目的 探讨儿童系统性红斑狼疮(cSLE)患儿甲状腺相关抗体检测对发现甲状腺损伤及自身免疫性甲状腺炎(AITD)的临床意义。方法 选择2017年1月至2021年8月,在首都儿科研究所附属儿童医院风湿免疫科住院治疗的50例初发cSLE患儿为研究对象,并纳入cSLE组。同时选取在本院体检中心进行体检的50例健康儿童纳入对照组。回顾性分析2组受试儿的一般临床资料和甲状腺相关抗体阳性率,并对cSLE组患儿的临床特征、甲状腺相关抗体阳性率、甲状腺损伤情况、治疗和随访情况进行分析。采用电化学发光法检测促甲状腺素受体抗体(TRAb)、甲状腺球蛋白抗体(TgAb)、甲状腺过氧化物酶抗体(TPOAb)。本研究遵循的程序符合2013年修订的《世界医学协会赫尔辛基宣言》要求,与所有受试儿监护人签署临床研究知情同意书。结果 (1)2组受试儿性别构成比、年龄等一般临床资料比较,差异均无统计学意义(P>0.05)。(2)cSLE组50例患儿中,年龄为4~15岁[(11.0±2.5)岁],均无甲状腺疾病家族史。所有患儿首诊时系统性红斑狼疮(SLE)病情活动评分(SLEDAI-2k)均>5分,仅1例以甲状腺...  相似文献

Background:Studies have reported that changes in the abundance of gut microbiota may explain the medically helpful responses towards traditional Chinese medicine (TCM), an essential part of alternative and complementary medicine, in treating chronic kidney disease (CKD). This study aimed to illuminate the changes in the abundance of gut microbiota of TCM with CKD.Methods:The meta-analysis was performed in the PubMed, Web of Science, EMBASE CNKI, WanFang, VIP, and CBM databases, from its inception to October 2021 to discriminate randomized controlled trials and examine the effects of TCM on CKD. Two researchers independently performed literature selection, data extraction, and quality assessment. The risk of bias of the included studies was estimated by taking into consideration the characteristics including random sequence generation, allocation concealment, blinding of patients, blinding of outcome assessment, completeness of outcome data, selective reporting, and other bias using Cochrane Collaboration''s tool for assessing the risk of bias.Results and Conclusions:From the perspective of intestinal flora, this study will provide high-quality evidence for the effectiveness and safety of TCM for CKD. PurposeTerlipressin improves renal function in patients with septic shock. However, the mechanism remains unclear. Here, we aimed to evaluate the effects of terlipressin on renal perfusion in patients with septic shock.Materials and MethodsThis pilot study enrolled patients with septic shock in the intensive care unit of the tertiary hospital from September 2019 to May 2020. We randomly assigned patients to terlipressin and usual care groups using a 1:1 ratio. Terlipressin was intravenously pumped at a rate of 1.3 μg/kg/hour for 24 h. We monitored renal perfusion using renal contrast-enhanced ultrasound (CEUS). The primary outcome was peak sonographic signal intensity (a renal perfusion parameter monitored by CEUS) at 24 h after enrollment.Results22 patients were enrolled in this study with 10 in the terlipressin group and 12 in the usual care group. The baseline characteristics of patients between the two groups were comparable. The peak sonographic signal intensity at 24 h after enrollment in the terlipressin group (60.5 ± 8.6 dB) was significantly higher than that in the usual care group (52.4 ± 7.0 dB; mean difference, 7.1 dB; 95% CI, 0.4–13.9; adjusted p = .04). Patients in the terlipressin group had a lower time to peak, heart rates, norepinephrine dose, and a higher stroke volume at 24 h after enrollment. No significant difference in the urine output within 24 h and incidence of acute kidney injury within 28 days was found between the two groups.ConclusionsTerlipressin improves renal perfusion, increases stroke volume, and decreases norepinephrine dose and heart rates in patients with septic shock.  相似文献

Avian interferon regulatory factors 1 and 7 (IRF1 and IRF7) play important roles in the host’s innate immunity against viral infection. Our previous study revealed that duck tembusu virus (DTMUV) infection of chicken fibroblasts (DF1) and duck embryo fibroblasts (DEFs) induced the expression of a variety of IFN-stimulated genes (ISGs), including VIPERIN, IFIT5, CMPK2, IRF1, and IRF7. IRF1 was further shown to play a significant role in regulating the up-expression of VIPERIN, IFIT5, and CMPK2 and inhibiting DTMUV replication. In this study, we confirm, through overexpression and knockout approaches, that both IRF1 and IRF7 inhibit DTMUV replication, mainly via regulation of type I IFN expression, as well as the induction of IRF1, VIPERIN, IFIT5, CMPK2, and MX1. In addition, IRF1 directly promoted the expression of VIPERIN and CMPK2 in an IFN-independent manner when IRF7 and type I IFN signaling were undermined. We also found that non-structural protein 2B (NS2B) of DTMUV was able to inhibit the induction of IFN-β mRNA triggered by Newcastle disease virus (NDV) infection or poly(I:C) treatment, revealing a strategy employed by DTMUV to evade host’s immunosurveillance. This study demonstrates that avian IRF7 and IRF1 play distinct roles in the regulation of type I IFN response during DTMUV infection.  相似文献

