A probe assembly for simultaneous transesophageal echocardiography and transesophageal cardioversion has been developed. This probe allows cardioversion with the delivery of much lower energy than the standard external approach. Details of the probe construction and its use are described, as is the prospect for future practice. The use of a combined probe may be the technique of choice for patients who require both cardioversion and transesophageal echocardiography.  相似文献

One of the potentially troublesome sequelae of limb amputations is the development of stump neuromas at the severed ends of major nerves. The ability to define them and to distinguish them from other causes of stump pain is of considerable clinical significance. Computed tomography was performed on ten lower limb amputees with stump pain. Five patients had neuromas that were manifest as focal or generalized alteration in the caliber, size, or contour of the nerve trunk in the affected stump. The remaining five patients each had an abnormality detected; these abnormalities included heterotopic bone formation, popliteal artery aneurysm, lipoma, scar tissue, and abscess in the contralateral limb.  相似文献

Endovascular obliteration of hypervascular lesions of the head and neck has become clinically accepted, but it may cause stroke and peripheral cranial nerve palsy. By using a flow-controlled technique to deliver the materials and by knowing the vascular anatomy of the cranial nerves, these problems are less likely to occur. Occasionally, though, vascular anatomy is distorted by the lesion or is anomalous in its distribution. A provocative test of lidocaine injected into the appropriate artery seems to offer a functional test of whether the capillary bed will tolerate small-particle or liquid plastic occlusion. Twenty-six patients had various branches of their external carotid arteries challenged with lidocaine. Three developed transient palsies, and their treatments were modified. None of the 26 patients developed a complication of embolization.  相似文献

We compared self-adhesive, dual-function monitor/defibrillation electrode pads to standard chest monitoring leads and hand-held electrode paddles in the management of prehospital ventricular fibrillation in a single urban paramedic service. Shocks were delivered more quickly following paramedic arrival with self-adhesive pads than with hand-held paddles (1.6 vs 2.5 min; P less than .001). Ventricular fibrillation was terminated more frequently when shocks were delivered using the self-adhesive pads (55 of 58 patients, 95%) than when shocks were delivered using hand-held paddles (49 of 69 patients, 71%; P less than .005). Initial shocks delivered with self-adhesive pads were especially effective, converting 40 of 58 (69%) patients to an organized rhythm on the first or second shock; this was true of only 24 of 69 (35%) patients shocked with hand-held paddles (P less than .001). Patient survival to hospital admission improved when self-adhesive pads were used: 30 of the 58 (52%) patients shocked with self-adhesive pads achieved hospital admission, while only 21 of 69 patients (30%; P less than .025) survived to admission when hand-held paddles were used. In addition, electrical artifact that interfered with accurate rhythm interpretation was far more prevalent when standard monitoring electrodes were used, including artifact that resulted in inappropriate shock delivery (23% of patients monitored with standard electrodes vs 3% of patients monitored with self-adhesive pads; P less than .005). Self-adhesive monitor/defibrillation pads are superior to standard monitoring leads and hand-held electrode paddles in the management of prehospital ventricular fibrillation.  相似文献

Summary A 30-year-old male had a headache for one month and was evaluated with both computed tomography (CT) and magnetic resonance (MR). These scans demonstrated an obstructing pineal cyst containing layered acute and subacute blood products by MR criteria. The concurrent scans allowed correlation between CT and MR findings in this rare complication of an unusual entity, explained his headache (and the development of later upward gaze paresis), provided a precise surgical/anatomic approach, and gave a good final clinical result. The report illustrates appropriate CT and MR images and pathological specimen.The work reported herein was performed under the Navy Clinical Investigation Program, case report no. 84-16-1968-62. Real-time ultrasound backscatter imaging is a new method of evaluating relative integrated backscatter in a clinically applicable manner. The potential clinical utility of real-time backscatter imaging of diseased tissue depends on recognition of normal variations in cyclic backscatter when measured from different echocardiographic image orientations. The view dependence of cyclic backscatter variation was studied in normal human volunteers. In twenty normal male subjects (mean age 28 +/- 5 years) cyclic variation in integrated backscatter (diastolic minus systolic backscatter) was assessed in multiple left ventricular regions with four standard two-dimensional echocardiographic views (parasternal long-axis and short-axis views, and apical two-chamber and four-chamber views). M-mode backscatter imaging was performed from the standard parasternal long-axis view. Cyclic variation in backscatter was present in the septum only when imaged from the parasternal long-axis view (2.7 +/- 3.1 [standard deviation] decibels [dB], p less than 0.01 for diastole versus systole). The posterior wall of the left ventricle demonstrated cyclic variation of integrated backscatter when imaged from both the parasternal long-axis (4.6 +/- 1.6 dB, p less than 0.01) and short-axis views (2.8 +/- 2.2 dB, p less than 0.01). Cyclic variation in integrated backscatter was not demonstrated in inferoseptal, septal, or lateral wall regions when imaged from the parasternal short-axis view. Objectives

To evaluate the use of shear wave elastography in assessment of kidney allograft tubulointerstitial fibrosis.

