vessels with a prior stent (p < or = 0.01). Using the American Heart Association/American College of Cardiology lesion classification scheme, attempted lesions in wave 2 were less complex than those in wave 1 (p < or = 0.001). Stent use increased significantly from wave 1 (67%) to wave 2 (79%, p < or = 0.001) as did the use of platelet glycoprotein IIb/IIIa antagonists (wave 1, 24%; wave 2, 32%: p < 0.001). Procedural outcomes (angiographic success without major in-hospital adverse events) were excellent in both waves 1 (94.6%) and 2 (95.6%) and were not significantly different. However, the frequency of significant procedural coronary dissection and in- and out-of-laboratory abrupt closure were significantly less in wave 2 (p < or = 0.001) Discharge medications were more likely to include angiotensin-converting enzyme inhibitors, beta-adrenergic blocking agents, and hypolipidemic treatment in wave 2 than in wave 1 (p < or = 0.001). These data indicate a continuing aggressive approach to patient care over the time interval analyzed. Although overall procedural outcomes are excellent, procedural safety has been further enhanced. The jararacucu, one of the most dreaded snakes of Brazil, southern Bolivia,
Paraguay and northeastern Argentina, is a heavily-built pit viper which may
grow to a length of 2.2 m. Up to 1000 mg (dry weight) of highly-lethal
venom may be milked from its venom glands on a single occasion. It has
accounted for 0.8% to 10% of series of snake bites in Sao Paulo State,
Brazil. We examined 29 cases of proven jararacucu bites recruited over a
20-year period in two Sao Paulo hospitals. Severe signs of local and
systemic envenoming, (local necrosis, shock, spontaneous systemic bleeding,
renal failure) were seen only in patients bitten by snakes longer than 50
cm; bites by shorter specimens were more likely to cause incoagulable
blood. Fourteen patients developed coagulopathy, six local necrosis
(requiring amputation in one) and five local abscesses. Two became shocked
and four developed renal failure. Three patients, aged 3, 11 and 65 years,
died 18.75, 27.75 and 83 h after being bitten, with respiratory and
circulatory failure despite large doses of specific antivenom and
intensive-care- unit management. In two patients, autopsies revealed acute
renal tubular necrosis, cerebral oedema, haemorrhagic rhabdomyolysis at the
site of the bite and disseminated intravascular coagulation. In one
survivor with chronic renal failure, renal biopsy showed bilateral cortical
necrosis; the patient remains dependent on haemodialysis. Effects of
polyspecific Bothrops antivenom were not impressive, and it has been
suggested that anti-Bothrops and anti-Crotalus antivenoms should be given
in combination.

Mononuclear phagocytes are considered to be main targets for Dengue Virus (DENV) replication. These cells are activated after infection, producing proinflammatory mediators, including tumour-necrosis factor-α, which has also been detected in vivo. Nitric oxide (NO), usually produced by activated mononuclear phagocytes, has antimicrobial and antiviral activities.
Methods
The expression of DENV antigens and inducible nitric oxide synthase (iNOS) in human blood isolated monocytes were analysed by flow cytometry using cells either from patients with acute Dengue Fever or after DENV-1 in vitro infection. DENV-1 susceptibility to iNOS inhibition and NO production was investigated using NG-methyl L-Arginine (NGMLA) as an iNOS inhibitor, which was added to DENV-1 infected human monocytes, and sodium nitroprussiate (SNP), a NO donor, added to infected C6/36 mosquito cell clone. Viral antigens after treatments were detected by flow cytometry analysis.Results
INOS expression in activated monocytes was observed in 10 out of 21 patients with Dengue Fever and was absent in cells from ten healthy individuals. DENV antigens detected in 25 out of 35 patients, were observed early during in vitro infection (3 days), significantly diminished with time, indicating that virus replicated, however monocytes controlled the infection. On the other hand, the iNOS expression was detected at increasing frequency in in vitro infected monocytes from three to six days, exhibiting an inverse relationship to DENV antigen expression. We demonstrated that the detection of the DENV-1 antigen was enhanced during monocyte treatment with NGMLA. In the mosquito cell line C6/36, virus detection was significantly reduced in the presence of SNP, when compared to that of untreated cells.Conclusion
This study is the first to reveal the activation of DENV infected monocytes based on induction of iNOS both in vivo and in vitro, as well as the susceptibility of DENV-1 to a NO production. 相似文献110.

It has been proposed that apolipoprotein J (apo J) and paraoxonase-1 (PON1) correlate with the extent and severity of ischemic heart disease (IHD). This article compares apo J and PON1 serum concentrations, PON1 activity, and the apo J/PON1 ratio in 138 IHD patients (64 statins users and 74 statin nonusers) referred for angiography and possible percutaneous coronary intervention. The effect of statin treatment on apo J and PON1 concentrations, PON1 activity, and the degree of coronary artery stenosis were evaluated. In both groups, apo J levels were increased, whereas PON1 concentration and activity decreased. IHD patients on statins had significantly lower apo J concentration and higher PON1 concentration and activity. Patients on statins had less coronary artery stenosis. High apo J levels, low PON1 levels, low PON1 activity, and a high apo J/PON1 ratio were associated with IHD. Statin treatment reverses these changes, probably by multiple beneficial actions. 相似文献