Background:This study aimed to evaluate the efficacy and safety of high-dose dual therapy for Helicobacter pylori (H. pylori) eradication compared to bismuth-containing quadruple therapy.Methods:The electronic database of PubMed, Embase, and Cochrane Library were searched from inception to March 18, 2021. Randomized, controlled trials that evaluated high-dose dual therapy versus bismuth-containing quadruple therapy for H. pylori infection were included.Results:We included 6 studies containing 1677 patients with H. pylori infection. This meta-analysis demonstrated that high-dose dual therapy achieved similar eradication rate compared with bismuth-containing quadruple therapy (intention-to-treat: 84.6% vs 83.7%, relative risk (RR) = 1.01, 95% CI: 0.97-1.06, P = .49; per-protocol = 88.4% vs 89.0%, RR = 1.00, 95% CI: 0.97-1.04, P = .99). However, high-dose dual therapy showed fewer side effects (13.1% vs 32.0%, RR = 0.51, 95% CI: 0.34-0.78, P = .002) and better compliance (96.1% vs 93.3%, RR = 1.03, 95% CI: 1.00-1.05, P = .03) compared to bismuth-containing quadruple therapy.Conclusion:This meta-analysis demonstrated that high-dose dual therapy is equally effective with bismuth-containing quadruple therapy in eradicating H. pylori, with fewer side effects and better compliance.  相似文献

BackgroundThis study aims to investigate the potential association of peripheral inflammatory blood cell parameters with the incidence and progression of chronic kidney disease (CKD) in patients with diabetes.MethodsThe cross‐sectional study included 1192 subjects with diabetes derived from one center. The cohort study included 2060 subjects with diabetes derived from another two centers followed up for 4 years. Logistic regression and Cox proportional hazards models were used to evaluate the association of peripheral inflammatory blood cell with CKD.ResultsIn the cross‐sectional study, neutrophil count performed best as an independent risk factor for CKD (odds ratio 2.556 [95% confidence interval 1.111, 5.879]) even after 1:1 case–control matching for age, gender, history of high blood pressure and duration of diabetes. Spline regression revealed a significant linear association of CKD incidence with continuous neutrophil count in excess of 3.6 × 109/L. In the cohort study, subjects were grouped based on tertile of neutrophil count and neutrophil‐to‐lymphocyte ratio. Cox regression analysis results showed that only neutrophil count was independently associated with CKD progression (the highest group vs. the lowest group, hazard ratio 2.293 [95% confidence interval 1.260, 4.171]) after fully adjusting for potential confounders. The cumulative incidence of CKD progression in patients with diabetes gradually increased with increasing neutrophil count (53 (7.7%) subjects in the lowest group vs. 60 (8.2%) in the middle group vs. 78 (12.2%) in the highest group).ConclusionsThis study suggested that neutrophil count is an independent risk factor for progression of CKD in patients with diabetes.  相似文献

目的 探讨宫颈环形电切术(LEEP)联合重组人干扰素α-2b栓治疗宫颈上皮内瘤变(CIN)的临床效果.方法 选择经液基细胞学检查(TCT)、宫颈活检病理检查明确诊断为CIN Ⅰ~Ⅲ级患者82例,均于LEEP治疗前应用导流杂交法检测21种亚型人乳头瘤病毒(HPV DNA),均行LEEP治疗,观察组(41例)为LEEP术后加用重组人干扰素α-2b栓治疗3个疗程患者,分别于治疗后6、12个月行TCT及HPV DNA检测以判断两组的疗效.结果 观察组术后6个月治愈率90.2%,术后12个月治愈率100.0%;对照组术后6个月治愈率43.9%,术后12个月治愈率61.0%;两组6、12个月治愈率差异均有统计学意义(x2=19.93、19.89,均P<0.05).结论 宫颈环形电切术联合重组人干扰素α-2b栓治疗CIN疗效优于单纯宫颈环形电切术,提高HPV清除率.  相似文献

The nucleoside analogue cordycepin (3 '-deoxyadenosine, 3 '-dA), one of the components of Cordyceps militaris, has been shown to inhibit the growth of various tumor cells. However, the probable mechanism is still obscure. In this study, the inhibition of cell growth and changes in protein expression induced by cordycepin were investigated in BEL-7402 cells. Using the MTT assay and flow cytometry, we found that cordycepin inhibits cell viability and induces apoptosis in BEL-7402 cells. Additionally, the proteins were separated using two-dimensional polyacrylamide gel electrophoresis, and eight proteins were found to be significantly affected by cordycepin compared to untreated control; among them, two were downregulated and six were upregulated. Of the eight proteins, six were identified with peptide mass fingerprinting using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS) after in-gel trypsin digestion. These proteins are involved in various aspects of cellular metabolism. It is suggested that the effect of cordycepin on the growth of tumor cells is significantly related to the metabolism-associated protein expression induced by cordycepin.  相似文献

目的 研究精神分裂症患者采用阿立哌唑和喹硫平治疗的临床效果及安全性.方法 临床纳入84例本院2013年9月至2015年4月期间收治的精神分裂症患者,根据床位单双号分为两组,各42例;其中42例患者采用阿立哌唑治疗(阿立哌唑组),另42例患者采用喹硫平治疗(喹硫平组);观察两组患者治疗效果、生活质量以及不良反应情况.结果 阿立哌唑组和喹硫平组治疗总有效率分别为69.05%、64.29%,两组对比差异无统计学意义(P>0.05);治疗前两组患者各生活质量领域评分差异无统计学意义(P>0.05);治疗后阿立哌唑组患者生理、心理、环境、社会关系、独立性以及生活质量领域评分明显高于喹硫平组(均P< 0.05);阿立哌唑组、喹硫平组不良反应发生率分别为33.33%、28.57%,两组对比差异无统计学意义(P>0.05).结论 阿立哌唑与喹硫平在治疗精神分裂症的效果及不良反应上差异无统计学意义,但阿立哌唑治疗的患者在生活质量上明显高于喹硫平治疗患者,临床上应视患者具体情况给药.  相似文献
