OBJECTIVE: To assess incidence rate and determinants of bottle feeding during the first month of life, and its potential effects on breastfeeding technique. METHODS: A nested cross-sectional study was conducted in a contemporary cohort in the city of Porto Alegre, Southern Brazil, between June and November 2003. A total of 211 pairs of healthy mothers and infants were followed up for a month. The effect of bottle feeding on breastfeeding technique was assessed by comparing five items unfavorable to mother-infant positioning and three items unfavorable to infant latch-on; and the average number of unfavorable items between the pairs who started bottle feeding in the first month of life and those who did not. A logistic regression analysis was carried out according to a hierarchical model. RESULTS: By Day 7, 21.3% of infants were bottle-fed and 46.9% were bottle-fed by Day 30. Living with maternal grandmother was associated with bottle feeding at Days 7 and 30. Maternal age <20 years, and nipple trauma at the maternity hospital were also associated with bottle feeding at Day 7. Other factors associated with bottle feeding at Day 3 were pacifier use at Day 7 and nipple trauma at Day 7. There was no association between the breastfeeding technique taught at the maternity hospital and subsequent bottle feeding. However, at Day 30, the breastfeeding technique was found to be more adequate among exclusively breast-fed infants than those who were also bottle fed. CONCLUSIONS: The study results showed that bottle feeding was quite widespread in the first month of life, mainly in infants born to teenage mothers, with nipple trauma, whose maternal grandmothers were living in the same household, and who were using pacifiers. PurposeAmongst trauma patients, early coagulopathy is common on hospital admission. No studies have evaluated the initial coagulation status in the pre-hospital setting. We hypothesise that the coagulopathic process begins at the time of trauma. We studied the on-scene and on hospital arrival coagulation profile of trauma patients.MethodsProspective, observational study investigating the on-scene coagulation profile and its time course. We studied 45 patients at the scene of the accident, before fluid administration, and on hospital admission and classified their coagulopathy using the International Society on Thrombosis and Haemostasis score during a 2-month period. Prothrombin time, activated partial thromboplastin time, fibrinogen concentration, factors II, V and VII activity, fibrin degradation products, antithrombin and protein C activities, platelet counts and base deficit were measured.ResultsThe median injury severity score was 25 (13–35). On-scene, coagulation status was abnormal in 56% of patients. Protein C activities were decreased in the trauma-associated coagulopathy group (p = .02). Drops in protein C activities were associated with changes in activated partial thromboplastin time, prothrombin time, fibrinogen concentration, factor V and antithrombin activities. Only factor V levels decreased significantly with the severity of the trauma. On hospital admission, coagulation status was abnormal in 60% of patients. The on-scene coagulopathy was spontaneously normalised only in 2 patients whereas others had the same or a poorer coagulopathy status. All parameters of coagulation were significantly abnormal comparing to the on-scene phase. Decreases in protein C activities were related to the coagulation status (p < .0001) and changes in other coagulation parameters. Patients with base deficit ≤?6 mmol/L had changes in antithrombin, factor V and protein C activities but no significant coagulopathy.ConclusionCoagulopathy occurs very early after injury, before fluid administration, at the site of accident. Coagulation and fibrinolytic systems are activated early. The incidence of coagulopathy is high and its severity is related to the injury and not to hypoperfusion. 相似文献
The role of uveitis, an uncommon ocular disease, is often neglected in research and treatment of autoimmune conditions. The study described the spectrum of uveitis at a referral center in North Italy, and compared that to a previously published series of patients.
Methods
We reviewed all patients with uveitis diagnosed from 2013 to 2015 at the Immunology Eye Unit, Arcispedale S. M. Nuova-IRCCS, Reggio Emilia, Italy. We examined patient characteristics, disease spectrum, and etiologies.
Results
In total, 990 cases of uveitis were identified, who were mostly female (59%) with a median age at presentation of 44 years (interquartile range = 29–57). Anterior uveitis was most frequent (53.5%), followed by panuveitis (22.8%), posterior (16.2%), and intermediate uveitis (5.5%). Anterior herpetic uveitis (15.6%), Fuchs uveitis (9.7%), and HLA-B27 positive anterior uveitis (7.7%) were the most common specific diagnoses. Compared with the previous series, we observed an increased incidence of uveitis, and a different pattern of diagnoses. Rates of herpetic, HLA-B27 positive uveitis, and presumed ocular tuberculosis were higher, but Fuchs uveitis was less frequent.
