Inhibitors of voltage-gated sodium channels (Nav) have been used as anticonvulsants since the 1940s, while potassium channel activators have only been investigated more recently. We here describe the discovery of 2-amino-6-trifluoromethylthio-benzothiazole (SKA-19), a thioanalog of riluzole, as a potent, novel anticonvulsant, which combines the two mechanisms. SKA-19 is a use-dependent NaV channel blocker and an activator of small-conductance Ca2+-activated K+ channels. SKA-19 reduces action potential firing and increases medium afterhyperpolarization in CA1 pyramidal neurons in hippocampal slices. SKA-19 is orally bioavailable and shows activity in a broad range of rodent seizure models. SKA-19 protects against maximal electroshock-induced seizures in both rats (ED50 1.6 mg/kg i.p.; 2.3 mg/kg p.o.) and mice (ED50 4.3 mg/kg p.o.), and is also effective in the 6-Hz model in mice (ED50 12.2 mg/kg), Frings audiogenic seizure-susceptible mice (ED50 2.2 mg/kg), and the hippocampal kindled rat model of complex partial seizures (ED50 5.5 mg/kg). Toxicity tests for abnormal neurological status revealed a therapeutic index (TD50/ED50) of 6–9 following intraperitoneal and of 33 following oral administration. SKA-19 further reduced acute pain in the formalin pain model and raised allodynic threshold in a sciatic nerve ligation model. The anticonvulsant profile of SKA-19 is comparable to riluzole, which similarly affects NaV and KCa2 channels, except that SKA-19 has a ~4-fold greater duration of action owing to more prolonged brain levels. Based on these findings we propose that compounds combining KCa2 channel-activating and Nav channel-blocking activity exert broad-spectrum anticonvulsant and analgesic effects.

Aims  To investigate heroin and cocaine use in a sample of British prisoners, and to explore the characteristics of inmates who use these drugs for the first time while in prison.
Design, participants  A cross-sectional survey of all prisons in England and Wales conducted as part of a major national study of psychiatric morbidity. A total of 3142 prisoners (88.2% of those selected) completed a structured interviewer-administered questionnaire.
Measurements  Interview measures of personal demographics, social history, psychiatric morbidity and drug use. Personality disorders were diagnosed via the Structured Clinical Interview for DSM-IV (SCID-II) and neurotic symptoms were assessed using the revised Clinical Interview Schedule (CIS-R).
Findings  More than 60% of the heroin users and cannabis users reported that they had used these drugs in prison compared with less than a quarter of the life-time cocaine users. More than a quarter of the heroin users reported that they had initiated use of this drug in prison. The extent of an individual's experience of prison was related more consistently to heroin and/or cocaine use in and out of prison than other personal background, social history or psychiatric variables assessed.
Conclusions  The findings indicate that prisons are a high-risk environment for heroin and other drug initiation and use. Although related to drug use, psychiatric variables were not generally associated with initiation in prison, which was dominated by prison exposure. There is a need to explore ways of reducing heroin initiation in prison as part of a broader risk-prevention strategy.  相似文献

As one of the earliest-known mammaliaforms, Haramiyavia clemmenseni from the Rhaetic (Late Triassic) of East Greenland has held an important place in understanding the timing of the earliest radiation of the group. Reanalysis of the type specimen using high-resolution computed tomography (CT) has revealed new details, such as the presence of the dentary condyle of the mammalian jaw hinge and the postdentary trough for mandibular attachment of the middle ear—a transitional condition of the predecessors to crown Mammalia. Our tests of competing phylogenetic hypotheses with these new data show that Late Triassic haramiyids are a separate clade from multituberculate mammals and are excluded from the Mammalia. Consequently, hypotheses of a Late Triassic diversification of the Mammalia that depend on multituberculate affinities of haramiyidans are rejected. Scanning electron microscopy study of tooth-wear facets and kinematic functional simulation of occlusion with virtual 3D models from CT scans confirm that Haramiyavia had a major orthal occlusion with the tallest lingual cusp of the lower molars occluding into the lingual embrasure of the upper molars, followed by a short palinal movement