Methods

Shear wave elastography assessment was carried out by two independent operators in kidney transplant recipients who underwent allograft biopsy for clinical indications (i.e. rising creatinine >15% or proteinuria >1 g/day). Allograft biopsies were interpreted by the same pathologist according to the 2013 Banff Classification.

Results

A total of 40 elastography scans were carried out (median creatinine 172.5 μmol/L [interquartile range 133.8–281.8 μmol/L]). Median tissue stiffness at the cortex (22.6 kPa [interquartile range 18.8–25.7 kPa] vs 22.3 kPa [interquartile range 19.0–26.5 kPa], P = 0.70) and medulla (15.0 kPa [interquartile range 13.7–18.0 kPa] vs 15.6 kPa [interquartile range 14.4–18.2 kPa]) showed no significant differences between the two observers. Interobserver agreement was satisfactory (intraclass correlation coefficient of the cortex 0.84, 95% CI 0.70–0.92 and intraclass correlation coefficient of the medulla 0.88, 95% CI 0.78–0.94). The areas under the receiver operating characteristic curves for detection of tubulointerstitial fibrosis were estimated to be 0.75 (95% CI 0.61–0.89), 0.85 (95% CI 0.75–0.95) and 0.65 (95% CI 0.53–0.78) for cortical, medullary tissue stiffness and serum creatinine, respectively.

Conclusions

Shear wave elastography can be used as a non‐invasive tool to evaluate kidney allograft fibrosis with reasonable interobserver agreement and superior test performance to serum creatinine in detecting early tubulointerstitial fibrosis.  相似文献

Triphasic shocks have been evaluated for endocardial defibrillation but not for open-chest epicardial defibrillation. The purpose of this study was to compare the efficacy and safety of biphasic versus triphasic shocks for epicardial defibrillation in a porcine model. Twenty-two adult swine (18-28 kg) were deeply anesthetized and intubated. After 30 seconds electrically induced VF, each pig received truncated exponential biphasic (7.2-ms positive pulse duration and 7.2-ms negative pulse duration, total waveform duration 14.4 ms) and triphasic (4.8/4.8/4.8 ms, total waveform duration 14.4 ms) epicardial shocks. Pigs in group 1 (n = 11) received epicardial biphasic and triphasic shocks from large hand held paddle electrodes (44.2 cm2); pigs in group 2 (n = 11) received shocks from small paddle electrodes (15.9 cm2). Shocks were given at five selected energy levels (3-30 J) in random sequence. Four shocks were delivered at each energy level to construct an energy versus percentage of success curve. In group 1 (large paddle electrodes), percentage of shock success was significantly higher for triphasic shocks at the energy levels of 3, 5, 10, and 20 J compared to biphasic shocks. In group 2 (small paddle electrodes), triphasic shocks yielded a significantly higher percentage of shock success than biphasic shocks at the energy levels of 5, 10, and 20 J). Shock induced ventricular tachycardia was similar for both waveforms; asystole was rare. For open-chest defibrillation, triphasic waveform shocks were superior to biphasic waveform shocks for VF termination at energy levels of 3-20 J and were as safe as biphasic shocks.  相似文献

Dasatinib is a potent tyrosine kinase inhibitor that is used to treat chronic myeloid leukemia in patients resistant or intolerant to imatinib mesylate. While designed to inhibit Abl and Src kinases, dasatinib shows multitarget effects, including inhibition of the macrophage colony‐stimulating factor (M‐CSF) receptor c‐fms. We have shown previously that dasatinib abrogates osteoclast formation and activity in vitro owing, in part, to its specificity for c‐fms. In this study we examined whether dasatinib could significantly alter bone volume in a model of physiologic bone turnover. Sprague‐Dawley rats were administered dasatinib (5 mg/kg/day) or vehicle by gavage or zoledronic acid (ZOL; 100 µg/kg/6 weeks) subcutaneously. Following 4, 8, and 12 weeks of treatment, serum biochemical, bone morphometric, and histologic analyses were performed. Whole‐body bone mineral density and tibial cortical thickness where unchanged in the dasatinib‐ or ZOL‐treated animals relative to controls. However, micro–computed tomographic (µCT) analysis of cancellous bone at the proximal tibias showed that trabecular volume (BV/TV) and thickness (Tb.Th) were increased in dasatinib‐treated animals at levels comparable with those of the ZOL‐treated group. These changes were associated with a decrease in osteoclast numbers (N.Oc/B.Pm) and surface (Oc.S/BS) and decreased serum levels of the osteoclast marker c‐terminal collagen crosslinks (CTX‐1). Mineral apposition rate (MAR), bone‐formation rate (BFR), and levels of the serum osteoblast markers osteocalcin and N‐terminal propeptide of type I procollagen (P1NP) were not altered significantly in the dasatinib‐treated animals relative to controls. These studies show that dasatinib increases trabecular bone volume at least in part by inhibiting osteoclast activity, suggesting that dasatinib therapy may result in dysregulated bone remodeling. © 2010 American Society for Bone and Mineral Research  相似文献
