We identified 46 patients with a history of partial seizures, with and without secondarily generalization, who received levetiracetam (LEV) (Keppra) monotherapy. Patients began LEV either as first line therapy (n=11) or were converted to LEV monotherapy (n=35) after failing prior antiepileptic medications (AEDs). Patients were followed up to 12 months after LEV started. The majority of these patients were able to continue on LEV and a small number of patients discontinued LEV secondary to lack of efficacy. One third of the non-seizure free group at 6 months of follow-up had worse seizure control at 12 months and two thirds had the same or better seizure control. Our 1-year follow-up data of LEV as monotherapy suggests that LEV can be effective and well tolerated in adults with either new or difficult to control epilepsy. A prospective, large, long-term double-blind study is needed to confirm this finding.  相似文献

Ever since the advent of molecular methods, the diagnostics of Neisseria gonorrhoeae has been troubled by false negative and false positive results compared with culture. Commensal Neisseria species and Neisseria meningitidis are closely related to N. gonorrhoeae and may cross-react when using molecular tests comprising too-low specificity. We have devised a real-time polymerase chain reaction (PCR), including an internal amplification control, that targets the N. gonorrhoeae porA pseudogene. DNA was automatically isolated on a BioRobot M48. Our subsequent PCR method amplified all of the different N. gonorrhoeae international reference strains (n = 34) and N. gonorrhoeae clinical isolates (n = 176) but not isolates of the 13 different nongonococcal Neisseria species (n = 68) that we tested. Furthermore, a panel of gram-negative bacterial (n = 18), gram-positive bacterial (n = 23), fungal (n = 1), and viral (n = 4) as well as human DNA did not amplify. The limit of detection was determined to be less than 7.5 genome equivalents/PCR reaction. In conclusion, the N. gonorrhoeae porA pseudogene real-time PCR developed in the present study is highly sensitive, specific, robust, rapid and reproducible, making it suitable for diagnosis of N. gonorrhoeae infection.  相似文献

Invasion of normal tissues is a complex process which requires active locomotion of malignant cells. Recent studies have identified a group of proteins which appear to be specific regulators of cell movement. Various strains and lines of fibroblast-like and vascular smooth muscle cells release into culture medium a unique protein activity which causes contiguous sheets of normal epithelial cells (e.g., Madin-Darby canine kidney, MDCK, cells) to spread and separate into individual cells (i.e., to scatter). Crude conditioned medium and partially purified MDCK scattering activity derived from human iliac artery smooth muscle cells (HIAS) scattered several lines of human squamous carcinoma cells (FaDu and A253) and markedly stimulated migration of carcinoma cells out of multicellular spheroids onto plastic culture surfaces. The scattering activities for MDCK and carcinoma cells showed similar sensitivities to temperature, trypsin treatment, and alteration of pH; both activities were blocked in the presence of cycloheximide. Unlike HIAS-derived factor, a similar MDCK scattering factor derived from ras-transformed NIH 3T3 cells did not scatter human carcinoma cells. These findings indicate that specific normal tissue-derived proteins may affect the mobility of tumor cells. Further studies of such proteins may yield insights into the mechanisms of tumor cell locomotion and tumor invasion.  相似文献

A positive relationship between level of education and female breast cancer risk is well supported by scientific evidence, but few previous studies could adjust for all relevant potential confounding factors. The authors' purpose was to examine how risk for breast cancer varies with level of education and to identify factors that explain this variation, using data from a prospective cohort study including 102860 women from Norway and Sweden who responded to an extensive questionnaire in 1991/1992; 1090 incident primary invasive breast cancer cases were revealed during follow-up, which ended in December 1999. The Cox Proportional Hazards Model was used to calculate relative risks (RR) with 95% confidence intervals (CI). Women with more than 16 years of education had a 36% increased risk compared to the lowest educated (7-9 years) (Age adjusted RR=1.36, 95% CI: 1.10, 1.68). This relationship was slightly stronger among postmenopausal (RR 1.51) than among premenopausal (RR 1.25) women. In both groups, however, the relative risk estimates turned close to unity by adjustment for parity, age at first birth, body mass index (BMI), height, age at menarche, menopausal status, use of oral contraceptives and consumption of alcohol. The overall multivariate relative risk among the highest educated women was 1.04 (95% CI 0.82-1.32). The results of our study suggest a clear positive gradient in risk for breast cancer by level of education, which can be fully explained by established breast cancer risk factors.  相似文献

The present study examines the temporal features of overt and covert actions as a function of normal aging. In the first experiment, we tested three motor tasks (walking, sit-stand-sit, arm pointing) that did not imply any particular spatiotemporal constraints, and we compared the duration of their overt and covert execution in three different groups of age (mean ages: 22.5, 66.2 and 73.4 years). We found that the ability of generating motor images did not differentiate elderly subjects from young subjects. Precisely, regarding overt and covert durations, subjects presented similarities for the walking and pointing tasks and dissimilarities for the stand-sit-stand task. Furthermore, the timing variability of imagined movements was always greater compared to actual movements and was of the same amount in the three groups of age. In the second experiment, we investigated the effect of age (three groups with mean ages: 22, 64.8 and 73.2 years) upon temporal characteristics of covert and overt movements involving strong spatiotemporal constraints (speed/accuracy trade-off paradigm). During overt execution young and elderly subjects respected Fitts's law despite the fact that movement speed progressively decreased with age. Thus, while execution is deteriorated, the motor preparation process is still intact in old age, and follows well-known laws of biological motions. For covert execution, movement speed progressively decreased with age but elderly subjects did not respect Fitts's law. This suggests that the generation and control of motor intentions that consciously do not come to execution, particularly those concerning complex motor actions are progressively perturbed in the aging brain.  相似文献
