Background After neoadjuvant chemotherapy, women with locally advanced breast cancer (LABC) undergo a modified radical mastectomy or
lumpectomy with axillary lymph node dissection (ALND) and radiotherapy. Sentinel lymphadenectomy (SL) is accepted for axillary
evaluation in early breast cancer. We assessed the feasibility and predictive value of SL after neoadjuvant chemotherapy.
Methods Eligible women received neoadjuvant therapy for LABC and were scheduled to undergo a definitive surgical procedure. Vital
blue dye SL was attempted followed by level I and II axillary dissection.
Results SL was successful in 29 of 34 patients (detection rate, 85%). Thirteen patients (45%) had positive nodes, and eight (28%)
had negative nodes on both SL and ALND. In five patients (17%), the sentinel node was the only positive node identified. Overall,
there was a 90% concordance between SL and ALND. The false-negative rate and negative predictive value were 14% and 73%, respectively.
Among the subgroup without inflammatory cancer, the detection and concordance rates were 89% and 96%, respectively. The false-negative
rate was 6%, and the negative predictive value was 88%.
Conclusions SL after neoadjuvant chemotherapy may reliably predict axillary staging except in inflammatory breast cancer. Further studies
are required to assess the utility of SL as the only mode of axillary evaluation in these women. 相似文献

AIM: To evaluate Quality of life(QoL) in chronic heart failure(CHF) in relation to Neuroticism personality trait and CHF severity.METHODS: Thirty six consecutive, outpatients with Chronic Heart Failure(6 females and 30 males, mean age: 54 ± 12 years), with a left ventricular ejection fraction ≤ 45% at optimal medical treatment at the time of inclusion, were asked to answer the Kansas City Cardiomyopathy Questionnaire(KCCQ) for Quality ofLife assessment and the NEO Five-Factor Personality Inventory for personality assessment. All patients un-derwent a symptom limited cardiopulmonary exercise testing on a cycle-ergometer, in order to access CHF severity. A multivariate linear regression analysis us-ing simultaneous entry of predictors was performed to examine which of the CHF variables and of the person-ality variables were correlated independently to QoL scores in the two summary scales of the KCCQ, namely the Overall Summary Scale and the Clinical Summary Scale.RESULTS: The Neuroticism personality trait score had a significant inverse correlation with the Clinical Sum-mary Score and Overall Summary Score of the KCCQ(r =-0.621, P 0.05 and r =-0.543, P 0.001, respec-tively). KCCQ summary scales did not show significant correlations with the personality traits of Extraversion, Openness, Conscientiousness and Agreeableness. Mul-tivariate linear regression analysis using simultaneous entry of predictors was also conducted to determine the best linear combination of statistically significant univari-ate predictors such as Neuroticism, VE/VCO2 slope and VO2 peak, for predicting KCCQ Clinical Summary Score. The results show Neuroticism(β =-0.37, P 0.05), VE/VCO2 slope(β =-0.31, P 0.05) and VO2 peak(β = 0.37, P 0.05) to be independent predictors of QoL. In multivariate regression analysis Neuroticism(b =-0.37, P 0.05), the slope of ventilatory equivalent for carbon dioxide output during exercise,(VE/VCO2 slope)(b =-0.31, P 0.05) and peak oxygen uptake(VO2 peak),(b = 0.37, P 0.05) were independent predictors of QoL(adjusted R2 = 0.64; F = 18.89, P 0.001).CONCLUSION: Neuroticism is independently associat-ed with QoL in CHF. Pumping blood in one direction is the main function of the heart, which is equipped with valves that ensure unidirectional flow. During early embryonic life, blood circulates in one direction through the primitive circulatory system, in spite of the complete lack of valves. A mathematical model has been developed in order to describe the coordinated fashion of the blood circulation in the circulatory system of the embryo. The blood flow is considered as one-dimensional, employing the momentum and continuity equations. Also a constitutive state equation is used relating pressure and circuit lumen. The one-dimensional model is analysed numerically and solved with a dispersion-relation-preserving (DRP) scheme of finite difference, which is of fourth order accurate. The size of the mean flow depends on the embryonic heart rate and the contraction grade of primordial heart tube. Three functions in time and space are calculated, the cross section area of the lumen, the building up internal pressure and the velocity of the blood inside the circuit. The pumping activity of the embryo circulatory system is shown by presentation of the mean flow-rate of the blood as a function of embryonic heart rate for several contraction grades of primordial heart tube. 相似文献

