Andrology: Analysis of sperm movement in relation to the oxidative stress created by leukocytes in washed sperm preparations and seminal plasma

The addition of luminol to unprocessed semen samples resultedin the generation of chemiluminescent signals, the intensityof which was highly correlated with the level of leukocyte contamination.Despite the spontaneous oxidant-generating capacity of seminalleukocytes, no correlations were observed between leukocytecontamination and the fertility status of the subjects or anyaspect of the semen profile, including the motility of the spermatozoaor their performance in a hyaluronate penetration assay. Luminol-dependentchemiluminescence and leukocyte contamination were also correlatedin washed sperm suspensions prepared either by repeated centrifugationor on discontinuous Percoll gradients. However, in such spermsuspensions, the spontaneous generation of oxidants by contaminatingleukocytes (>2x10⁴ leukocytes/ml) was invariably associatedwith a decreased capacity for movement. Moreover, causativeassociations between leukocyte contamination, reactive oxygenspecies generation, lipid peroxidation and impaired sperm motilitywere revealed by experiments involving the selective additionor removal of activated leukocytes. From these observationswe can conclude that low concentrations of leukocytes are acommon feature of the human ejaculate and can impair sperm function,particularly in the absence of seminal plasma. These findingshave implications for our understanding of the importance ofleukocytospermia in defining the fertility of human spermatozoain vivo and in vitro.     

Gut contents: A significant contaminant of Mytilus edulis whole body metal concentrations

Ingested matter can have a significant effect on whole body metal concentration measurements in Mytilus edulis. Depuration of mussels in clean seawater for 36 h prior to dissection eliminates most of these contaminating gut contents. Depuration followed by metal analyses is the most direct method of determining mussel tissue metal bioburdens. After being transplanted into a plume of primary treated sewage effluent in Salem Harbor, Massachusetts for 32 days, Al, Cr, and Fe concentrations in depurated mussels were significantly lower than those determined for either non-depurated mussels or for depurated mussels to which fecal concentrations of Al, Cr, and Fe were added back in. Although mathematical methods developed by both Ouellette (1978) and Boehm et al. (1988) could be applied to non-depurated mussels in order to correct for errors associated with gut metal contamination, these indirect methods were not as reliable as depuration prior to analysis.     

Expression of type-X collagen in osteoarthritis

The present study was undertaken to examine how osteoarthritis affects the expression of type-X collagen, a hypertrophic chondrocyte-specific collagen in articular cartilage. A well characterized sheep polyclonal antiserum, as well as three mouse monoclonal antibodies against canine type-X collagen, was used to immunolocalize type-X collagen in human and canine joints. Its expression in osteoarthritic cartilage was altered in several locations. In the canine osteoarthritic joints, type-X collagen increased in and just above the zone of calcified cartilage and was present diffusely throughout the calcified matrix. In both the human and canine cartilage, type-X collagen was localized around cell clones in the transitional zone of cartilage. This is surprising, since that region of the cartilage does not calcify and one of the proposed roles of type-X collagen is in mineralization. Thus, the osteoarthritic process may damage the matrix in the superficial layer and induce changes leading to the expression of the hypertrophic chondrocyte phenotype.     

Solubility of neurofibrillary tangles and ultrastructure of paired helical filaments in sodium dodecylsulphate

