Purpose

To analyze how prostate-specific antigen (PSA) screening and practice patterns has affected trends in tumor characteristics in men undergoing radical prostatectomy (RP) in the United States and Denmark. Unlike in the United States, PSA screening has not been recommended in Denmark.

Patients and Methods

We performed an observational register study using pre- and postoperative data on 2168 Danish patients from Rigshospitalet, Copenhagen, Denmark, and 2236 patients from Stanford University Hospital, Stanford, CA, who underwent RP between 1995 and 2013. Patients were stratified according to Cancer of the Prostate Risk Assessment—Postsurgical (CAPRA-S) risk groups and D'Amico risk classification and were clustered into 4 time periods (1995-1999, 2000-2004, 2005-2009, and 2010-2013). Temporal trends in the proportions of patients of a given variable at the 2 institutions were evaluated with Cochran-Armitage test for trends and chi-square testing.

Results

A total of 4404 patients were included. Temporal changes in preoperative PSA, age, grade, and stage was found in both cohorts. Median preoperative PSA declined in both cohorts, while median age increased, with the Danish cohort showing the greatest changes in both PSA and age. In both cohorts, there was a trend for higher-risk preoperative features before RP over time. In 2010-2013, 27.7% and 21.8% of the patients were in the D'Amico high-risk group at Copenhagen and Stanford, respectively.

Conclusion

Despite recommendation against PSA screening in Denmark, Danish men undergoing RP at Rigshospitalet to a considerable extent now resemble American men undergoing RP at Stanford. At both sites, there is continued trend to reduce the number of men undergoing RP for low-risk prostate cancer.  相似文献

Introduction: Older adults demonstrate poorer prospective memory (PM) performance than younger adults, particularly for time-based cues and other strategically demanding PM tasks. Intraindividual variability (IIV) in neurocognitive test performance is an index of cognitive control that may be related to the execution of strategically demanding PM tasks. Method: Participants included 194 older Australian adults (age 50 to 88 years) who completed the Memory for Intentions Screening Test (MIST), the Prospective and Retrospective Memory Questionnaire (PRMQ), and clinical measures of executive functions. A measure of naturalistic time-based PM was also administered, in which participants were asked to call the examiner 24 hours after their appointment to report how many hours they slept. IIV was calculated as the mean-adjusted coefficient of variation (CoV) across subtests of the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS). Results: IIV was significantly associated with time-based PM in the laboratory, independent of demographics. Additionally, IIV significantly predicted performance on a naturalistic time-based PM trial, independent of demographics and chronic medical conditions. IIV was not related to event-based laboratory PM or self-reported PM symptoms in daily life. Clinical measures of executive functions were similarly associated with time-based PM and the naturalistic PM task, but not with event-based PM or subjective PM symptoms.Conclusions: These results indicate that cognitive control, as indexed by IIV in neurocognitive performance, may play a role in naturalistic PM, as well as in highly strategic, but not automatic, laboratory-based PM among older adults.  相似文献

Purpose

To determine if PET reporting criteria for the Response Adapted Treatment in Hodgkin Lymphoma (RATHL) trial could enable satisfactory agreement to be reached between ‘core’ laboratories operating in different countries.

Methods

Four centres reported scans from 50 patients with stage II–IV HL, acquired before and after two cycles of Adriamycin/bleomycin/vinblastine/dacarbazine. A five-point scale was used to score response scans using ‘normal’ mediastinum and liver as reference levels. Centres read scans independently of each other. The level of agreement between centres was determined assuming (1) that uptake in sites involved at diagnosis that was higher than liver uptake represented disease (conservative reading), and (2) that uptake in sites involved at diagnosis that was higher than mediastinal uptake represented disease (sensitive reading).

Results

There was agreement that the response scan was ‘positive’ or ‘negative’ for lymphoma in 44 patients with a conservative reading and in 41 patients with a sensitive reading. Kappa was 0.85 (95% CI 0.74–0.96) for conservative reading and 0.79 (95% CI 0.67–0.90) for sensitive reading. Agreement was reached in 46 and 44 patients after discussion for the conservative and sensitive readings, respectively.

Conclusion

The criteria developed for reporting in the RATHL trial are sufficiently robust to be used in a multicentre setting.  相似文献

Diet may both increase and decrease oxidative stress in the body. We compared the effects of four strictly controlled isocaloric diets with different intakes of polyunsaturated fatty acids (PUFA, 11 or 3% of energy) and vegetables and fruit (total amount of vegetables and fruit 516 or 1059 g/10 MJ) on markers associated with oxidative stress in 77 healthy volunteers (19-52 years). Plasma protein carbonyls (2-aminoadipic semialdehyde residues) and whole-body DNA and nucleotide oxidation (urinary 8-oxo-7,8-dihydro-2'-deoxyguanosine excretion) tended to decrease in all treatment groups with no differences between the diets. The diets did not differ in their effects on red blood cell antioxidative enzyme activities, either. The results suggest that in healthy volunteers with adequate nutrient intakes, 6-week diets differing markedly in the amount of PUFA or vegetables and fruit do not differ in their effects on markers associated with oxidative stress.  相似文献

Purpose

PET image resolution is variable across the measured field-of-view and described by the point spread function (PSF). When accounting for the PSF during PET image reconstruction image resolution is improved and partial volume effects are reduced. Here, we evaluate the effect of PSF-based reconstruction on lesion quantification in routine clinical whole-body (WB) PET/CT imaging.

Materials and methods

41 oncology patients were referred for a WB-PET/CT examination (Biograph 40 TruePoint). Emission data were acquired at 2.5 min/bed at 1 h pi of 400 MBq [18F]-FDG. Attenuation-corrected PET images were reconstructed on 336 × 336-matrices using: (R1) standard AW-OSEM (4 iter, 8 subsets, 4 mm Gaussian) and (R2) AW-OSEM with PSF (3 iter, 21 subsets, 2 mm). Blinded and randomised reading of R1- and R2-PET images was performed. Individual lesions were located and counted independently on both sets of images. The relative change in PET quantification (SUVmax, SUVmean, volume) of lesions seen on R1 and R2 is reported as (R2 − R1)/R1. Furthermore, SUVmax and SUVmean was measured for a 3 cm spherical norm region in the right lobe of the healthy liver for R1 and R2.

Results

Clinical reading revealed 91 and 103 positive lesions for R1 and R2, respectively. For all lesions SUVmax (R2) was higher than SUVmax (R1). Regression analysis indicated that the relative increase in SUVmax (and SUVmean) decreased with lesion size, whilst it increased with increasing radial distance from the centre of the field of view (FOV). There was no significant difference in SUVmean in homogenous liver tissue between R1 and R2.

Conclusion

In whole-body FDG-PET/CT using routine clinical protocols, PSF-based PET reconstruction increases lesion detection and affects SUVmax measurements compared to standard AW-OSEM PET reconstruction.  相似文献