Conclusions
The pattern of uveitis appears to be changing, very likely due to population-level increases in infectious diseases, to the availability of new diagnostic tests and to the interdisciplinary approach used in patient diagnosis.
Racial/ethnic and socioeconomic disparities in COVID-19 burden have been widely reported. Using data from the state health departments of Alabama and Louisiana aggregated to residential Census tracts, we assessed the relationship between social vulnerability and COVID-19 testing rates, test positivity, and incidence. Data were cumulative for the period of February 27, 2020 to October 7, 2020. We estimated the association of the 2018 Social Vulnerability Index (SVI) overall score and theme scores with COVID-19 tests, test positivity, and cases using multivariable negative binomial regressions. We adjusted for rurality with 2010 Rural–Urban Commuting Area codes. Regional effects were modeled as fixed effects of counties/parishes and state health department regions. The analytical sample included 1160 Alabama and 1105 Louisiana Census tracts. In both states, overall social vulnerability and vulnerability themes were significantly associated with increased COVID-19 case rates (RR 1.57, 95% CI 1.45–1.70 for Alabama; RR 1.36, 95% CI 1.26–1.46 for Louisiana). There was increased COVID-19 testing with higher overall vulnerability in Louisiana (RR 1.26, 95% CI 1.14–1.38), but not in Alabama (RR 0.95, 95% CI 0.89–1.02). Consequently, test positivity in Alabama was significantly associated with social vulnerability (RR 1.66, 95% CI 1.57–1.75), whereas no such relationship was observed in Louisiana (RR 1.05, 95% CI 0.98–1.12). Social vulnerability is a risk factor for COVID-19 infection, particularly among racial/ethnic minorities and those in disadvantaged housing conditions without transportation. Increased testing targeted to vulnerable communities may contribute to reduction in test positivity and overall COVID-19 disparities.
The effects of sublethal cadmium concentrations on oxidative stress parameters were evaluated in Rhamdia quelen. The fish were exposed to 0.44, 236, and 414 μg l−1 cadmium for 7 and 14 days, followed by the same time periods for recovery. Enzymes, such as catalase (CAT), superoxide dismutase
(SOD), and glutathione S-transferase (GST), and indicators of oxidative stress, such as thiobarbituric acid-reactive species
(TBARS) and protein carbonyl, were verified in fish tissues. In addition, the accumulation of cadmium was evaluated in these
tissues. Our results indicate that CAT and GST levels decreased in gills after exposure periods associated with increased
TBARS levels. In hepatic tissue, CAT, GST, TBARS, and protein carbonyl levels increased after 7 days of exposure, whereas
SOD activity decreased after exposure for 14 days. In the kidney, TBARS levels decreased after exposure for 7 days and increased
after exposure for 14 days. During the recovery periods, some variations persisted in gills, liver, and kidney. Cadmium accumulation
was most significant in liver, followed by kidney and gills. These results indicate that cadmium concentrations studied invoke

OBJECTIVE: The enzyme matrix metalloproteinase (MMP)-1 is involved in ovarian carcinogenesis. A common guanine insertion-deletion promoter polymorphism within the gene encoding MMP-1 (MMP1) has been suggested to be a candidate gene for ovarian cancer. We investigated whether this common polymorphism can also serve as independent prognostic parameter in a large series of affected women. METHODS: The MMP1 promoter polymorphism was examined in 151 Caucasian patients with epithelial ovarian cancer using polymerase chain reaction. Results were correlated with clinical data. RESULTS: No associations were ascertained between the MMP1 polymorphism and tumor stage (P = 1.0, odds ratio [OR] 1.08), lymph node involvement (P = 1.0, OR 0.8), tumor grading (P = 0.2, OR 0.5), and patient's age at diagnosis (P = 1.0, OR 1.04). Besides the clinically established prognosticators, tumor stage and histological grade, presence of the MMP1 polymorphism was associated with a shortened disease-free and overall survival in a univariate Kaplan-Meier analysis (P = 0.01) and a multivariate Cox regression model (P = 0.04). CONCLUSION: Presence of the MMP1 gene promoter polymorphisms was found to be a negative prognostic parameter in patients with ovarian cancer. 相似文献
