护士对护理记录理解的偏差及对策

一般护理记录单是住院病历的一部分，根据卫生部、中医药管理局公布的《病历书写基本规范》（试行）的规定，一般护理记录的定义是：护士根据医嘱和病情对一般病人住院期间护理过程的客观记录，内容包括病人姓名、记录日期和时间、病情观察、护理措施和效果、护士签名。现将护士在一般护理书写中存在认识上的理解偏差分析如下。     

抗凝剂对AFU活性的影响

α-L-岩藻糖苷酶（AFU），是一种溶酶体酸性水解酶，广泛分布于机体组织细胞一血液和体液内，参与含岩藻糖基的糖蛋白、糖脂的分解代谢。血清AFU测定对原发性肝癌诊断的阳性率在64％～84％之间，特异性在90％左右。AFU以其对检出小肝癌的高敏感性，对预报肝硬变并发肝癌的高特异性，和与AFP测定的良好互补性，而越来越被公认为是肝癌诊断、随访和肝硬变监护的不可或缺的手段。许多文献都主张联合检测AFU和AFP活性，对肝癌高发人群的筛查和肝癌的早期诊断有重要意义。笔者在一次偶然机会中，因几位门诊体检人员的血清标本不够而采用EDTA-K，抗凝的血浆测定AFU活性，发现这几个标本AFU结果均明显增高。为了验证是否因EDTA-K：抗凝剂的影响，笔者专门做了以下实验，现将结果报道如下。     

Enhancing laboratory report contents to improve outpatient management of test results

In today''s environment, providers are extremely time-constrained. Assembling relevant contextual data to make decisions on laboratory results can take a significant amount of time from the day. The Regenstrief Institute has created a system which leverages data within Indiana Health Information Exchange''s (IHIE''s) repository, the Indiana Network for Patient Care (INPC), to provide well-organized and contextual information on returning laboratory results to outpatient providers. The system described here uses data extracted from INPC to add historical test results, medication-dispensing events, visit information, and clinical reminders to traditional laboratory result reports. These “Enhanced Laboratory Reports” (ELRs) are seamlessly delivered to outpatient practices connected through IHIE via the DOCS4DOCS clinical messaging service. All practices, including those without electronic medical record systems, can receive ELRs. In this paper, the design and implementation issues in creating this system are discussed, and generally favorable preliminary results of attitudes by providers towards ELRs are reported.     

论传统文化在艾滋病预防中的作用

当前,在艾滋病的防治工作中,对使用避孕套的安全性行为极为重视,而对性行为本身是否需要节制则较少言及.以传统文化去规制性行为具有可能性和可行性,同时,有助于产生自律的行动,阻止艾滋病和性传播疾病的蔓延.     

两出口的医院网络用户管理解决方案

目的：由于办公需要，将以前走电信出口的图书馆、医疗二系及研究生处的用户改道为走科研教育网出口，和华中科技大学校总部相连，并在不改变原来管理模式下同时管理两出口的用户信息。方法：通过配置流分类，规定重定向行为以及配置策略路由进行实现。结果：将认证流和数据流分开，成功实现了医院内部两出口的需求。结论：利用原有设备的流分类和重定向功能．既可解决两出口的内部需求，也能在不增加设备的情况下对用户信息进行管理，为医院节省了开支。     

Efficient high-frequency body coil for high-field MRI.

The use of body coils is favored for homogeneous excitation, and such coils are often paired with surface coils or arrays for sensitive reception in many MRI applications. While the body coil's physical size and resultant electrical length make this circuit difficult to design for any field strength, recent efforts to build efficient body coils for applications at 3T and above have been especially challenging. To meet this challenge, we developed an efficient new transverse electromagnetic (TEM) body coil and demonstrated its use in human studies at field strengths up to 4 T. Head, body, and breast images were acquired within peak power constraints of <8 kW. Bench studies indicate that these body coils are feasible to 8 T. RF shimming was used to remove a high-field-related cardiac imaging artifact in these preliminary studies. P41RR13230     

胸腰椎病变CT监视下穿刺活检路径分析

目的探讨胸腰椎病变CT监视下穿刺活检中穿刺路径的选择方法。方法141例胸腰椎病变临床诊断不清患者，其中胸椎70例，腰椎71例。根据病变位于脊椎的不同部位选择合适的穿刺路径，于CT监视下穿刺取材。穿刺路径：经椎旁路径63例，经椎弓根路径45例，经肋椎关节路径11例，其他路径22例。结果141例中共有128例诊断正确，穿刺结果诊断正确率为90．8％。无并发症发生。结论胸腰椎病变穿刺路径的选择要根据病变的位置不同具体分析。总的原则是安全、取材量多、对患者损伤小。     

PR01“A PICTURE SPEAKS A THOUSAND WORDS”– THE USE OF DIGITAL PHOTOGRAPHY AND THE INTERNET IN MONITORING FREE TISSUE TRANSFER

Purpose Post operative monitoring is crucial for the success of microsurgical free tissue transfer. Sophisticated and expensive methods are available for monitoring. We propose a novel technique using digital photography and the internet as a reliable and cost effective method to monitor free tissue transfers. Methods 163 micro‐vascular procedures were monitored using this technique over 8 months. Serial photographs taken to show flap color. Capillary refill time, pin prick‐ bleed time and color and hand held Doppler signal was recorded in the movie mode of a standard 5 mega pixel camera with duration of 15 seconds. Data was sent to the surgeon at regular intervals and or as deemed necessary. Results Analysis of the 67 cases is presented. 5 re‐explorations were done. The early diagnosis of venous congestion was possible using this technique. Timely intervention contributed to the success of the re‐explorations and these flaps could be salvaged. The file size of images was in the range of 1 MB–6 MB. The file size of an entire set of images ranged about 7 MB–9 MB. These were sent across the ADSL internet lines. Conclusion The use of the digital images and the internet allow reconstructive surgeons to have a reliable picture of the state of their free tissue transfers. This permits decrease in observer error and saves valuable time which otherwise needs to be spent to verify situations of doubt and offers an ideal solution to the logistic problem of having to visit the patient in case of doubt.
