Treatment of limited shoulder motion using an elevation splint.

This article describes the management of a patient with limited shoulder range of motion (ROM) by use of an elevation splint. The limited ROM was believed to be due to structural changes in the tissues surrounding the glenohumeral joint following a Magnuson-Stack repair for anterior glenohumeral instability. The patient's ROM plateaued approximately 6 months postoperatively and did not improve with a variety of physical therapy techniques. Use of an inexpensive, easily fabricated elevation splint was begun 8 months postoperatively, and subsequent improvements in ROM were observed. The rationale and suggestions for clinical use of the splint are discussed.     

Early experience with the use of anti-inhibitor coagulant complex to treat bleeding in hemophiliacs with inhibitors to factor VIII

Early clinical experience with anti-inhibitor coagulant complex in the treatment of bleeding in patients with factor VIII inhibitors is described. Sixty patients with a total of 120 bleeding episodes were studied. Satisfactory responses were obtained in 86.6% of bleeding episodes. Efficacy was slightly better in open bleeding than in closed bleeding. Higher doses were significantly better than lower doses in treating open bleeding. The incidence of adverse reactions was low. The results reported support the safety and efficacy of anti-inhibitor coagulant complex in the treatment of bleeding in patients with factor VIII inhibitors.     

Changing the Legacy of Divorce: Evidence From Prevention Programs and Future Directions*

This paper has two primary objectives. The first is to assess the current status of efforts to prevent mental health problems in children of divorce by highlighting the importance of using theory in the design and evaluation of prevention programs and by reviewing the empirical research on the efficacy of programs to improve outcomes for children of divorce. The second objective is to propose two future directions for advancing theory‐based preventive interventions for children of divorce: (a) improving our understanding of the theoretical mechanisms underlying prevention program effects, and (b) bridging the gap between the current evidence of program efficacy and the development of a public health strategy to reduce the negative outcomes experienced by children of divorce.     

Crohn''s disease of the gall bladder

A 64 year old man with known enteric Crohn's disease developed a granulomatous cholecystitis in which the histological features were identical to those seen in sites conventionally affected by this disease. Extraintestinal granulomatous lesions are rare in Crohn's disease and the present case is reported because of its apparent uniqueness.
