Abstract: Serum amino grams and daily losses of glutamine (Gin) and other amino acids (AAs) into diafiltrate were measured during the first 5 days of continuous venovenous hemodiafiltration (CVVHDF) in 6 ICU patients with acute renal failure (ARF). Four patients had ARF as a part of multiple organ failure (MOF) of septic origin, and 2 patients had isolated ARF because of primary renal disease. During the study, all the patients received defined total parenteral nutrition (TPN). The mean daily AA losses into dialysate were relatively low (0.61 ± 0.1 g N ) and reached 4.5% of the daily AA substitution. Gln represented 32.7 ± 5.9% of the total AA losses (0.19 ± 0.04 g N ). Serum levels of Gin (p = 0.002) and of most other AAs were significantly lower in the patients than in the control subjects (AA analysis in 16 healthy volunteers). Phenylalanine (Phe) was the only AA that was increased significantly (p < 0.01) in the patients. The mean patient serum concentrations of Phe and tyrosine were significantly higher (p < 0.03) than the correspondent concentrations in dialysate, but the lysine concentration was higher in dialysate (p < 0.03). The serum and dialysate concentrations of other AAs did not differ. Gin in serum decreased significantly (p < 0.03) on the second day of CVVHDF but returned to the baseline levels subsequently. Serum concentrations of Phe increased on the second day of CVVHDF (p < 0.05). Serum concentrations of other AAs remained stable during the whole study. We conclude that Gin losses into dialysate during CVVHDF are relatively low, but CVVHDF itself may induce changes in Gin metabolism and distribution that are reflected by a decrease of serum Gin levels at the institution of this treatment. Energy expenditure in leisure-time physical activity (LTPA) was measured using the Minnesota LTPA Questionnaire in 35- to 74-year-old black and white residents of Minneapolis-St. Paul, Minnesota. Estimates of the geometric mean LTPA energy expenditure were 129 and 204 kcal per day for black and white men (p<.05) and 91 and 123 kcal per day for black and white women (p<.05). The percentage of individuals expending 2000 kcal or more per week in LTPA was significantly lower in black men than white men (25 vs. 35%; p=.01) but was not different in black versus white women (18 vs. 17%). Although black men and women reported greater occupational physical activity than their white counterparts, LTPA and job activity were unrelated in all race and sex groups. In both races, LTPA energy expenditure declined with age. LTPA increased with level of formal education, and the largest LTPA difference between blacks and whites was observed in those who had a high-school diploma or less. Blacks had lower participation rates than whites in most of the individually assessed physical activities. The present experiment studied whether a dysfunction of the noradrenergic neurons is related to spatial learning impairment by investigating the levels of noradrenaline in the brain and periphery as well as the acquisition of water maze task in saline-pretreated young rats, in noradrenergic neurotoxin (DSP-4)-pretreated young rats and in saline-pretreated aged rats. Aged rats, which had an increased escape latency onto the hidden platform, revealed a decreased noradrenaline content in the heart, but not in the hippocampus, striatum, or hypothalamus, whereas DSP-4-pretreated rats had decreased noradrenaline content in the brain; the acquisition of water maze task was not impaired. These results suggest that the peripheral noradrenergic system can show age-related changes different from those in the central noradrenergic system, and they failed to provide support for the hypothesis that decreased activity of the central noradrenergic nerves is related to impairment in the acquisition of the water maze task. 相似文献

A case-control study with matched pairs was initiated to investigate the relationship between shoulder-neck complaints and activity in the upper trapezius muscle. The matching was done so that the physical demands from work (external exposure) were equal for both the case and the control. Each pair was also matched for gender, age, working hours, and employment time. Male (n = 18) and female workers (n = 78) employed in both manual and office work were included. Muscle activation levels and pause patterns during work and muscle activity during tests of attention, coordination, and rest were recorded by surface electromyography. The results showed consistent associations between pain and signs of increased activation of the upper trapezius for the cases in the manual group. No such associations were observed in the office group. The results are consistent with the hypothesis that muscle activation patterns may in some instances, but not in all, explain why some workers develop pain while others do not in work situations where the physical demands are similar. 相似文献

Growth retardation is a major complication in children with uremia. Protein restriction, calorie deficit, metabolic acidosis,
renal osteodystrophy, and endocrinologic disturbances contribute to the growth failure. The effect of these factors on growth
retardation can be attenuated in part by therapy with vitamin D metabolites, adequate nutrition, alkalization, and dialysis.
Linear growth in children with uremia is markedly retarded despite normal or increased levels of circulating serum growth
hormone. An increased growth hormone level in children with uremia is due to normal growth hormone secretion from the pituitary
gland and impaired growth hormone clearance in the kidney. However, the elevated growth hormone level does not lead to a commensurate
rise in serum insulin-like growth factor I (IGF-I); the serum IGF-I level is decreased or normal in relation to the degree
of renal failure. This discrepancy suggests growth hormone resistance in the liver in uremia. Recent molecular techniques
open a new era in studying the gene expression for growth hormone or IGF-I. There is no doubt today that growth hormone treatment
has the beneficial effect of growth promotion in children with uremia, which also suggests endogenous growth hormone resistance