along the cusp rows alternating between upper and lower molars. This movement differs from the minimal orthal but extensive palinal occlusal movement of multituberculate mammals, which previously were regarded as relatives of haramiyidans. The disparity of tooth morphology and the diversity of dental functions of haramiyids and their contemporary mammaliaforms suggest that dietary diversification is a major factor in the earliest mammaliaform evolution.Haramiyidans are among the first mammaliaforms to appear during the Late Triassic in the evolutionary transition from premammalian cynodonts. Their fossils have a cosmopolitan distribution during the Late Triassic to the Jurassic (18), tentatively with the youngest record in the Late Cretaceous of India (9). Most of these occurrences are of isolated teeth. For this reason, Haramiyavia clemmenseni (1) holds a special place in mammaliaform phylogeny: It is the best-preserved Late Triassic haramiyid with intact molars, nearly complete mandibles, and also postcranial skeletal elements (Figs. 1 and and22 and SI Appendix, Figs. S1–S4) (1). By its stratigraphic provenance from the Tait Bjerg Beds of the Fleming Fjord Formation, East Greenland (Norian-Rhaetic Age) (7), Haramiyavia is also the oldest known haramiyid (5, 7). The cusp rows occlude alternately between the upper and lower molars. Primarily because of similarities in molar morphology, haramiyids are considered to be related to poorly known theroteinids of the Late Triassic (5, 13) and eleutherodontids of the Middle to Late Jurassic (1417). Collectively haramiyids and eleutherodontids are referred to as “haramiyidans” (10, 14, 15, 18, 19). Recent discoveries of diverse eleutherodontids or eleutherodontid-related forms with skeletons from the Tiaojishan Formation (Middle to Late Jurassic) of China (1820) have greatly augmented the fossil record of haramiyidans, ranking them among the most diverse mammaliaform clades of the Late Triassic and Jurassic.Historically, it has been a contentious issue whether haramiyidans (later expanded to include theroteinids and eleutherodontids) are closely related to the more derived multituberculates from the Middle Jurassic to Eocene (13) or represent a stem clade of mammaliaforms excluded from crown mammals (21, 22). The conflicting placement of haramiyidans was attributable in part to the uncertainties in interpreting the isolated teeth of most Late Triassic haramiyids (21, 22). More recent phylogenetic disagreements have resulted from different interpretations of mandibular characters in Haramiyavia (1720, 2325), which has not been fully described (figure 2 in ref. 1).Here we present a detailed study of the mandibles and teeth of Haramiyavia from the exhaustive documentation during initial fossil preparation (Fig. 1 and SI Appendix, Figs. S1–S4), from scanning electron microscopy (SEM) images, and from computed tomography (CT) scans and 3D image analyses of the two fossil slabs with mandibles (MCZ7/95A and B), plus a referred specimen of upper molars in a maxilla (MCZ10/G95) (Figs. 2 and and3,3, SI Appendix, Figs. S5–S8 and Tables S2 and S3, and Movie S1). This study compares the utility of two functional assays for von Willebrand factor (VWF), the ristocetin cofactor assay (VWF:RCo) and the collagen-binding assay (VWF:CBA). We analysed a group of 32 patients with type 2 von Willebrand disease (VWD) (25 patients with type 2M, six with type 2A and one with type 2B) and 22 normal control subjects. VWF:RCo/VWF antigen (VWF:Ag) ratios and VWF:CBA/VWF:Ag ratios were compared between the patient and control groups. In the six patients with type 2A VWD, both VWF:RCo/VWF:Ag ratios and VWF:CBA/VWF:Ag ratios were discordant (< or = 0.7). In the 25 type 2M VWD patients, the VWF:CBA/VWF:Ag ratios were concordant (> 0.7), but the VWF:RCo/VWF:CBA ratios were discordant (< or = 0.7) (P = 0.001) compared with control subjects. Thus, VWF:RCo/VWF:Ag ratios were discordant in both type 2M and 2A VWD patient groups indicating a functional abnormality. However, VWF:CBA/VWF:Ag ratios were discordant in the type 2A VWD group but not in the type 2M VWD group. Our study showed that VWF:CBA is sensitive to functional variants associated with the loss of high-molecular-weight multimers, i.e. type 2A and 2B in VWD, but the assay was unable to discriminate defective platelet-binding VWD variants with normal multimeric patterns such as type 2M VWD. It was concluded that the VWF:CBA assay should be used in association with rather than as a replacement for the VWF:RCo assay.  相似文献
