Objective assessment of trauma severity in patients with combined injuries

The work presents an analysis of using scales VPH-P(MT) and VPH-P(SP) on the basis of a retrospective investigation of results of treatment of 268 patients with a combined injury of the abdomen. A conclusion is made that these scales have reserves for an improvement. The authors propose to supplement the scale VPH-P(SP) with the index of the patient's age, and the scale VPH-P(MT)--with an index of the blood loss volume.     

Dyslipoproteinemia and prognosis of the course of ischemic heart disease after coronary bypass

The article deals with the observation of 88 patients with ischemic heart disease, 62 of them had coexistent dyslipoproteidemia. More pronounced atherosclerotic lesions of the coronary artery were found to develop in patients with dyslipoproteidemia influencing the degree of ischemic heart disease before coronary bypass. Within the period of 10 years after operation dyslipoproteidemia patients have 3.5 times higher risk of the development of myocardial infarction, more than 2 times higher risk of reappearance of stenocardia and death from cardiovascular diseases. Patients with dyslipoproteidemia have 2 times higher probability of generalization of the atherosclerotic process in patients with is chemic heart disease at later terms after coronary bypass.     

Contents and methods of training of foreign students

By analyzing their own experience in training foreign students, the authors concluded that University education in phthisiology may be improved by increasing a role of independent work by using the special text-book prepared by the Department of Tuberculosis, Kursk State Medical University.     

Morphology and various problems of the pathogenesis of alcoholic microangiopathy

Alcoholic microangiopathy was studied on the biopsy material from kidneys, skin and salivary glands of alcoholic patients by means of histochemical, immunohistochemical and electron-microscopic methods. Its formation is mainly caused by the direct action of ethanol on microcirculatory vessels. Alcohol-induced microangiopathy is morphologically manifested by the accumulation of intermediate filaments in the cytoplasm of endotheliocytes and perivascular infiltrate macrophages. The expression of pathological changes in the cytoskeleton is reduced due to the abstinence from alcohol. Glomerulonephritis in alcoholic patients, that can be considered as a peculiar manifestation of alcoholic microangiopathy, is immunocomplex with predominance in these immune complexes of IgA in combination with IgG and C3 complement fraction. Morphologically it is characterized by the accumulation of intermediate filaments in the cytoplasm of podocytes, mesangiocytes, nephrocytes and some endothelial cells, as well as early and quick formation of fibroplastic transformation and cirrhosis of the kidney.