Summary Temporal cortex from 14 cases of Alzheimer-type dementia and 6 cases of Down's syndrome, all selected for severe Alzheimer pathology, was homogenised in distilled water, NaOH, or sodium dodecylsulphate (SDS) containing 0.1% -mercaptoethanol. The homogenates were stained with Congo red, and the neurofibrillary tangles and plaque cores were counted under crossed-polarisation microscopy. The number of tangles and plaque cores in the water-treated extracts was not related to age, sex, postmortem interval or duration of dementia. The number of tangles after extraction in SDS or NaOH, as a percentage of tangles in water-treated extracts, was 57±25 (mean±SD) for 1% SDS, 43±17 for 5% SDS and 37±22 for 0.2 M NaOH. Plaque cores were essentially insoluble in all three agents. The percentage of tangles insoluble in 1% SDS did not correlated with age or post-mortem interval but decreased with increasing duration of dementia. Enhanced tangle solubility with increasing duration of dementia suggests that the nature of tangles changes with time; one possibility is that this reflects transformation of intracellular to extracellular tangles. Paired helical filament (PHF) length and the number of repeats per PHF were measured in electron micrographs of PHF prepared with and without treatment by 1% SDS. There was no significant multimodality of PHF length to suggest that PHF broke at regular intervals. The mean repeat length (PHF length/number of repeats) was greater for PHF isolated in the presence of 1% SDS than in its absence, showing that SDS affects ultrastructure by untwisting PHF. An untwisting process may also occur in vivo producing the straight filaments found, together with PHF, in tangles and neurites.Supported by Miss E. Fixed prosthodontics in the elderly population. Life expectancy of fixed restorations, failures, and retreatment methods.

The restorative needs of older dental patients challenge the ingenuity, anatomic knowledge, artistic skills, occlusal philosophies, and material knowledge of the clinician. Achieving the most secure foundation while simultaneously eliminating imperfections and incorporating a design that promotes good oral hygiene and a natural and attractive appearance are significant contributors to a patient's welfare. The treatment decision regarding fixed prosthodontics for elderly patients requires the balancing of two opposing arguments: 1. In patients who are older, and who are perhaps medically or physically compromised, and, in addition, who may be on a limited budget (or perceived limited budget), it is important to fabricate dental prostheses that are as good as possible to minimize the likelihood that the prosthesis will need to be remade in the future when the patient is likely to be even more compromised financially, medically, or physically, and also to minimize the stress on the patient of accommodating to something that is less than an optimal dental solution. 2. Patients in this age group often anticipate financial strain in the future, perhaps realistically in view of the increasing percent of older adults who are institutionalized (5% of persons 65 years old or older, 20% of persons 80 years old or older). Also, many are reluctant to invest large amounts of money in their teeth when they are already quite elderly and realize they may not live long enough to make the investment "worthwhile." Educating the patient regarding average life expectancy is sometimes helpful, but the experience of many clinical dentists is that many elderly persons either do not believe the numbers, require greater certainty in their "investments," or do not place as high a value on their dental health as they do other aspects of their lives (in a context in which there are more needs than resources to pay for them). Finally, many older adults, contrary to the popular bumper sticker, are trying to preserve as many resources for their children and grandchildren as possible. The final decision should be made with sensitivity to the overall needs of the patient, and with the assistance of a well-informed patient or other responsible party.     

Selective attention and auditory event-related potentials in somatization disorder

Janet suggested a disorder of attention was fundamental to hysteria. This idea was investigated in this study by examining mismatch negativity (MMN), which depends upon a subtraction of cortical potentials evoked by background stimuli from those evoked by target stimuli. Ten patients with somatization disorder (SMD) were compared with ten normal individuals. MMN was smaller in somatizers, particularly at the central recording site. One interpretation of this observation is that somatizers respond more similarly than normals to "relevant" and "irrelevant" stimuli. This finding of an impairment in attentional processing in somatization disorder suggests a subtle neurophysiological disturbance.     

Quality care at risk in CMHCs: The need for Guidelines for Psychiatric Practice

The author discusses what he considers the inadequate attention paid to quality of care in many CMHCs. He suggests that the deterioration of quality is related to the exodus of competent psychiatrists. The Guidelines for Psychiatric Practice in Community Mental Health Centers should help to assure that quality care is delivered in a cost-effective manner. These guidelines should ultimately be incorporated into an accreditation process for CMHCs, and funding should be tied to such accreditation.This paper was presented at the Annual Meeting of the American College of Mental Health Administration, Woodstock, VT, March, 1988.