Despite the well-known beneficial effects of the intra-aortic balloon pump (IABP) generally, there are still some clinical conditions accompanied by IABP ineffectiveness. The aim of this study was the investigation of the independent effects of arterial stiffness and blood pressure on acute IABP effectiveness. For this purpose, a mock circulatory system and 20 patients with cardiogenic shock due to acute myocardial infarction, were employed. It was shown that IABP acute efficiency was determined primarily by arterial compliance (AC) rather than blood pressure alone. IABP induced low hemodynamic effects in patients with systolic blood pressure > 80 mm Hg but with increased AC, whereas IABP resulted in greater hemodynamic effectiveness in cases with systolic pressure < 70 mm Hg but lower AC. The present study provides evidence concerning the hemodynamic conditions, which might lead to optimization of IABP or to the prediction of its acute hemodynamic performance, based on both measurements of AC and blood pressure. 相似文献

OBJECTIVE: Discrete morphologic, enzymatic and functional changes in skeletal muscle mitochondria have been demonstrated in patients with peripheral arterial disease (PAD). We examined mitochondrial respiration in the gastrocnemius muscle of nine patients (10 legs) with advanced PAD and in nine control patients (nine legs) without evidence of PAD. METHODS: Mitochondrial respiratory rates were determined with a Clark electrode in an oxygraph cell containing saponin-skinned muscle bundles. Muscle samples were obtained from the anteromedial aspect of the gastrocnemius muscle, at a level 10 cm distal to the tibial tuberosity. Mitochondria respiratory rate, calculated as nanoatoms of oxygen consumed per minute per milligram of noncollagen protein, were measured at baseline (V(0)), after addition of substrates (malate and glutamate; (V(SUB)), after addition of adenosine diphosphate (ADP) (V(ADP)), and finally, after adenine nucleotide translocase inhibition with atractyloside (V(AT)). The acceptor control ratio, a sensitive indicator of overall mitochondrial function, was calculated as the ratio of the respiratory rate after the addition of ADP to the respiratory rate after adenine nucleotide translocase inhibition with atractyloside (V(ADP)/ V(AT)). RESULTS: Respiratory rate in muscle mitochondria from patients with PAD were not significantly different from control values at baseline (0.31 +/- 0.06 vs 0.55 +/- 0.12; P =.09), but V(sub) was significantly lower in patients with PAD compared with control subjects (0.43 +/- 0.07 vs 0.89 +/- 0.20; P <.05), as was V(ADP) (0.69 +/- 0.13 vs 1.24 +/- 0.20; P <.05). Respiratory rates after atractyloside inhibition in patients with PAD were no different from those in control patients (0.47 +/- 0.07 vs 0.45 +/- P =.08). Compared with control values, mitochondria from patients with PAD had a significantly lower acceptor control ratio (1.41 +/- 0.10 vs 2.90 +/- 0.20; P <.001). CONCLUSION: Mitochondrial respiratory activity is abnormal in lower extremity skeletal muscle in patients with PAD. When considered in concert with the ultrastructural and enzymatic abnormalities previously documented in mitochondria of chronically ischemic muscle, these data support the concept of defective mitochondrial function as a pathophysiologic component of PAD. 相似文献

Purpose: The aim of this study was to assess the clinical efficacy of distigmine bromide, an anti-cholinesterase agent, deemed to improve detrusor function thereby restoring normal voiding patterns in patients suffering from detrusor underactivity. Materials and methods: A total of 27 patients (11 men and 16 women) with poor detrusor function were included in the study. The diagnosis was established using pressure-flow studies. All patients received distigmine bromide at a dose of 5 mg three times daily for 4 weeks and re-attended for a follow-up urodynamic investigation. The results of baseline pressure-flow studies were compared to those after completion of treatment. Results: Treatment with distigmine bromide resulted in a statistically significant reduction of residual volume and percent residual volume, obviating the need for intermittent self-catheterisation in 11 patients. In addition, maximum flow rate and detrusor pressure at maximum flow increased, although not significantly. The drug was generally well tolerated by the majority of patients. Conclusion: Distigmine bromide shows clinical efficacy in patients with poor detrusor function and may therefore be used alternatively in selected cases. 相似文献
